Contemporary limbal stem cell transplantation - a review.

Conjunctivalization of the cornea is the hallmark of limbal stem cell deficiency (LSCD). This is often associated with persistent corneal epithelial defects and a fibrovascular pannus. LSCD can be unilateral or bilateral and partial or total. In partial LSCD involving the visual axis sequential sector conjunctival epitheliectomy (SSCE) is a useful option. In total LSCD, transplantation of limbal tissue or of ex vivo expanded sheets is the mainstay. In unilateral cases autolimbal transplant is the procedure of choice. In bilateral cases living (related) and cadaver donors are considered. The fo…

Single-cell analysis of heterogeneity and molecular changes in cultured corneal epithelial stem cells during serial passage.

The maintenance of corneal epithelial homeostasis relies on limbal stem cells (LSCs) located at the limbus. Although short-term cultured LSC transplantation effectively treats LSC deficiency, prolonged culture leads to stemness loss and abortive colony formation, and the mechanisms remain elusive. In this study, we employed single-cell transcriptomics to investigate LSC population dynamics and changes in gene expression during extended serial culture. Transcriptomic data from 22 708 cells revealed 19 clusters, identifying 3 distinct limbal progenitor populations (Progenitors 1-3) with unique t

Postoperative complications of conjunctival limbal autograft surgery.

PURPOSE: To report postoperative complications of conjunctival limbal autograft in patients with unilateral total limbal stem cell deficiency (LSCD). METHODS: In this retrospective observational case series, medical charts of 34 patients with unilateral total LSCD with at least 6 months of follow-up were reviewed. Main outcome measure was postoperative complications. RESULTS: The mean follow-up period was 17.2 ± 6.3 (range, 6-33) months. The postoperative complications observed included conjunctival encroachment over the graft, small graft, graft dislodging, thick graft, improper position, g…

Human Umbilical Cord-Derived Mesenchymal Stem Cells Promote Corneal Epithelial Repair In Vitro.

Corneal injuries are among the leading causes of blindness and vision impairment. Trauma, infectious keratitis, thermal and chemical (acids and alkali burn) injuries may lead to irreversible corneal scarring, neovascularization, conjunctivalization, and limbal stem cell deficiency. Bilateral blindness constitutes 12% of total global blindness and corneal transplantation remains a stand-alone treatment modality for the majority of end-stage corneal diseases. However, global shortage of donor corneas, the potential risk of graft rejection, and severe side effects arising from long-term use of im…

Sludging: A Postoperative Clinical Finding After Ocular Surface Stem Cell Transplantation.

PURPOSE: The objective of this study was to describe a postoperative clinical finding coined "sludging." Sludging is defined as a noninflammatory vascular phenomenon observed following ocular surface stem cell transplantation (OSST) characterized by dilated blood vessels localized to the graft. Also, the aim of our study was to identify associated risk factors and the impact on OSST graft outcomes. METHODS: A total of 261 eyes of 196 patients who underwent OSST to treat severe limbal stem cell deficiency from 2006 to 2016 were included in this retrospective review. Clinical characteristics we…

Corneal surface reconstruction for the chemical injured eye by transplanting autologous cultivated limbal epithelial sheet "Nepic®".

BACKGROUND: "Nepic®" (Japan Tissue Engineering Co., Ltd., Gamagori, Japan) is an autologous cultivated limbal epithelial sheet, approved in 2020 in Japan for the reconstruction of the corneal surface in cases of limbal stem cell deficiency (LSCD). Because the surgical procedure known as cultivated limbal epithelial cell sheet transplantation (CLET) has only recently been introduced and the number of cases remains limited, accumulating clinical experience is essential to ensure the safety and success of this procedure. Herein, we report the clinical course of a patient with unilateral LSCD who …

Keratoprosthesis in dry eye disease.

Bilateral corneal blindness with severe dry eye disease (DED), total limbal stem cell deficiency with underlying corneal stromal scarring and vascularization, combined with adnexal complications secondary to chronic cicatrizing conjunctivitis is a highly complex situation to treat. In such eyes, procedures such as penetrating keratoplasty alone or combined with limbal stem cell transplantation are doomed to fail. In these eyes, keratoprosthesis (Kpro) or an artificial cornea is the most viable option, eliminating corneal blindness even in eyes with autoimmune disorders such as Stevens-Johnson …

Evaluation of the factors that influence surgical outcome in conjunctival-limbal allograft transplantation.

OBJECTIVE: To evaluate the surgical results and the variables affecting the outcomes in conjunctival-limbal allograft transplantation (CLAL). METHODS: Patients who underwent CLAL for limbal stem cell deficiency (LSCD) between 2007 and 2019 were included in the study. LSCD staging was performed according to the staging system developed by the 'Limbal Stem Cell Working Group'. Stage 1C and higher stage LSCD patients were included in the study. 'Successful surgical outcome' was defined as improvement in LSCD stage at 1 year postoperatively. RESULTS: A total of 19 eyes of 19 LSCD patients were i…

Ocular Surface Changes After Sulfur Mustard Exposure in Rabbits, Monitored by Impression Cytology.

PURPOSE: Ocular injuries after exposure to sulfur mustard (SM) are characterized by acute corneal erosion and inflammation of the anterior segment that may be followed by delayed corneal neovascularization and epithelial defects, associated with limbal stem cell deficiency in part of the exposed eyes. This study aimed to further clarify the mechanism of the late injury by monitoring SM-induced cytological alterations in the ocular surface, in relation to the clinical symptoms, using impression cytology (IC). METHODS: Rabbit eyes were exposed to SM vapor (n = 20) and were clinically observed u…

WNT16b promotes the proliferation and self-renewal of human limbal epithelial stem/progenitor cells via activating the calcium/calcineurin A/NFATC2 pathway.

Our previous finding revealed that WNT16b promoted the proliferation of human limbal epithelial stem cells (hLESCs) through a β-catenin independent pathway. Here, we aimed to explore its underlying molecular mechanism and evaluate its potential in the treatment of limbal stem cell deficiency (LSCD). Based on the findings of mRNA-sequencing, the expression of key molecules in WNT/calcineurin A/NFATC2 signalling pathway was investigated in WNT16b-co-incubated hLESCs and control hLESCs. An epithelial wound healing model was established on Wnt16b-KO mice to confirm the regulatory effect of WNT16b …
