Effects of perineal massage during childbirth on maternal and neonatal outcomes in primiparous women: A systematic review and meta-analysis.

BACKGROUND: Perineal massage during childbirth has been recommended as an effective measure to prevent perineal injury. However, the overall effects of perineal massage during childbirth on maternal and neonatal outcomes in primiparous women remain inconclusive. Particularly, the effects of perineal massage begun during different stages of labor need to be further investigated. OBJECTIVES: To comprehensively review the effects of perineal massage during childbirth on primiparous health outcomes, including perineal-related outcomes, duration of labor, hemorrhage and postpartum perineal pain, a

A meta-analysis of upright positions in the second stage to reduce instrumental deliveries in women with epidural analgesia.

BACKGROUND: Epidural analgesia is associated with an increased risk of instrumental delivery. We, in this study, present a systematic review in order to assess the effectiveness of maintaining an upright position during the second stage of labor to reduce instrumental deliveries among women choosing epidural analgesia. The study population included women with uncomplicated pregnancies at term with epidural analgesia established in the first stage of labor. METHODS: We searched MEDLINE, EMBASE, and CINAHL databases and the Cochrane Trials Register up to July 2003 and cross-checked the referenc

Application of second-stage passive descent in morbidly obese women.

OBJECTIVE: To estimate whether passive descent in the second stage of labor in morbidly obese parturients is associated with reduced active pushing times and improved labor outcomes compared with immediate pushing. METHODS: This was a retrospective cohort study of all consecutive women with a body mass index (BMI) of 40 or greater admitted for term labor that reached the second stage of labor from 2004 to 2008 at a single institution. Detailed information was collected for maternal demographics, labor progress, and neonatal outcomes. Length of active pushing, length of total second stage, and

Pushing the bounds of second stage in term nulliparas with a predictive model.

BACKGROUND: Management of the second stage of labor continues to be a clinical challenge with unclear indications for abandoning attempts at spontaneous vaginal delivery. The conflict between diminishing chances of spontaneous vaginal delivery and increasing maternal and neonatal morbidity is difficult to quantify, leading to significant variation in management between providers, and variation in rates of operative vaginal delivery and cesarean birth. OBJECTIVE: The objective of the study was to develop an hourly prediction model for spontaneous vaginal delivery during the second stage of lab

Epidural analgesia and severe perineal tears: a literature review and large cohort study.

OBJECTIVE: Our objectives were to study the association between epidural analgesia and risk of severe perineal tears (SPT), and identify additional risk factors for SPT. METHODS: We conducted a historical cohort study of women with term delivery between 2006 and 2011. Inclusion criteria were an uncomplicated singleton pregnancy, cephalic presentation and vaginal delivery. Multivariate logistic regression models were constructed to study the association between epidural analgesia and SPT, controlling for potential confounders. Additional models studied the association between prolonged second

Adjusting models to better predict obstetric anal sphincter injury (OASIS) in forceps-assisted vaginal deliveries: A retrospective cross-sectional trial.

OBJECTIVE: Obesity and maternal age are increasing among pregnant patients. The understood effect of body mass index (BMI), advanced maternal age (AMA), and second stage of labor on obstetric anal sphincter injury (OASIS) at delivery is varied. The objective of this study was to assess whether incorporating BMI, second stage of labor length, and AMA into a model for predicting OASIS among forceps-assisted vaginal deliveries (FAVD) had a higher predictivity value compared to models without these additions. METHOD: This was an IRB-approved retrospective cohort study of singleton gestations who

The role of the angle of progression in predicting spontaneous vaginal delivery in women with singleton pregnancies.

OBJECTIVE: This study aimed to evaluate the diagnostic accuracy of the angle of progression (AOP), measured by ultrasound at the onset of the second stage of labor, as a predictor of spontaneous vaginal delivery in term singleton pregnancies, and to identify a cutoff value for the AOP angle that could predict normal delivery in women with occiput anterior (OA) cephalic presentation. METHODS: This prospective, observational study was carried out at a single tertiary center between February 2023 and January 2025. The study population consisted of patients at the beginning of the second stage of

Parturients' Stated Preferences for Labor Analgesia: A Discrete Choice Experiment.

OBJECTIVE: The objective was to investigate the extent to which treatment benefits, risks and costs affected parturients' preferences for labor analgesia. METHODS: We recruited 248 healthy parturients prior to labor at an antenatal ward and administered a discrete choice experiment survey. Parturients were asked to choose among four hypothetical forms of labor analgesia: epidural analgesia, pethidine, Entonox and no analgesia, which were defined by: pain score, duration of second stage of labor, risks of instrumental delivery, back pain and permanent nerve injury, and out-of-pocket cost. Clinical experience on tcPco2 during labor.

tcPco2 measurements in the fetus during labor were evaluated by analysing the clinical experience in 224 cases. This additional mode of supervision was performed in combination with continuous cardiotocography (CTG) and intermittent fetal blood sampling (FBA) in cases with suspect, prepathologic or pathologic heart rate patterns. The prechosen measuring temperature was 39 degrees C in 105 and 44 degrees C in 119 cases. The normal range of the tcPco2 was defined by calculating the mean value and two standard deviations in cases without hypoxic complications. The absolute values of the normal ra

[Progress of labor and neonatal morbidity in primiparity with breech presentation].

Progress of labour in 100 consecutively delivered term ( > or = 37 weeks) primiparous breech births was analysed and correlated with fetal outcome according to different durations of first and second stages. All women had sonographic biometry prior to admission and epidural anaesthesia (PDA) early in established labour. Women with nonreassuring fetal heart rate tracings during any stage of labour were delivered by caesarean section, and were excluded. Birth weights were between 1980 g and 4090 g. There was no perinatal mortality, but eight neonates sustained birth associated trauma (6 Erb's pa

Correlation of intrapartum translabial ultrasound parameters with computed tomographic 3D reconstruction of the female pelvis.

AIMS: Intrapartum translabial ultrasound [ITU] can be an objective, reproducible and more reliable method than digital vaginal examination when evaluating fetal head position and station in prolonged second stage of labor. However, two-dimensional (2D) ultrasound is not sufficient to demonstrate the ischial spines and other important "landmarks" of the female pelvis. Therefore, the purpose of this study was to evaluate the distance of the interspinous plane as a parallel line to the infrapubic line in 2D ITU with the help of 3D computed tomography and digital reconstruction. RESULTS: Mean dis

Secondary repair of the perineum following childbirth.

The aim of the study was to evaluate secondary repair of the perineum following perineal trauma at childbirth. Patients who underwent secondary repair reported with symptoms of superficial dyspareunia, vulvo-vaginal pain, vaginal stenosis and broken or gaping wound. The study included 103 women over a 16-year period that underwent refashioning of the perineum at Northwick Park Hospital by the same surgeon. Among the cases, 81.6% were nulliparous and 18.4% were multiparous. Labour was induced in 30.1%, and 38% of the women required instrumental delivery. The length of the second stage of labour

An Appraisal of Labor Augmentation: An Update on Current Evidence and Practice.

Augmentation of labor remains the cornerstone of intrapartum care, typically employed to expedite delivery by enhancing the frequency, strength, and duration of uterine contractions. The decision to augment depends on the stage of labor and the woman's obstetric history. While up to half of all women who enter spontaneous or induced labor may require augmentation, its use is not without risk. Although augmentation in nulliparous women during the first stage of labor is generally uncontroversial, its application in multiparous women, during the second stage of labor, in breech presentation, or

[Labor pain-causes, pathways and issues.].

In the first stage of labor, pain is caused by distension of the cervix and low uterine segments in combination with isometric contraction of the uterus. Pain in the second stage of labor is dominated by tissue damage in the pelvis and perineum. Labor pain is due to an activation of nociceptors partly resulting from ischemia. The impulses thus generated are conducted into the spinal cord by afferent C fibers from the cervix and lower uterine segments, and by afferent Adelta and C fibers from the pelvis, pelvic organs and perineum. Labor pain is referred to the dermatomes T(11) and T(12) in the
