From Resection to Rehabilitation in One Day: Digital Workflow for Mandibular Reconstruction with Fibular Free Flap and Immediate Dental Rehabilitation Using CAD/CAM Guides at the Point of Care.

By using virtual surgical planning (VSP) and 3D printed guides, complex maxillofacial defects can be reconstructed with high accuracy and predictability. A fully digital workflow resulting in a modular all-in-one 3D printed guide system for fibula osteotomies, bone segment positioning, fully guided dental implant placement and dental prosthesis fixation for mandibular reconstruction was developed at Ghent University Hospital. A follicular ameloblastoma of the left mandible was resected in a 28-year-old male. The defect was reconstructed with a two-segment fibular free flap with immediate place

Prosthodontic Management of Combination Syndrome Using Digital Scanning and Direct Metal Laser Sintering Crowns: A Novel Approach.

Combination syndrome presents unique challenges in prosthetic rehabilitation due to severe maxillary ridge resorption, supra-eruption of mandibular anterior teeth, and occlusal imbalances. This case report describes the management of a 76-year-old male patient with an edentulous maxilla and partially edentulous mandible, rehabilitated using a conventional maxillary complete denture and a mandibular removable partial denture since the patient declined implant-based treatment due to financial and surgical constraints. A mucostatic impression technique was used to avoid displacing hyperplastic ti

Evaluation of Biomechanical Effects of Mandible Arch Types in All-on-4 and All-on-5 Dental Implant Design: A 3D Finite Element Analysis.

This study evaluates the biomechanical effects of different implant configurations in various mandibular arch types using finite element analysis (FEA). Stress distribution and deformation patterns were analyzed under different loading conditions in square, U-shaped, and V-shaped arches. The results indicate that increasing the number of implants generally reduces cortical bone stress, particularly in U and V arches, while implant-level stress tends to increase. Under molar loading, cortical bone stress in the square arch decreased by 16.9% (from 90.61 MPa to 75.27 MPa) with the All-on-5 syste

SEMAC + VAT for Suppression of Artifacts Induced by Dental-Implant-Supported Restorations in Magnetic Resonance Imaging.

The purpose of this study was to assess the feasibility of SEMAC + VAT to reduce artifacts induced by dental implant-supported restorations, such as its impact on the image quality. Dental-implant supported restorations were installed in a dry mandible. Magnetic resonance scans were acquired on a 3-Tesla MRI system. Artifact suppression (SEMAC + VAT) was applied with different intensity modes (weak, moderate, strong). Artifacts assessment was performed by measuring the mandible volume increase in MRI images prior (reference dataset) and after installation of dental implant-supported prosthesis

[The role of scintigraphy in the diagnosis of mandibular bone changes].

Investigating the scintigraphic images of jaws may have a diagnostic value of bone alterations of dental origin. Anterior view of whole body bone scintigraphy revealed hot spot on jaws of 61% (279) of patients. Twenty-six patients (mean age 58.3 year) from all of those who had increased tracer uptake (ITU) in the maxillo-mandibular region were called back for dental examination. 279 out of 459 (61%) patients had ITU in the maxillo-mandibular region. Dental examination revealed the dental origin of ITU in all cases. In case of periapical pathosis tracer uptake showed 28.79% increase compared to

A prospective 15-year follow-up study of mandibular fixed prostheses supported by osseointegrated implants. Clinical results and marginal bone loss.

In this prospective study 47 edentulous patients were treated with mandibular fixed prostheses supported by osseointegrated Brånemark implants and followed for 12 to 15 years. Three (1%) of the 273 inserted implants were lost, two before and one six years after placement of the fixed prosthesis. The cumulative success rate (CSR) of the implants was 98.9% both after 10 and 15 years. None of the fixed prostheses was lost and at the last follow-up, all patients had stable fixed prostheses in function (CSR 100%). The marginal bone loss around the implants was small, on average 0.5 mm during the fi

Prosthodontic rehabilitation of a patient diagnosed with multiple idiopathic root resorption.

A female college student was referred by her general dentist for evaluation of the interproximal caries-like lesions discovered during her biannual visit. The lesions were determined to result from external resorption of unknown origin. After the most affected teeth had been extracted and the other teeth were restored after periodontal surgery, an interim partial removable dental prosthesis was placed. Two months later, the resorption pattern had proliferated to the rest of her dentition, thus revising the treatment plan to involve full mouth extractions and to place immediate maxillary and ma

[Posterior cantilevered all-ceramic resin-bonded fixed dental prosthesis: A minimally invasive solution for restoring posterior edentulous spaces illustrated by a chairside clinical case].

INTRODUCTION: While the anterior cantilevered resin-bonded fixed dental prosthesis (RBFDP) is now well established among orthodontists and general practitioners, the posterior cantilevered RBFDP has only more recently gained attention in the international literature. The aim of this article is to provide an overview of this therapeutic option and its rationale, based on an original chairside clinical case performed in a single session. MATERIALS AND METHOD: A clinical case involving a 20-year-old female patient is presented. The patient exhibited a missing mandibular molar due to extraction f

Prosthetic rehabilitation for a patient treated for embryonal rhabdomyosarcoma.

A female patient, now aged 17 years, was diagnosed with rhabdomyosarcoma (RMS) in the right pterygopalatine fossa when she was 3 years old. The RMS was successfully treated by excision, but the subsequent radiation and polychemotherapy resulted in the complete anesthesia of the distribution area of the right trigeminal nerve and loss of vision in the right eye. The patient also experienced pain in the mandibular joints and masticatory muscles. Panoramic radiographs displayed a multiple agenesia of the permanent teeth and underdeveloped apices. Treatment involved the fabrication of a complete m

Multidisciplinary management of limited interocclusal space: a clinical report.

Prosthetic management of partial edentulism can be challenging with the presence of limited interocclusal space. The extrusion of opposing teeth combined with the alveolar extrusion of the edentulous areas reduces the space needed for fabricating a removable or fixed prosthesis when edentulous areas are present in the maxilla. This clinical report describes the treatment provided to a patient who presented with a limited interocclusal space on the posterior right quadrant. Before prosthetic rehabilitation, mandibular right posterior teeth were intruded, and the maxillary right posterior alveol

Can a unilateral total temporomandibular joint prosthesis affect the healthy contralateral temporomandibular joint? A systematic review.

Temporomandibular joint (TMJ) prostheses are considered an important alternative for the treatment of severe end-stage TMJ disorders. However, unilateral TMJ replacement may affect the healthy contralateral TMJ. The purpose of this study was to systematically review the literature on the effects of unilateral total TMJ prosthesis placement on the contralateral healthy TMJ. The PubMed, Scopus, Web of Science, and Cochrane Library databases were searched for English-language articles published up to December 2023. Inclusion criteria encompassed clinical studies (randomized, prospective, retrospe
