Comparison of maternal mortality and morbidity between trial of labor and elective cesarean section among women with previous cesarean delivery.

OBJECTIVE: This study was undertaken to assess the safety of trial of labor after previous cesarean delivery. STUDY DESIGN: Retrospective cohort study of 308,755 Canadian women with previous cesarean delivery between 1988 and 2000. Occurrences of in-hospital maternal death, uterine rupture, and other severe maternal morbidity were compared between women with a trial of labor and those with an elective cesarean section. RESULTS: Rates of uterine rupture (0.65%), transfusion (0.19%), and hysterectomy (0.10%) were significantly higher in the trial-of-labor group. Maternal in-hospital death rate

Promoting vaginal birth after cesarean section.

New data support the safety of vaginal birth after cesarean section (VBAC). Physicians and hospitals that provide standard obstetric care can also provide care for women attempting VBAC. The management of labor in such patients differs little from that in other patients, and an understanding of the normal course of labor in these patients should help eliminate inappropriate interventions. Family physicians can play a major role in promoting VBAC in appropriately selected patients.

COVID-19 Obstetrics Task Force, Lombardy, Italy: Executive management summary and short report of outcome.

From February 24, 2020, a COVID-19 obstetric task force was structured to deliver management recommendations for obstetric care. From March 1, 2020, six COVID-19 hubs and their spokes were designated. An interim analysis of cases occurring in or transferred to these hubs was performed on March 20, 2020 and recommendations were released on March 24, 2020. The vision of this strict organization was to centralize patients in high-risk maternity centers in order to concentrate human resources and personal protective equipment (PPE), dedicate protected areas of these major hospitals, and centralize

[The mother-placenta-fetus system in maternal thyroid diseases].

In maternal thyroid diseases, the placenta shows an increase in complications during pregnancy. There are signs of lesions as alterations, focal sclerosis, and fibrinoid necrosis. There is also a circulatory disorder as plethora and thrombosis. Progression in these changes correlates with the cases of miscarriage and other pathology in the obstetric history. The concurrence of obstetric and endocrine diseases increases the risk of pregnancy, labor, and delivery complications in different maternal thyroid diseases.

Epidural analgesia for a successful vaginal delivery in a parturient with Jarcho-Levin syndrome: case report.

BACKGROUND: There is a paucity of literature regarding obstetric anesthesia management in Jarcho-Levin Syndrome (JLS) patients and existing case reports are limited to pre-planned cesarean deliveries. CASE PRESENTATION: The patient is a 21-year-old woman with JLS, spondylocostal dysostosis (SCD) subtype, who went through a full-term vaginal delivery with labor neuraxial analgesia. This case report details a successful trial of labor and delivery, which was permitted after an extensive testing of her baseline functional status, as well as a review of the difficulties present in the provision o

Obstetric emergencies and adverse maternal-perinatal outcomes in Ethiopia; A systematic review and meta-analysis.

BACKGROUND: Obstetric emergencies are life-threatening medical problems that develop during pregnancy, labor, or delivery. There are a number of pregnancy-related illnesses and disorders that can endanger both the mother's and the child's health. During active labor and after delivery, obstetrical crises can arise (postpartum). While the vast majority of pregnancies and births proceed without a hitch, all pregnancies are not without risk. Pregnancy can bring joy and excitement, but it can also bring anxiety and concern. Preterm birth, stillbirth, and low birth weight are all adverse pregnancy

Management of the latent phase of labor.

1. Iatrogenic causes of prolonged latent phase such as sedation, narcotic analgesia, and epidural anesthesia should be avoided during the latent phase. 2. Cesarean delivery is not appropriate management solely for failure to progress in latent phase. 3. Multiparas in latent phase with favorable cervices should be treated with oxytocin stimulation. 4. Multiparas with unfavorable cervices and most nulliparas should be treated with therapeutic rest when they become exhausted during a long latent phase, without regard to an arbitrary time table. 5. Characteristics of latent phase in induced labors

An Appraisal of Labor Augmentation: An Update on Current Evidence and Practice.

Augmentation of labor remains the cornerstone of intrapartum care, typically employed to expedite delivery by enhancing the frequency, strength, and duration of uterine contractions. The decision to augment depends on the stage of labor and the woman's obstetric history. While up to half of all women who enter spontaneous or induced labor may require augmentation, its use is not without risk. Although augmentation in nulliparous women during the first stage of labor is generally uncontroversial, its application in multiparous women, during the second stage of labor, in breech presentation, or

The Predictive Accuracy of Prenatal Ultrasound for Macrosomia-A Retrospective Cohort Study.

OBJECTIVES: This study aimed to evaluate the accuracy of third-trimester ultrasound for fetal macrosomia and to assess obstetric outcomes associated with its prenatal suspicion. METHODS: Retrospective cohort study on women receiving prenatal care at a tertiary care hospital between November 2017 and December 2022. Singleton pregnancies with prenatal suspicion of macrosomia (estimated fetal weight ≥ 4000 g or > 90th percentile for gestational age) were included. Controls were singleton term deliveries without suspected macrosomia. Cases were categorized as true positives (TP) and false positiv

Trends of induction of labor with success rates: Experience of a tertiary care center.

OBJECTIVE: Induction of labor (IOL) is a common obstetric procedure performed when continuation of pregnancy poses maternal or fetal risks. Prostaglandin E2 (PGE2) is widely used for cervical ripening and induction. This study assessed the success rate of PGE2-induced labor and identified obstetric predictors of failed induction and cesarean delivery at a tertiary center in Riyadh. METHODS: This retrospective cross-sectional study included 1303 women who underwent IOL with PGE2 (tablet or pessary) at the Women's Health Hospital, National Guard Health Affairs, Riyadh, during 2023. Maternal cha

Parturients with hypertrophic cardiomyopathy: case series and review of pregnancy outcomes and anesthetic management of labor and delivery.

BACKGROUND: Advances in understanding the pathogenesis, diagnosis and management of hypertrophic cardiomyopathy have resulted in increased longevity and a better quality of life of affected patients considering pregnancy. Several case series which focused predominantly on obstetric details have reported generally good outcomes. However, there remains a paucity of data on the specifics of obstetric anesthesia in women with hypertrophic cardiomyopathy. METHODS: After Institutional Review Board approval, we reviewed antepartum transthoracic echocardiograms, cardiology, obstetric, anesthetic, and

Characteristics associated with severe perineal and cervical lacerations during vaginal delivery.

OBJECTIVE: To characterize potentially modifiable risk factors for third- or fourth-degree perineal lacerations and cervical lacerations in a contemporary U.S. obstetric practice. METHODS: The Consortium on Safe Labor collected electronic medical records from 19 hospitals within 12 institutions (228,668 deliveries from 2002 to 2008). Information on patient characteristics, prenatal complications, labor and delivery data, and maternal and neonatal outcomes were collected. Only women with successful vaginal deliveries of cephalic singletons at 34 weeks of gestation or later were included; we ex

Mode of delivery in women after liver transplantation.

AIM: A high rate of cesarean sections has been reported among high-risk pregnancies in liver transplant recipients. The aim of this study was to analyze the course of deliveries and the indications for cesarean sections in women after liver transplantation. MATERIALS AND METHODS: From 2001 to 2006, we noted 21 deliveries in 17 liver recipients. The mean age of women was 27.9 +/- 6.6 years and the mean time from transplantation to pregnancy was 4.3 +/- 3.6 years. Most patients were primigravidas on tacrolimus-based immunosuppressive regimens. We retrospectively analyzed obstetric data regardin

Adherence to Definitions of Labor Arrest Influence on Primary Cesarean Delivery Rate.

OBJECTIVE: The cesarean delivery rate in the United States is 31.9%. One of the leading indications for primary cesarean delivery is labor arrest. A modern understanding of the labor curve supports more time prior to the diagnosis of labor arrest. We conducted this study to examine the impact of adherence to the modern criteria for labor arrest and failed induction on rates of primary cesarean delivery and to identify predictors of meeting these criteria. STUDY DESIGN: We analyzed rates of primary cesarean deliveries overall and primary cesarean deliveries occurring due to arrest of dilation,

Vaginal delivery in patients with a prior cesarean section.

To assess the risks and benefits of vaginal delivery in patients with prior cesarean section, the English literature on this subject from 1950 to 1980 was reviewed. The following conclusions were reached: 1) Properly conducted vaginal delivery after cesarean section is relatively safe, with a 0.7% incidence of uterine rupture, 0.93 perinatal mortality, and no maternal deaths due to uterine rupture. 2) Of those patients allowed a trial of labor, 66.7% were successfully delivered vaginally. Successful vaginal delivery occurred in 74.2% of those patients with a nonrecurrent indication for their p

Women of the courtyard. A nurse's journey to treat obstetric fistulae in Niger.

Obstetric fistula is a devastating condition that results from prolonged or unassisted labor. It produces debilitating physical and emotional consequences caused by constant leaking of urine and/or feces. Because high-quality medical care is available throughout the developed world, unrepaired obstetric fistulae are virtually nonexistent in developed nations. However, the condition is rampant in many developing countries, including Niger, a nation in West Africa. This article explains what obstetric fistula is, why it is such a problem, and what nurses and other health care professionals can d
