Lupus Lymphadenitis With Intense Pentixafor Avidity But Absent FDG Uptake.

A 33-year-old woman presented with intermittent fever for 1 month, accompanied by pancytopenia and multiple enlarged lymph nodes in the bila … ive lymphoid hyperplasia; combined with elevated anti-ds-DNA, anti-SS-A, and anti-RNP titers, lupus lymphadenitis was diagnosed. After antisystemic lupus erythematosus therapy, the lymph nodes regressed significantly.

Nocardia asiatica Lymphadenitis in a Lung Transplant Recipient Following Travel.

We report a case of lymphocutaneous nocardiosis in a 73-year-old male lung transplant recipient after traumatic soil exposure abroad. The patient traveled from Portugal to Brazil to undergo lung transplantation and, 3 months later, while receiving immunosuppressive therapy (with prednisolone, mycophenolate mofetil, and tacrolimus), sustained a nasal laceration after a fall onto soil-contaminated ground in Brazil. A persistent ulcerated facial lesion subsequently developed and was initially misdiagnosed as sporotrichosis, leading to treatment with itraconazole. Despite partial improvement, the

Steroid-responsive generalised sterile necrotising pyogranulomatous lymphadenitis in a cat.

A 1.5-year-old neutered male cat was presented with fever, inappetence, lethargy and marked generalised lymphadenopathy of 4 days duration. Excisional biopsy of peripheral lymph nodes revealed necrotising pyogranulomatous lymphadenitis. Extensive investigation failed to identify an underlying infectious [bacterial (Bartonella, Mycobacteria, Mycoplasma, other Gram-positive or -negative bacteria), viral (feline infectious peritonitis), protozoal (toxoplasmosis), or fungal], neoplastic or inflammatory condition. No improvement was observed with short-term antimicrobial therapy with amoxicillin-cl…

Observation-Only Management of Cervicofacial Nontuberculous Mycobacterial Lymphadenitis in Children.

OBJECTIVES: To determine the natural history of nontuberculous mycobacterial (NTM) lymphadenitis in immunocompetent children managed nonoperatively. METHODS: A retrospective study including patients (< 18 years old) diagnosed with cervicofacial NTM lymphadenitis at a tertiary children's hospital between 2017 and 2023. Patients were either observed or surgically treated. Analysis focused on observation outcomes. Surgical patients were included for comparative context. RESULTS: Seventy-two patients were included; 45 (63%) were managed by observation-only and 27 (37%) underwent surgical excisio…

Unilateral inguinal lymphadenitis caused by Yersinia pseudotuberculosis. A case report.

Acute inguinal lymphadenitis is usually caused by lower extremity infection and sexually transmitted diseases, such as chancroid, lymphogranuloma venereum, genital herpes, or syphilis. Yersinia pseudotuberculosis is a non-spore forming, pleomorphic, non-lactose fermenting Gram negative bacillus and a member of the family Enterobacteriaceae, which is associated with diarrheal diseases. It also causes mesenteric lymphadenitis at the terminal ileum, which can be clinically indistinguishable from acute appendicitis (pseudoappendicitis). However, lymphadenitis in other regions caused by the organis…

Human Immunodeficiency Virus Lymphadenitis Patterns on Fine-Needle Aspiration Cytology.

OBJECTIVE: The aim of this work was to study the microscopic patterns of human immunodeficiency virus (HIV) lymphadenitis on fine-needle aspiration cytology (FNAC) and correlate them with cluster of differentiation 4 (CD4) counts. STUDY DESIGN: A retrospective study of known HIV-positive patients who underwent lymph node FNAC over a period of 5 years (2009-2013) was undertaken. The cytology slides were retrieved and reviewed. Out of 317 cases, 38 (11.7%) were diagnosed as HIV lymphadenitis. We analysed the cytomorphological patterns of HIV lymphadenitis and correlated them with the CD4 cell c…

Unusual case of cervical syphilis with Piringer-Kuchinka-like lymphadenitis.

Syphilis rarely presents with cervical lymphadenopathy. We describe a patient with a cervical mass associated with weight loss, asthenia, and night sweats. The mass was excised and histological examination suggested Piringer-Kuchinka lymphadenitis, which is usually present in cervical toxoplasmosis. To the best of our knowledge, Piringer-Kuchinka lymphadenitis has not previously been reported in cervical syphilis.

Comparison of Mycobacterium lentiflavum and Mycobacterium avium-intracellulare complex lymphadenitis.

BACKGROUND: Mycobacterium lentiflavum is considered a rare pathogen causing nontuberculous mycobacterial (NTM) lymphadenitis. METHODS: A multicenter, retrospective study was performed in immunocompetent children <14 years of age with microbiologically confirmed NTM lymphadenitis treated at 6 hospitals in Madrid, Spain, during 2000-2010. We compared children with M. lentiflavum and Mycobacterium avium-intracellulare complex infection. RESULTS: Forty-five microbiologically confirmed NTM lymphadenitis patients were identified: 19 (45.2%) caused by M. avium-intracellulare complex, 17 (40.5%) by …

Pulmonary infection with caseating mediastinal lymphadenitis caused by Mycobacterium gordonae.

It is often difficult to discern true mycobacterial infection from colonization due to Mycobacterium gordonae (M. gordonae) since this organism is ubiquitous and is commonly an innocuous saprophyte. This study reports a rare case of caseating hilar adenopathy and pulmonary disease caused by M. gordonae in a patient with chronic obstructive pulmonary disease (COPD) and rheumatoid arthritis (RA) on maintenance steroids and methotrexate. Pathologic exam and cultures of lymph node excision biopsy and bronchoalveolar lavage (BAL) confirmed the diagnosis. Triple antimycobacterial therapy with azithr

FDG PET/CT showing erythema nodosum associated with tuberculous lymphadenitis.

Erythema nodosum (EN) is histopathologically an acute septal panniculitis of subcutaneous adipose lobule. It can be either idiopathic or secondary to various underlying conditions. A female patient with EN underwent FDG PET/CT to search underlying cause. The images showed enlarged lymph nodes in the mediastinum with moderately elevated FDG uptake and multifocal increased FDG uptake over her lower extremities. The patient's condition was subsequently diagnosed with EN associated with mediastinal tuberculous lymphadenitis based on skin biopsy, tuberculin skin test, and treatment response.

Bacillus Calmette-Guerins vaccination at birth causing tuberculous granulomatous lymphadenitis.

A rare but severe complication of Bacillus Calmette-Guerin (BCG) vaccination is the development of BCG disease, which can result in necrotizing granulomatous lymphadenitis. Symptoms can present as late as several months following the BCG vaccination. The key finding in BCG disease is the formation of caseating granulomas in draining lymph nodes; detection of BCG organisms from tissue samples are evident.

Surgical treatment for nontuberculous mycobacterial (NTM) cervicofacial lymphadenitis in children.

PURPOSE: To compare surgical excision with surgical curettage in the treatment of nontuberculous mycobacterial (NMT) cervicofacial lymphadenitis in children. PATIENTS AND METHODS: Fifty children, 22 boys and 28 girls, with a PCR- or cultured-confirmed diagnosis of cervicofacial NTM infection were included in the study. Twenty-five children were randomized to surgical excision of the involved lymph nodes, and 25 children to surgical curettage. RESULTS: The median age of the children was 36 months (range, 14-120 months). All children had a red, fluctuating lymphadenitis, and there were no mark…

Myasthenia gravis aggravated by steroid-induced isolated mediastinal tuberculous lymphadenitis.

BACKGROUND: Myasthenia gravis (MG) is occasionally aggravated by chronic infection, of which there are many kinds. CASE REPORT: We report herein the case of a 56-year-old woman with MG aggravated by the activation of isolated mediastinal tuberculous lymphadenitis (MTL) during corticosteroid administration. The possibility of MTL had been disregarded in the differential diagnosis of aggravation of MG weakness, because MTL without pulmonary manifestations is uncommon even in areas where tuberculosis is endemic. CONCLUSIONS: This case suggests that chronic infections such as tuberculosis should…
