An evaluation of the relationship between maxillary sinus anterior wall depression and maxillary sinus opacity by computed tomography and panoramic radiography.

OBJECTIVE: Maxillary sinus anterior wall depression (AWD) has been associated with patterns such as the presence of panoramic innominate line and silent sinus syndrome. The aim of this study was to evaluate the relationship between AWD of maxillary sinus and both maxillary sinus opacity and the presence of innominate line using computed tomography (CT) and panoramic radiography (PR). MATERIALS AND METHODS: 250 CT and PR images (122 female, 128male) which had been taken for various reasons were randomly selected from the radiology archive of the our department. Innominate line visibility in PR…

Incidental treatment of maxillary dysesthesia with maxillary sinus lift procedure--a case report.

A 54-year-old woman presented with an 8-month history of dysesthesia to the left palate with referred pain to the left infraorbital distribution after extraction of the left maxillary first and second premolars. The clinical and radiologic examination revealed an alveolar crestal bone dehiscence suggestive of an occult oral antral communication (OAC) that had spontaneously healed. Stimulation of the mucosa at this bony dehiscence consistently reproduced the dysesthesia. A sinus lift procedure was performed in this region for implant site development and also resulted in resolution of the dyses

Inverted Maxillary Third Molar Impaction within the Maxillary Sinus: A Rare Case Report.

BACKGROUND The maxillary third molar is considered one of the most impacted teeth and can present in different types based on the direction and depth of the impaction. However, the inverted type of maxillary third molar impaction is a rare condition, with few reported cases in the current literature. CASE REPORT We report an extremely rare case of a 48-year-old man with an inverted maxillary third molar situated in the maxillary sinus in direct contact with the posterior wall of the sinus, with a portion of the coronal part perforating the infratemporal fossa. Because of the anatomical and str…

Evaluation of maxillary sinus expansion in children due to maxillary first molar extraction.

PURPOSE: This study is aimed at understanding the effects of maxillary first molar extraction on the expansion of maxillary sinus in children. SUBJECTS AND METHODS: 119 patients (aged 11-17 years) who had only one extracted maxillary first molar were included in the study. The superoinferior differences of the sinus floor position were measured in both dentate and edentulous sites on panoramic radiographs. The expansion of the maxillary sinus after maxillary first molar extraction was investigated in relation to fixed anatomic structures. The interorbital line (IL) and two zygomatic process l…

[Effect of severe periodontitis of maxillary molars on the maxillary sinus and the treatment principles of periodontitis related maxillary sinusitis].

Maxillary sinus is located in the maxilla bone with the largest volume in all sinuses and is closest to the roots of maxillary posterior teeth, especially the mesiobuccal root of the maxillary second molar. The thickness of the normal maxillary sinus mucosa is about 1 mm. When stimulated or infected, it shows thickening of the mucosa. Periodontitis is one of the most common oral diseases in human beings, and the complex root anatomy of the maxillary molars aggravates the difficulty of inflammation control. Severe periodontitis of maxillary molars without effective treatment can affect the maxi…

The impact of maxillary dimensions on determining surgical approach of fungal ball in the maxillary sinus.

Endoscopic middle meatal antrostomy (MMA) is commonly used for maxillary sinus (MS) fungal ball removal. For challenging cases involving anterior or inferior recess, an additional inferior meatal approach (IMA) might be needed. We analyzed the differences in MS dimensions on CT scans according to the surgical approach to suggest preoperative variables that could facilitate an additional IMA. CT scans of 281 adult patients who underwent ESS for the MS fungal ball (139 MMA, 62 MMA & IMA) were evaluated for comparative analysis of 8 MS measurements based on the surgical approach. Complete removal…

Is maxillary sinusitis and radiographic maxillary sinus opacification associated with an altered microbiology of MRONJ?

OBJECTIVE: Maxillary sinusitis can be a sequela of medication-related osteonecrosis of the jaw (MRONJ). This study aims to characterize the microbiome of maxillary MRONJ with concurrent maxillary sinusitis and radiographic maxillary sinus opacification to determine if there is a relationship between the microbiome of MRONJ and sinus disease. STUDY DESIGN: This retrospective case series was conducted using electronic health records from the University of Pennsylvania and affiliated hospitals. The target population was surgically managed maxillary MRONJ patients. The primary predictor variables…

Three dimensional evaluation of the effect of rapid maxillary expansion on maxillary sinus volume.

AIM: To compare the maxillary sinus volume (MSV) of posterior cross bite (PCB) patients before and after rapid maxillary expansion (RME) by cone-beam computed tomography (CBCT). METHODS: A total of 24 patients' CBCT scans that underwent RME therapy for their orthodontic treatment were used. CBCTs were obtained before (T1), immediately after (T2), and 6 months after (T3) RME. MSVs were calculated by using the 3D module of Dolphin 11.0 software. Repeated measures ANOVA (RMANOVA) was used to identify the MSV differences. RESULTS: The results showed no significant differences between the right a…

Application of the endoscopic prelacrimal recess approach to the maxillary sinus in unilateral maxillary diseases.

BACKGROUND: The endoscopic endonasal prelacrimal recess approach to the maxillary sinus provides wide access to the walls and recesses of the maxillary sinus, and its use has been reported in many maxillary sinus and skull base diseases. The objective of this study was to determine the indication and feasibility of endoscopic sinus surgery using the prelacrimal recess approach in unilateral maxillary sinus diseases. METHODS: We reviewed 192 cases of unilateral maxillary sinus diseases; over 2 years, 15 cases underwent endoscopic surgery via the prelacrimal recess approach. Data regarding preo…

Anatomic (positional) variation of maxillary wisdom teeth with special regard to the maxillary sinus.

The removal of wisdom teeth is one of the most common interventions in oral surgery. In order to avoid complications, a profound knowledge of the anatomy of teeth and adjacent tissues is crucial. In the case of maxillary wisdom teeth, their relationship to the maxillary sinus, to the pterygoid fossa, to the maxillary tuber and the adjacent venous plexus is particularly important. Three-dimensional (3D) imaging, for example by means of cone beam computed tomography (CBCT), is increasingly utilized in practice. However, the necessity of CBCT imaging is still a matter of intensive debate. The aim…

Endoscopic removal of a maxillary third molar displaced into the maxillary sinus via the socket.

Removal of a maxillary third molar is a common dental procedure, and oral surgeons rarely experience accidental displacement of a maxillary impacted third molar into the maxillary sinus as a complication of this procedure. In the case in which such displacement occurs, the molar is removed via a transantral (Caldwell-Luc) approach through bone removal or window osteotomy of the anterior wall of the maxillary sinus. However, this approach is highly invasive because 2 surgical fields are required at the socket and the anterior wall of the maxillary sinus. Here, we report as a minimally invasive …

[Acquired enophthalmos associated with hypoplasia of the maxillary sinus and asymptomatic chronic maxillary sinusitis].

BACKGROUND: We report a case of progressive enophthalmos, maxillary sinus hypoplasia and radiological signs of sinusitis in a young woman. CASE REPORT: The patient had no history of sinus surgery or facial trauma. There were no signs of sinusitis. Functional endoscopic sinus surgery was proposed to restore the communication between the nose and the antrum. DISCUSS: The particular aspects of asymptomatic maxillary sinus and causes of maxillary sinus hypoplasia are discussed. We suggest that obstruction of the osteomeatal complex causes negative pressure in the maxillary sinus provoking bone al…

Maxillary sinusitis caused by dental implants extending into the maxillary sinus and the nasal cavities.

This report describes the case of a patient who underwent osseointegrated dental implant placement. The implants were misplaced inside the nasal fossae and in the right maxillary sinus, causing chronic purulent sinusitis. CT scan without contrast showed signs of right maxillary sinusitis and confirmed the misplacement of four dental implants that surfaced into the nasal cavities. The imaging also revealed the presence of another implant that emerged inside the maxillary sinus. The patient underwent functional endoscopic sinus surgery with complete symptom remission at the long-term follow-up. …

Ectopic eruption of a maxillary third molar tooth in the maxillary sinus: a case report.

Ectopic eruption of teeth into regions other than the oral cavity is rare although there have been reports of teeth in the nasal septum, mandibular condyle, coronoid process, palate, chin, and maxillary sinus. Occasionally, a tooth may erupt in the maxillary sinus and present with local sinonasal symptoms attributed to chronic sinusitis. We present a case of an ectopic maxillary third molar tooth that caused chronic sinusitis in the maxillary sinus.

[The value of maxillary sinus irrigation in children with maxillary sinusitis using the Waters film].

Eighty children with chronic sinusitis receiving a conservative treatment were followed. They found that without any further therapy the Waters view of 30 children became normal. The usefulness of irrigation of the maxillary sinus was studied in the other 50 children. In one group of 25 children a sinus endoscopy and sinus irrigation was performed. In a control group of another 25 children no further treatment was performed. A comparison between the Waters view and sinus endoscopy is also included in the study. The authors found little difference in the treatment results in the two groups. Fur…

Penetration of erythromycin stearate into maxillary sinus mucosa and secretion in chronic maxillary sinusitis.

The penetration of oral erythromycin stearate (Abboticin), administered in a dosage of 500 mg three times a day, into the maxillary sinus mucosa and secretion was studied in 15 patients (22 sinuses) operated on for chronic maxillary sinusitis. The average concentration in serum was 2.3 microgram/ml, 1.2 microgram/ml in secretion, and 1.8 microgram/ml in mucosa. These concentrations are highly effective against diplococci and most aerobic and anaerobic streptococci (MIC value 0.06 microgram/ml) but not against Haemophilus influenzae (MIC value for 80% of 2 microgram/ml).
