What happens after a failed LIFT for anal fistula?

BACKGROUND: Ligation of the intersphincteric fistula tract (LIFT) was developed to treat transsphincteric anal fistulas. The aftermath of a failed LIFT has not been well documented. METHODS: Retrospective chart review of LIFT procedure for transsphincteric anal fistula between March 2012 and September 2016. RESULTS: 53 patients with LIFT procedure were identified, 20 (37.7%) had persistent fistula with median followup of 4 months. Following LIFT, recurrence of fistula was transsphincteric (75%) or intersphincteric (25%) (p = NS). Persistent transsphincteric fistulas after LIFT were treated w

Temple and Postauricular Dissection in Face and Neck Lift Surgery.

Periauricular paresthesia may afflict patients for a significant amount of time after facelift surgery. When performing face and neck lift surgery, temple and posterior auricular flap dissection is undertaken directly over the auriculotemporal, great auricular, and lesser occipital nerve territory, leading to potential damage to the nerve. The auriculotemporal nerve remains under the thin outer superficial fascia just below the subfollicular level in the prehelical area. To prevent damage to the auriculotemporal nerve and to protect the temporal hair follicle, the dissection plane should be ke

The Use of a Stylet to Aid the Lifting of the Epiglottis With a Video Laryngoscope.

This study involved 154 anesthetized patients requiring endotracheal intubation, in whom the epiglottis was visible but not lifted, which restricted the view of the glottis. A simple stylet was used to lift the epiglottis, thus improving the view of the glottis with the Glide Scope®, allowing tracheal tubes to be inserted into the patients. The stylet easily resolved the difficulty of lifting the epiglottis when using a video laryngoscope, thus facilitating endotracheal intubation in patients undergoing general anesthesia.

A novel patient-controlled bidirectional palatal lift appliance.

OBJECTIVE: Palatal lift appliances have a role in management of velopharyngeal dysfunction for immobile palates of adequate length where surgery is contraindicated. Conventional appliances involve acrylic/wire work adjustment over successive appointments until they can be tolerated without gagging. A novel appliance has been developed where the lifting plate is incrementally distalized by the patient and vertically adjusted to optimize soft palate positioning. METHOD: The design, construction, and utility of the appliance, which was developed in Dundee Dental Hospital, are described. PARTICI…

A simplified palatal lift prosthesis for neurogenic velopharyngeal incompetence.

Velopharyngeal incompetence is a contributing factor to speech disorders and implies the presence of hypernasality, inappropriate nasal escape, and decreased air pressure during speech. One prosthetic treatment is a rehabilitative procedure employing a palatal lift prosthesis (PLP), which reduces hypernasality by approximating the incompetent soft palate to the posterior pharyngeal wall and consists of two parts, the anterior denture base and the palatal lifting plate, which are connected with steel wires; however, it seems difficult to reproduce the mobility of the soft palate in speaking, an…

Percutaneous trigger finger release: the 'lift-cut' technique.

One hundred and eighty patients with 240 trigger digits were treated by percutaneous release using a 'lift-cut' technique. All patients were reviewed at 3 months following release. Overall, 94% achieved an excellent or good result. Ten patients experienced recurrent symptoms and required a subsequent open release. There was no clinical evidence of digital nerve or flexor tendon injury. We recommend this technique as a safe and effective outpatient procedure.

The effect on wear of lift-off in total knee arthroplasty.

Femoral lift-off has been reported in total knee arthroplasty using dynamic fluoroscopic analysis. However, the effect of femoral lift-off on ultra-high-molecular-weight polyethylene (UHMWPE) is not clear. The objective of this study was to evaluate the effect of lift-off movement on the wear of UHMWPE using a pin-on-disc testing machine. Using oxidised UHMWPE, delamination was observed in 70% of the discs with lift-off movement, as against none in without lift-off movement (p=0.0603). In non-oxidised UHMWPE the depth of the wear track was increased in the impact area after lift-off, but there

Reducing eyelid retraction following subperiosteal face lift.

The most common complication of lower blepharoplasty is malposition of the lower eyelid. This deformity may present as scleral show, ectropion, entropion, or canthal dystopia, with dry eye or epiphora. These problems occurred frequently in our initial series of patients undergoing blepharoplasty with subperiosteal face lift. The key to prevention lay in avoiding the potential causes of these problems: overresection of eyelid skin, retraction of the orbital septum, orbicularis oculi dysfunction, postoperative swelling, and downward pull from the facial suspension. This article reviews the cases

Endoscopic-assisted midface lift utilizing retrograde dissection.

Endoscopic facial surgery is gaining wide acceptance, as results are providing comparable if not superior to the standard, superficial musculoaponeurotic system rhytidectomy, with minimal scarring. Frontal branch facial nerve injury remains a troublesome complication. The purpose of our study was to determine if a subciliary retrograde dissection would decrease facial nerve injury and allow for more accurate midfacial suspension. Fresh cadaver dissections were performed using endoscopic equipment. Anterograde temporal incisions in the scalp and retrograde subciliary incisions were studied. The

Instruction during lifting: How analogy and explicit instruction influence performance and motor control.

PURPOSE: This study aimed to examine the effects of analogy (ANA) and explicit (EXP) verbal instruction on motor performance and muscle activation during a weightlifting task. Traditional coaching strategies often rely on explicit cues; however, recent research suggests that analogy-based instructions may offer a more efficient route to motor learning by promoting implicit control. SCOPE: Twenty novice participants were randomly assigned to one of three conditions (ANA, EXP, or control), with the sequence of conditions counterbalanced across three consecutive days. Day 1 involved familiarizat…

Control of Posture During Lifting: Effects of Weight Knowledge and Load Position Asymmetry.

Lifting objects challenges postural stability and requires the generation of anticipatory (APA) and compensatory postural adjustments. This study investigated the combined effects of perturbation predictability and asymmetric load placement on postural control during a bimanual lifting task. Sixteen healthy adults completed a randomized crossover study involving repeated lifts of a box containing a load equal to 5% of their body weight that was positioned asymmetrically under three conditions: known, unpredictable, and predictable. Surface electromyography from trunk and lower limb muscles and…
