Clinical anatomy of the accessory mandibular foramen: application to mandibular ramus osteotomy.

PURPOSE: An accessory foramen around the mandibular foramen is called an accessory mandibular foramen (AMF). The clinical anatomy of the AMF has not been well described. The aim of this study was to reveal the clinical anatomy of the AMF for a better understanding of its implication during ramus surgeries. METHODS: Twenty-two sides fresh-frozen cadaveric heads with a mean age of 76.2 ± 14.4 years at death underwent dissection. The neurovascular bundles passing through the AMF were observed. Additionally, a hemi-face of a latex injected embalmed cadaver was dissected medially to laterally and

A novel technique to improve postoperative stability of orthognathic surgical anterior open bite correction using temporary anchorage devices: a case report.

BACKGROUND: Postoperative stability is a significant problem in the orthognathic management of anterior open bite malocclusion. The general tendency of modern preadjusted fixed appliances is towards unwanted and unplanned extrusion of the maxillary incisor and canine teeth as the dental arch is levelling. Following surgical repositioning of the jaws, the relapse potential of the extruded anterior dentition will be to intrude, leading to some reopening of the surgically corrected anterior open bite. CASE PRESENTATION: A 19-year-old male white Caucasian patient presented with a clinically signi

Synergistic benefits of hyperbaric oxygen therapy for postoperative palatal oro-nasal fistula following segmental Le Fort 1 osteotomy.

Segmental maxillary osteotomy, a surgical technique used in orthognathic surgery, involves the risk of palatal mucosa tearing, which can lead to severe complications. Herein, we report the case of a woman with Angle class II malocclusion who underwent multiple elective segmental maxillary osteotomy and augmentation genioplasty procedures for the correction of her malocclusion and the enhancement of her facial profile. A week after surgery, the patient developed a palatal ulcer. We adopted a comprehensive treatment approach involving 14-day hyperbaric oxygen therapy and conventional modalities;…

A surgery-first approach using single-jaw rotational mandibular setback in low-angle mandibular prognathism.

For the treatment of low-angle mandibular prognathism, rotational mandibular setback surgery is usually performed with Le Fort I maxillary osteotomy to rotate the maxillomandibular complex simultaneously. However, this maxillary surgery can be replaced with the orthodontic intrusion of maxillary posterior teeth. Single-jaw rotational mandibular setback surgery can be done with a surgery-first approach by planning orthodontic rotation of the maxillary occlusal plane with the simulation of the postsurgical forward mandibular rotation. This case report describes this approach applied to a 19-year…

New approach for the surgico-orthodontic treatment of hemifacial microsomia.

One of the important clinical findings in hemifacial microsomia (HFM) is facial asymmetry (FA) with unilateral underdevelopment and malformation of the mandible and/or maxilla. Unilateral distraction osteogenesis (UDOG) of the mandible has been used for correction of FA in HFM patients. However, return to the original FA status often occurs because of contraction of the distracted bony segment of the mandible and insufficient compensatory downward growth and skeletal midline correction of the maxilla on the affected side. Although bimaxillary UDOG procedure in the maxilla with Le Fort I osteot

Anatomic basis of Le Fort 1 impaction osteotomy: a radiological study.

PURPOSE: In Le Fort 1 osteotomy when a maxillary impaction is necessary, surgeons have to face different anatomical problems. (1) To determine the best bone resection route, they have to consider the situation of dental roots, infraorbital foramen and maxillary artery. (2) In case of Le Fort 1 osteotomy combined with a mandibular sagittal split osteotomy, the palate has to be replaced in horizontal position although there is no anatomical landmark. (3) In case of Gummy smiles, it can be due to either long face or short upper lip. The main objective was to identify safe bony landmarks to perfor

An orthodontic-orthognathic patient with obstructive sleep apnea treated with Le Fort I osteotomy advancement and alar cinch suture combined with a muco-musculo-periosteal V-Y closure to minimize nose deformity.

In some severe skeletal Class III patients, mandibular setback surgery using sagittal split ramus osteotomy (SSRO) is performed to correct mandibular protrusion. However, in patients diagnosed with obstructive sleep apnea syndrome (OSAS), the risk of OSAS worsening as a result of the SSRO is very high. Maxillary advancement could reduce the degree of mandibular retropositioning and expand the skeletal framework in the pharyngeal region, leading to enlargement of the airway. However, nasal deformity is an undesirable outcome of the procedure. This case report describes a 23-year-old man with a

Three-dimensional assessment of facial soft-tissue asymmetry before and after orthognathic surgery.

We developed a new three-dimensional (3D) method of analysis of facial asymmetry and applied it to 44 patients: 20 Class III cases treated by bimaxillary osteotomy; 12 Class III cases treated by maxillary advancement alone, and 12 Class II cases treated by bimaxillary operations. 3D images were taken within a week before operation (T1), and one (T2), three (T3), and six (T4) months after operation. Landmarks were digitised on each 3D model and facial asymmetry scores were calculated. In the bimaxillary osteotomy group, facial symmetry improved after operation. In the maxillary advancement grou…
