Critical evaluation of 62 patients totalizing 63 operations with acute traumatic rupture of knee ligaments in a predominantly rural population. In 21 cases (30%) the lesions have been the result of typical work-injuries. A specific older group of patients was concerned (average age 50.2 years). 75% of these cases presented additional intrarticular lesions at operation. The postoperative evaluation 1-5 years later confirmed the heavy use of the knees in this rural population. We therefore strongly advocate an early primary repair of the ruptured ligaments of the knee especially in work-injuries

A retrospective survey of 135 posterior dislocations and fracture-dislocations of the hip was carried out in order to define the pattern of associated knee injuries. Thirty-five patients had sustained a significant injury to the knee, of which twenty-five were clearly attributable to a direct blow on the front of the knee (fractured patella, traumatic chondromalacia, fractures of femoral and tibial condyles) and ten were compatible with valgus, varus or rotational forces (medial, lateral and cruciate ligament tears). The second type of injury has not been widely recognised but it is important …

BACKGROUND: Significant heterogeneity exists in the patient-reported outcome measures (PROMs) used to assess outcomes in knee osteoarthritis (KOA), with limited evidence exploring the correlation and conversion among the various PROMs. Our study aims to explore the correlation and conversion between the Oxford Knee Score (OKS), Knee Injury and Osteoarthritis Outcome Score (KOOS-12) and Knee Society Score (KSS) in patients with non-operatively treated KOA.
METHODOLOGY: This was a prospective cohort study of 131 patients who received non-operative treatment (standardized physical therapy and an

Knee cartilage injury, a prevalent clinical condition leading to pain and functional impairment, presents significant challenges for complete restoration due to the tissue's limited regenerative capacity. This narrative review examines the paradigm shift towards intelligent treatment strategies encompassing the entire patient journey, from preoperative planning to postoperative rehabilitation. We detail the application of advanced surgical navigation systems (e.g., Holoknee), robotic assistance, and artificial intelligence (AI) in enhancing the precision of diagnostic assessment, surgical exec

PURPOSE: Multiligament knee injuries (MLKIs) are complex and often associated with significant functional impairments. Data on return to sport (RTS) and return to work (RTW) remain limited, particularly for recreational athletes and the general population. The purpose of the study was to evaluate RTS, RTW and physical activity levels after surgical treatment of MLKIs.
METHODS: Patients surgically treated for MLKI at a single Level 1 Trauma centre from 2013 to 2020 were included. Patient-reported outcomes included the Tegner score and work status before and after injury. Physical activity was …

Vascular injury is a feared complication of any surgical procedure. This study examined the incidence of vascular injury during total knee arthroplasty (TKA), the circumstances and timing of injury intraoperatively, and acute management. Eighteen cases of catastrophic vascular injury after primary TKA (12 of 19,577; 0.06%) or revision TKA (6 of 4453; 0.1%) were identified. Catastrophic injury was defined as any vascular injury requiring vascular surgery. Chart review was performed to identify the timing of vascular injury, the injured blood vessel, and acute management. The Knee Society Score

BACKGROUND: The diagnosis of necrotising soft tissue infections (NSTIs, necrotising fasciitis, myositis and cellulitis) may be clinically challenging, and can result in fatal outcomes.
CASE PRESENTATION: A previously healthy woman in her sixties fell and cut her right patella. The wound was complicated by localised infection, which subsequently developed into a bacterial bursitis. She responded to intravenous antibiotics and was followed up at the outpatient clinic. Nineteen days later she was admitted with the same symptoms and clinical presentation as at the previous admission. She was star

The knee joint is one of the most important joints in the human body. The meniscus is a half-moon cartilage which locates between the medial and lateral condyle of the thighbone and the medial and lateral condyle of the shin in the knee joint, and it has multiple functions such as bearing load, cushioning and stabilising joints. It is easy to be injured because of its complex structure and functions. In this study, the physical fitness of patients with meniscus injury was tested using kinematic measurement, and the data were collected for biomechanical analysis on the parameters such as the an

Fat is not just used by the body as bulk tissue. In addition to its role in storing energy and regulating hormone action, fat is used in some parts of the body for its mechanical properties. The anatomy of anterior knee fat is more complex than it appears at first sight and is capable of withstanding considerable compressive and shear stress. Specific lesions occur when such mechanical stress exceeds the physiological limits and are yet little known. Superficial fat can be the site of either acute injury by closed degloving called the Morel-Lavallée lesion or chronic injury, when subject to re

We describe injuries to the posterior root of the medial meniscus in patients with spontaneous osteonecrosis of the medial compartment of the knee. We identified 30 consecutive patients with spontaneous osteonecrosis of the medial femoral condyle. The radiographs and MR imaging were reviewed. We found tears of the posterior root of the medial meniscus in 24 patients (80%). Of these, 15 were complete and nine were partial. Complete tears were associated with > 3 mm of meniscal extrusion. Neither the presence of a root tear nor the volume of the osteonecrotic lesion were associated with age, bod

OBJECTIVE: To study the occurrence rate of meniscus injury in osteoarthritis (OA) of knee joint and inquire into the significance of meniscus injury in diagnosis and treatment of OA.
METHODS: 125 knees of 120 cases with OA as the first diagnosis were observed when debridement was done under arthroscopy. The occurrence rate of meniscus injury and the relationship between the degree of OA and meniscus injury were analyzed. The curative effect of selective joint space debridement with meniscus fitting and resection was evaluated.
RESULTS: Pain was the most common symptom of this group. 32 cases

OBJECTIVE: The objective of this study was to study injury and reconstruction of the anterior cruciate ligament (ACL) and their effects on knee osteoarthritis.
DESIGN: This manuscript discusses the function of knee ligaments, including the basic mechanical properties, the structural properties of their respective bone-ligament-bone complexes, as well as their time- and history-dependent viscoelastic characteristics. The in-situ forces in the ACL and its replacement grafts and knee kinematics before and after ACL reconstruction are also examined.
RESULTS: A robotic/universal force-moment sens

From January, 1979 to May, 1989, 107 patients with problems related to anterior cruciate ligament (ACL) were treated in our hospital. 100 of the patients had anterior cruciate ligament injury confirmed by arthrotomy or arthroscopy. The remaining 7 patients were found to be normal either by arthroscopy or arthrotomy. 29 patients had fresh ACL injury and 71 old. All the patients had history of trauma of the knee joint. Swelling and pain in the affected knee joint took place in fresh cases and few of them complained of instability or deformity of the knee. On examination, floating patella test wa

Magnetic resonance imaging (MRI) is now commonly used to diagnose knee pathology. Based on results in a study conducted in a private practice setting, the diagnostic value of MRI for all articular cartilage surfaces, menisci, ligaments, and soft tissue structures of the knee was determined. Subjects included 102 consecutive patients undergoing arthroscopic knee surgery for whom MRI findings also were available. Sensitivity, specificity, accuracy, positive predictive value (PPV), and negative predictive value (NPV) of MRI were calculated. Sensitivity was highest for meniscus tears: 73.2% for me

Complete dislocation of the knee is a relatively rare condition. When it occurs as a result of high-velocity injury, such as in a road traffic accident, associated vascular injury is generally suspected. In low-velocity injuries, however, distal pulses are often maintained throughout, and the possibility of vascular injury may erroneously be discounted. We report four cases of low-velocity dislocation of the knee, only one of which had an overt vascular disruption, but three of which had arterial damage. On the basis of our experiences, we recommend arteriography in all cases of complete dislo

Swelling of the knee joint occurred in 8 of 35 patients with spinal cord injury; in seven of the eight patients the swelling developed within the first eight months after injury. Six of the eight patients had flaccid paraplegia. None of the patients showed any constitutional or local changes other than swelling. No laboratory evidence of inflammation could be detected. Swelling disappeared in two to six weeks and no sequelae were noted. Although trauma appeared to be the most likely cause of the swelling. Only one of four joints showed hemarthrosis on aspiration.

PURPOSE: The purpose of this study was to determine whether the incidence of posterolateral knee injuries, as well as other concurrent knee ligament injuries, could be obtained from a prospective evaluation of magnetic resonance imaging scans in patients with an acute knee injury with a hemarthrosis.
METHODS: We prospectively documented all patients with acute (within 30 days) knee injuries with a hemarthrosis over a 90-day period who underwent 1.5-T magnetic resonance imaging at a large referral imaging facility with 6 sites, effectively covering a large metropolitan area. The anterior cruci…
