We report the case of a patient in whom lichen nitidus with hemorrhage was found on the palm and the thigh. Histopathologically, the small vessels in the lesions showed extensive changes and were increased in number. The capillary walls revealed degenerative changes with extravasation of erythrocytes. On electron microscopy, amorphous, electron-dense deposits were observed along the outside of the endothelial cells. These findings can explain the presence of the hemorrhagic changes. We describe this as a case of purpuric lichen nitidus.

Lichen planus is a common skin and mucosal disease, with very rare symptomatic oesophageal involvement. We report a case of painful dysphagia due to oesophageal lichen planus in a 60-year-old woman who also had oral, cutaneous and genital lichen planus lesions. Steroid treatment produced considerable improvement of all lesions and a rapid symptomatic remission.

The most common form of vulvar dermatose is the lichen sclerosus. More frequent after menopause, it can be observed at all ages, even in childhood. Vulvar lichen sclerosus manifests usually by a vulvar pruritus and (or) orificial dyspareunia, however in 15% of cases it remains asymptomatical. The lesions can extend to all the vulva, giving it a very particular pearly white aspect, or remain localized as a "leucoplasia", requiring biopsy to confirm the diagnosis. It is mostly the hyperplastic lichen sclerosus (consisting of a marked epithelial hyperplasia) which risks degeneration. In case of n

A 58-year-old Japanese woman with lichen striatus had asymptomatic multiple erythemato-papular plaques in arranged linear or band-like patterns over the right half side of her chest, back, and the right upper and lower extremities, following Blaschko's lines. A biopsy specimen demonstrated a dermoepidermal interface inflammation with a lichenoid lymphoid cell infiltrate into the upper and middle dermis. All lesions spontaneously cleared within 2 months. Lichen striatus in an adult should be differentiated from other acquired erythemato-papular dermatoses which may follow Blaschko's lines. Its

I report familial lichen nitidus in a 33-year-old father and his 3-year-old daughter. Histopathology of small papules in both cases revealed typical features of lichen nitidus. Immunohistochemical examination showed UCHL1-positive, L-26-negative lymphocytes and HAM56-positive histiocytes and multinucleated giant cells in the dermal infiltrate in both cases.

Lichen planus is an inflammatory pruritic dermatosis in which T lymphocytes attack the basal epidermis, producing characteristic clinical and histological lesions. Mild cases can be treated successfully with rest, topical corticosteroids with or without wet dressings, or occlusion. Severe generalised cases may require systemic corticosteroids. Chronic, aggressive lesions may need additional therapy ranging from psoralen plus ultraviolet A (PUVA) to retinoids or cyclosporin. Complications ranging from infectious, bullous disease or ulceration require special treatment considerations.

The case report of a 83-year-old woman with lichen ruber ulcerosus is reviewed. Clinically the patient showed extensive ulcerations of the sole of one foot, with synechiae of the toes. In addition, there was subtotal atrophic alopecia of the scalp, vulvar atrophy, and circumscribed cutaneous lesions of lichen planus. The disease started with chronic recurrent paronychiae. The ulcerative defect was successfully treated with mesh grafting, which is the therapy of choice.
