Comparison of customized plate navigation and traditional methods in mandibular reconstruction: enhanced accuracy and efficiency.

This randomized controlled trial evaluated the three-dimensional (3D) accuracy of condylar positioning in mandibular reconstruction using pre-bent plates with customized plate navigation (CPN) compared with the traditional method (TM). Thirty patients with mandibular tumours were randomized to the CPN or TM group (15 each). Pre- and postoperative computed tomography quantified deviations in intercondylar distance and angulation, 3D displacement of the most superior point (assessed in superior-inferior, anterior-posterior, and medial-lateral directions on the affected condyle), and affected con…

A soft tissue-sparing positioning guide for the DCIA flap in mandibular reconstruction: a pilot study.

BACKGROUND: Precise placement of bone grafts is critical for functional and aesthetic outcomes in mandibular reconstruction. Conventional positioning guides typically require extensive stripping of periosteum and surrounding soft tissues, which may increase surgical trauma. This pilot study aimed to assess the feasibility and preliminary clinical performance of a novel soft tissue-sparing positioning guide for mandibular reconstruction. METHODS: This single-arm case series feasibility study included five female patients with segmental mandibular defects. Preoperative computed tomography data …

Mandibular Reconstruction With a Patient-Specific Implant Following Surgical Excision of an Acanthomatous Ameloblastoma in a Dog.

Canine acanthomatous ameloblastoma (CAA) is an invasive benign epithelial odontogenic tumour most commonly affecting the mandible of large breed dogs. To the author's knowledge, this report describes the first computer-aided design patient-specific implant (PSI) that has been placed for a critical sized bone defect in mandibular reconstruction of a dog in the UK. The aim was to restore mandibular stability using a regenerative approach combining a titanium locking plate and compression-resistant matrix infused with recombinant human bone morphogenetic protein-2 (rhBMP-2) to bridge the 85 mm ma…

A rare paediatric mandibular desmoid tumour: emphasis on growth-conscious resection and reconstructive strategy.

Desmoid-type fibromatosis (DTF) of the mandible is a rare, benign histological tumour that behaves aggressively in paediatric patients. We report the case of a 15-year-old boy with a 1-year history of a painless mandibular mass resulting in facial asymmetry. Imaging revealed a large, well-defined lesion of the right mandibular ramus and angle. Incisional biopsy confirmed DTF. Surgical resection was performed via hemimandibulectomy with preservation of the condyle. Immediate reconstruction was achieved using a titanium plate, preserving both facial aesthetics and oral function. Histopathologica

The fibula osteoseptocutaneous flap: evolution in concepts, techniques, and technologies during mandibular reconstruction - a review.

The fibula-free flap has evolved from its initial description to be a reliable workhorse-free flap providing bone, soft tissue, and a reliable skin paddle. The senior author has been refining this technique since the mid-1980s and has personally performed over 950 hundred cases of the fibula-free flap. The following paragraphs detail an evolution in surgical concepts related to this technique's refinement and serves as a roadmap detailing contemporary mandibular reconstruction.

Prevalence of free flap failure in mandibular osteoradionecrosis reconstruction: a systematic review and meta-analysis.

Our study aimed to estimate the prevalence of total free flap failure following free flap reconstruction for mandibular osteoradionecrosis (mORN) and assess the impact of potential moderators on this outcome. A comprehensive systematic literature search was independently conducted by two reviewers using the Medline, Scopus, Web of Science and Cochrane Library databases. Quality assessment of the selected studies was performed, and prevalence estimates with 95% confidence intervals (CI) were calculated. Outlier and influential analyses were conducted, and meta-regression analyses was employed t

Hybrid registration of the fibula for electromagnetically navigated osteotomies in mandibular reconstructive surgery: a phantom study.

PURPOSE: In mandibular reconstructive surgery with free fibula flap, 3D-printed patient-specific cutting guides are the current state of the art. Although these guides enable accurate transfer of the virtual surgical plan to the operating room, disadvantages include long waiting times until surgery and the inability to change the virtual plan intraoperatively in case of tumor growth. Alternatively, (electromagnetic) surgical navigation combined with a non-patient-specific cutting guide could be used, requiring accurate image-to-patient registration. In this phantom study, we evaluated the accu

The use of TMJ Concepts prostheses to reconstruct patients with major temporomandibular joint and mandibular defects.

Temporomandibular joint (TMJ) reconstruction may be required in complex cases in which there are additional mandibular or zygomatic arch defects. The reconstructive options include autogenous tissue, alloplastic material, or combinations of these. The authors describe 4 cases in which TMJ reconstruction was performed with TMJ Concepts customized joint prostheses. The prosthetic components were designed to restore major defects in the zygomatic arch and the mandibular ramus and body, including one case in which the mandibular component was used to restore total mandibular continuity. The prosth
