Invited Commentary: Medicaid Policy and Pregnancy Outcomes-Toward a Reproductive Autonomy Framework.

The Medicaid program covers more than 70 million people and is the largest single health insurance payer for pregnancy and delivery in the United States. In this issue of the Journal, Margerison et al. (Am J Epidemiol. 2021;190(8):1488-1498) investigate the extent to which expansion of Medicaid coverage to nonpregnant low-income adults under the Affordable Care Act may have improved prepregnancy or birth outcomes. They found that Medicaid expansions by states were not associated with changes in prepregnancy health, including smoking and obesity. Similarly, there were no changes in preterm birt…

Postpartum Care Utilization Among Women with Medicaid-Funded Live Births in Oregon.

INTRODUCTION: Postpartum care is an important strategy for preventing and managing chronic disease in women with pregnancy complications (i.e., gestational diabetes (GDM) and hypertensive disorders of pregnancy (HDP)). METHODS: Using a population-based, cohort study among Oregon women with Medicaid-financed deliveries (2009-2012), we examined Medicaid-financed postpartum care (postpartum visits, contraceptive services, and routine preventive health services) among women who retained Medicaid coverage for at least 90 days after delivery (n = 74,933). We estimated postpartum care overall and am…

COVID-19 and Routine Childhood and Adolescent Immunizations: Evidence from Louisiana Medicaid.

The COVID-19 pandemic disrupted routine vaccinations for children and adolescents. However, it remains unclear whether the impact has been different for children and adolescents from low-income families. To address this, we compared monthly routine vaccination use per 1000 vaccine-eligible children and adolescents enrolled in Louisiana Medicaid in the years before (2017-2019) and during the COVID-19 pandemic (2020). Compared to the 2017-2019 average vaccination rates, we found a 28% reduction in measles, mumps, and rubella (MMR), a 35% reduction in human papillomavirus (HPV), and a 30% reducti…

Financial incentives to promote cardiac rehabilitation participation and adherence among Medicaid patients.

PURPOSE: Cardiac rehabilitation (CR) improves medical outcomes after myocardial infarction or coronary revascularization. Lower socioeconomic status (SES) patients are less likely to participate in and complete CR. The aim of this study was to test whether financial incentives may increase participation and adherence to CR among lower-SES patients. METHODS: Patients eligible to participate in CR with Medicaid insurance coverage were approached for inclusion. Patients were placed on an escalating incentive schedule of financial incentives contingent upon CR attendance. CR participation was com…

Smoking among pregnant women with Medicaid insurance: are mental health factors related?

Smoking during pregnancy is the single most modifiable risk factor for poor birth outcomes, yet it remains prevalent among low-income women. This study examined factors associated with continued smoking and quitting among pregnant women. A total of 2,203 Medicaid-eligible pregnant women were screened at their first enhanced prenatal services visit for risk factors including demographics, health behaviors (smoking, alcohol and drug use), mental health (history of mental health disorders, current depressive symptoms), and stress. Smoking status was divided into non-smokers, quitters (quit smokin…

Healthcare costs within a year of respiratory syncytial virus among Medicaid infants.

Limited research exists on the economic impact of respiratory syncytial virus lower respiratory infection (RSV LRI) among vulnerable infant populations. This study evaluated healthcare costs of full-term and late-preterm Medicaid infants with RSV LRI within 1 year of infection. Medicaid administrative claims were used to conduct a retrospective study of infants born 2003-2005. Full-term and late-preterm infants <1 year old were assigned to groups based on RSV LRI and unspecified bronchiolitis/pneumonia (UBP) diagnosis codes and stratified by setting of diagnosis. Infants without evidence of RS…

Evaluating the role of Medicaid expansion on survival in early-onset colorectal cancer.

BACKGROUND: The Affordable Care Act expanded Medicaid eligibility to states on staggered timelines, creating a natural experiment to assess health policy effects on colorectal cancer (CRC) outcomes. METHODS: We conducted a retrospective quasi-experimental cohort study using the Surveillance, Epidemiology, and End Results database (2006-2019). Patients with CRC <65 years were included in primary analyses; an early-onset colorectal cancer (EOCRC) cohort aged <50 years was examined. States were classified by Medicaid expansion timing, and each expansion group was propensity score-matched against…

National Trends in Child and Adult Medicaid Coverage and Reimbursement for Endodontic Procedures.

INTRODUCTION: Medicaid is the largest source of health coverage in the United States and provides dental benefits to eligible low-income families. Federal policies establish minimum dental service requirements for children, while states have flexibility regarding the coverage of dental services for adults in any capacity. The objective of this study was to evaluate endodontic procedure coverage, reimbursement, and dentist participation for Medicaid beneficiaries. METHODS: This retrospective, cross-sectional study evaluated child and adult dental Medicaid benefits in 2021. For children, 18 end…

Epilepsy, Intellectual and Developmental Disabilities, and Contraception in U.S. Medicaid Claims Data.

OBJECTIVE: To evaluate how epilepsy and intellectual and developmental disabilities (IDD) affect contraceptive provision among females with Medicaid insurance. METHODS: We obtained Medicaid claims data from 2016-2021 for females aged 12-51 years. We excluded individuals with diagnosis codes for menopause and those who underwent sterilization in 2016-2017. Using a validated definition of epilepsy, we identified females who met the criteria for epilepsy in 2016-2017, and we identified a comparison group of females enrolled in 2016-2017 who never met the criteria for epilepsy. We used Internatio…

Postpartum Patient Perspectives on the US Medicaid Waiting Period for Permanent Contraception.

OBJECTIVE: The Medicaid Consent to Sterilization policy is a known barrier to permanent contraception (PC) fulfillment and is associated with disparities in fulfillment. While physician perspectives regarding the policy are well described, knowledge of how patients with Medicaid seeking PC perceive this waiting period is limited. STUDY DESIGN: We interviewed 81 participants with a documented desire for PC at discharge from their hospital-based delivery at four medical centers across the United States. Interviews were audio-recorded, transcribed, and analyzed using rapid qualitative methodolog…

Landscape of Emergency Medicaid and Health Care Coverage for Undocumented Immigrants in the US.

IMPORTANCE: Undocumented immigrants represent a disproportionate share of uninsured individuals in the US, owing to federal restrictions that limit access to affordable health care coverage. Emergency Medicaid (EM) and other state-sponsored programs offer mechanisms for receiving necessary medical care, but their scope and provisions are poorly understood by patients, clinicians, and policymakers alike. OBJECTIVE: To examine the national policy landscape of EM and other state programs designed to provide health care coverage for undocumented immigrants. EVIDENCE REVIEW: Between January 2024 …
