Classification of acute leukemias.

Because of the increasing recognition of the importance of genetic events to the diagnosis and treatment of the acute leukemias, the proposed new World Health Organization (WHO) classification incorporates genetic aberrations and immunology as major defining features in addition to morphology. In a hierarchal approach, genetic changes have precedence in the acute myeloid leukemias and immunology and genetic changes have precedence in the acute lymphoblastic leukemias. Four major groups of acute myeloid leukemia are recognized: 1) Acute myeloid leukemias with recurrent genetic abnormalities, 2)…

Usefulness of cytogenetics in leukemias.

Present study consists of cytogenetic evaluation in 141 cases referred to our centre for various leukemias. This includes 110 cases of CML, 10 of ALL, 16 of AML (M3), 2 of AML(M2), 2 of MDS and 1 of CMML. The conventional cytogenetic study was carried out in all the cases using G Banding technique. Of the 141 patients studied, 17 patients showed secondary chromosomal alterations along with primary chromosomal alterations. In two patients of CML with secondary chromosomal alteration t(4:9:22), molecular cytogenetic technique (FISH) has been carried out which has confirmed the primary observatio…

Immunohistochemistry in leukemias and lymphomas.

The characterization of the antigenic and immunologic properties of lymphoid and hematologic neoplasms has dramatically increased our unders … f antigens. Unique immunophenotypic profiles have now been described for most forms of lymphoma and leukemia. Classic Hodgkin's disease, for example, has the singular combination of CD15 and CD30 positivity, in the absence of CD45 (see Fig. 1 ). Additional examples are mantle cell lymphoma with its unique nuclear expression of cyclin D1, and hairy cell leu …

Chemotherapy of secondary leukemias.

Chemotherapy of secondary leukemias is currently still considered to be associated with poor results. However, recent data suggest that the response to remission induction may substantially differ according to the previous medical history of the patients. Therapy related leukemia, arising following exposure to previous alkylating agents or radiotherapy, is often associated with chromosomal abnormalities involving chromosomes 5 and 7 and has a particularly bad response, whereas AML after exposure to epipodophyllotoxins or topoisomerase-II active agents could have a somewhat better response. Acu…

Leukemia and lymphoma in childhood.

The leukemias and lymphomas together account for over 40% of the malignant disorders in childhood. Radiologists who deal with these patients must understand some aspects of the biology, natural history, and clinical manifestations of the disease in order to provide useful imaging consultative services. This article summarizes these issues briefly, gives an overview of the common imaging appearances, and recommends imaging studies to be utilized in the initial staging evaluation of newly presenting patients.

The genetics of familial leukemia.

Familial leukemia is rare, but, as is the case with other cancer family syndromes, its study is likely to lead to the identification of genes causative of the far more common, sporadic cases. I review the clinical and, what is known of the molecular genetic features of familial leukemia. I propose a nosology based on whether the leukemia is a component of a medical syndrome or exists as a solitary disease, the apparent mode of inheritance, and the distribution of leukemia types and subtypes in affected family members. I review the recent findings from my group that leukemia is inherited with '…

[Progress in diagnosis of leukemia].

Recent advances in molecular biological techniques have contributed to the tremendous progression made in the field of diagnosis of leukemia. Discovery of T- or B-lymphocyte associated genes, tumor specific genes and genes involved in chromosomal translocation has made it possible to detect leukemia cells by Southern blotting, PCR, RT-PCR or fluorescence in situ hybridization (FISH). The recently developed FISH is a simple, rapid and accurate method and requires a very small amount of specimen (about 500-1000 cells). It is possible to obtain results within 48 hours of sampling. This lecture we…

Megakaryoblastic leukemia in a dog.

A 7-year-old spayed Louisiana Catahoula Leopard dog was examined to determine the cause of shifting forelimb lameness, anorexia, and letharg … rate was characterized by a mixture of megakaryoblasts and typical megakaryocytes. Megakaryoblastic leukemia (M7) is the designation proposed by the Animal Leukemia Study Group for myeloproliferative neoplasms of megakaryocytic lineage.

[Pseudothrombocytes in acute leukemias].

Pseudoplatelets are the cytoplasmic fragments in the size of platelets originated not from megakaryocytes but from other blood cells which were observed in some patients with leukemia and infection. The clinical importance of pseudoplatelets is manifested in the falsely increased number of platelets in these patients. Transmission electron microscopy was used in the cytologic examination of 23 patients with different types of acute leukemia. The presence of pseudoplatelets in the form of budding of the cytoplasm of the granulocytic, monocytic and lymphocytic strains of various maturity has bee…

Teniposide in lymphomas and leukemias.

The epipodophyllotoxins, etoposide and teniposide, have been used in leukemias and malignant lymphomas for the past 15 years. Although etoposide has acquired a place in many first-line protocols for lymphomas and, more recently, for leukemias, the role of teniposide has remained limited. Teniposide is a more potent inhibitor of topoisomerase II than etoposide, and has a less toxic effect on hematopoietic progenitor cells. Both drugs have been regarded as equitoxic and cross-resistant. The role of teniposide in front-line treatment of leukemias has only been established in childhood acute lymph…

The association of Tn and leukemia.

Tn polyagglutination or persistent mixed field polyagglutination is a rare acquired erythrocytic state which has been associated with haematologic abnormalities including four reported cases of acute leukaemia. Recent information has proven that the abnormality is clonal and the haematologic findings suggest a mutation of a somatic stem cell. The red cell membrane defect results from a deficiency of B-3-D galactosyltransferase. Tn polyagglutination has been described in patients presenting with acute leukaemia and has preceded acute leukaemia. The Tn syndrome has similarities to other forms of

Priapism in children with leukemia.

A case of priapism in a child with chronic granulocytic leukemia is presented, and the English literature is reviewed. While common in adult leukemia patients, priapism is rare in children with these neoplasms. Sludging of blood in the corpora cavernosa is almost universally accepted as the mechanism behind the development of painful and persistent erection in these patients. Permanent fibrosis of the cavernosal tissue may not occur in leukemic priapism as it does in other than chronic granulocytic leukemia. Therapy directed toward the priapism is almost uniformly unsuccessful. However, the pa…
