Vitreous management in Yamane's technique for crystalline lens dislocation: anterior vitrectomy or PPV?

OBJECTIVE: To study the postoperative visual outcomes and surgical complications of anterior/pars plana vitrectomy and concurrent Yamane's IOL fixation for crystalline lens dislocation. METHODS: Fifty-three patients (56 eyes) with crystalline lens subluxation/dislocation were enrolled in this retrospective interventional study. Patients received anterior/pars plana vitrectomy and concurrent Yamane's IOL fixation. Main outcome measures were postoperative BCVA and surgical complications. Proportion of spontaneous PVD and preoperative undetected retinal holes/degeneration (PURH/D) were recorded.…

Bilateral Immediate Sequential Vitrectomy and Lensectomy for Bilateral Lens Dislocation in Severe Neonatal Marfan Syndrome.

A newborn with severe neonatal Marfan syndrome (NMS), characterized by aortic root dilation, mitral and tricuspid valve regurgitation, diaphragmatic eventration with respiratory distress, intestinal malrotation, and various skeletal abnormalities, developed bilateral pupillary block from luxated crystalline lenses. He underwent bilateral immediate sequential vitrectomy and lensectomy to prevent further episodes of pupillary block. Bilateral surgery was performed due to the extremely high anesthesia risk. NMS is a rare and severe form of the connective tissue disease that can present with numer

A rare case of traumatic subretinal migration of crystalline lens, corroborated histologically.

Blunt trauma is the most common cause of posterior dislocation of the crystalline lens. We describe a rare case of subretinal migration of crystalline lens through the giant retinal tear following blunt ocular trauma. This incidental finding of subretinal dislocation of lens following blunt ocular trauma was confirmed on histopathological examination of the enucleated eye. This complication has not been described by histopathological examination in literature so far.

[Bilateral ciliary zonule defect].

Anterior segment colobomas can be found isolated or in combination with changes in the posterior segment. A coloboma of the ciliary zonule leads to alteration of the lens profile with myopia and astigmatism. Amblyopia and cataract development may also occur. We present two cases with a coloboma of the zonule with different degrees of expression and the most important differential diagnoses are discussed.

Spontaneous dislocation of posterior polar cataract.

We report an unusual case of unilateral spontaneous lens dislocation in a 52-year-old woman with a posterior polar cataract. We postulate that the increasing lens size secondary to nuclear sclerosis may have exerted pressure on a thinned posterior capsule, causing the capsule to rupture spontaneously. The unusual weak posterior capsule in posterior polar cataract is a well-recognized risk factor in phacoemulsification surgery. We believe this is the first report case of spontaneous lens dislocation secondary to posterior capsule rupture in posterior polar cataract.

Lens luxation in a CD-1 mouse.

Lens luxation was found in a male CD-1 (ICR) mouse. Ophthalmologic examinations revealed conical cornea anterior synechia and corneal neovascularization in the right eye. The lens was dislocated heterocentrically within the posterior chamber. Histologically, anterior lens luxation and adhesion between the iris and cornea were observed.

Phacoemulsification with combined capsular tension ring, supracapsular glued IOL, and optic capture in progressive subluxations for expanding and immobilizing the capsular bag.

To report and assess a new technique of phacoemulsification with capsular tension ring (CTR), supracapsular intra-scleral haptic fixation (ISHF), and optic capture in eyes with progressive subluxations and to assess stability of capsular bag-CTR-IOL optic complex. Cases with progressive subluxation first underwent phacoemulsification with CTR implantation. Supracapsular ISHF in the form of glued IOL was then performed by externalizing the haptics through a plane below the iris and above the anterior capsule via sub-flap sclerotomies created 180 degrees apart as for glued IOL. Intra-scleral hap

Bilateral clear lens extraction and intraocular lens implantation in a child with microspherophakia and Marfan syndrome.

We report the case of a 4-year-old boy with Marfan syndrome whose parents reported he had had low visual acuity since birth. On examination, there was microspherophakia and a small subluxation of the lens. The objective refraction was -23.75 - 2.75 x 70 in the right eye and -25.50 -3.50 x 90 in the left eye. Since the microspherophakia and the high myopia severely affected the boy's quality of life, clear lens extraction, anterior vitrectomy, posterior surgical capsulotomy via the pars plana, and intraocular lens implantation were performed. Two years postoperatively, the patient had centered

Managing a dropped nucleus during the phacoemulsification learning curve.

Three patients had a pars plana vitrectomy to remove retained nuclei within 72 hours after phacoemulsification performed by a surgeon making the transition from extra-capsular cataract extraction to phacoemulsification. After vitrectomy, the nuclei were brought to the midvitreous cavity from the retinal surface with a posterior segment phacofragmenter, emulsified, and completely removed. Then, a posterior chamber intraocular lens was implanted in the sulcus through the previous cataract surgery incision and remained well centered postoperatively. Postoperative complications included cystoid ma

Acute angle-closure glaucoma in a highly myopic patient secondary to Weill-Marchesani syndrome: histopathologic lens features.

The authors describe a case of acute angle-closure glaucoma in a highly myopic patient secondary to Weill-Marchesani syndrome (WMS) and histopathologic features of his lens. A 37-year-old male patient visited our clinic for ocular pain with elevated intraocular pressure (19/57 mmHg). The slit-lamp examination showed an inferiorly subluxated lens in the right eye, and anterior dislocated microspherophakia with corneolenticular contact in the left eye. The physical examination showed brachydactyly and relatively short stature. To control the IOP and to improve visual acuity, lens extraction, ant

[Clinical results of the aphakia correction by means of secondary implantation of the iris-fixated anterior chamber intraocular lens].

The authors refer about the results of the secondary implantation of the anterior chamber intraocular lens (AC IOL) Artisan Aphakia. Those specially constructed iris claw lenses for aphakia correction, fixated on the iris, were implanted at the Department of Ophthalmology in Hradec Králové (Königgrätz, Czech Republic) during the period August 2002-May 2005 in 51 cases (51 eyes, 48 patients). The most common indication (33 % of cases) was the previous trauma with lens damage and consequent postoperative aphakia. The second most common cause (in 29 %) of AC IOL use was the absence of the lens af
