Utilization and access to metered-dose inhalers and sustainable alternatives among older adults in the United States: a population-based study.

BACKGROUND: Attaining the Paris Agreement goal of limiting global temperature rise to below 1.5-2 °C requires the healthcare sector to achieve net-zero emissions by 2050. This necessitates adopting diverse strategies, including replacing carbon-intensive metered-dose inhalers (MDIs) with more environmentally sustainable dry powder inhalers (DPIs) and soft mist inhalers (SMIs). While several European countries have successfully made this transition, patterns of MDI use and barriers to adopting sustainable alternatives in the United States remain poorly understood. Thus, we assessed inhaler util

Clinical and economic burden of chronic kidney disease in Medicare Fee-for-Service beneficiaries with and without comorbid type 2 diabetes and heart failure: A retrospective cohort study.

BACKGROUND: Chronic kidney disease (CKD) is common in older adults and is often associated with type 2 diabetes (T2DM) and heart failure (HF). However, little is known about the burden of newly diagnosed CKD in Medicare Fee-for-Service (FFS) beneficiaries, including those with comorbid T2DM or HF. OBJECTIVE: To quantify the clinical and economic burden of CKD in Medicare FFS beneficiaries, including those with comorbid T2DM or HF. METHODS: In this retrospective cohort study using 100% Medicare FFS claims data (Parts A, B, D) from 2014 to 2022, beneficiaries with incident CKD (based on a diag

Association Between Changes in CMS Reimbursement Policy and Drug Labels for Erythrocyte-Stimulating Agents With Outcomes for Older Patients Undergoing Hemodialysis Covered by Fee-for-Service Medicare.

IMPORTANCE: In 2011, the US Centers for Medicare & Medicaid Services (CMS) changed its reimbursement policy for hemodialysis to a bundled comprehensive payment system that included the cost of erythrocyte-stimulating agents (ESAs). Also in 2011, the US Food and Drug Administration revised the drug label for ESAs, recommending more conservative dosing in patients with chronic kidney disease. In response to concerns that these measures could have adverse effects on patient care and outcomes, the CMS and the FDA initiated a collaboration to assess the effect. OBJECTIVE: To assess the effects of

A case study in generic drug use: should there be risk adjustment in incentive payments for the use of generic medications?

BACKGROUND: Encouraging generic drug use has reduced health care costs for payers and consumers, but the availability of therapeutically interchangeable medications or generic medications of choice is not equal across disease states. The extent to which systems of care are able to substitute with generics is not well understood. OBJECTIVES: To (a) define and measure the maximum generic rate (MGR) of currently prescribed drugs within an academic medical group in and (b) illustrate differences across drugs associated with selected underlying diseases. METHODS: Prescription claims data were exa

Beyond comorbidity: expanding the definition and measurement of complexity among older adults using administrative claims data.

BACKGROUND: Studies of patients with multiple chronic conditions using claims data are often missing important determinants of treatments and outcomes, such as function status and disease severity. We sought to identify and evaluate a class of function-related indicators (FRIs) from administrative claims data. POPULATION: The study cohort comprised US Medicare beneficiaries aged 65 years or older with Parts A and B fee-for-service and Part D coverage, with a hospitalization for acute myocardial infarction during 2007. METHODS: Measures during the year before admission included the FRIs, demo

Clinical Factors and Social Determinants of Health Predict Elder Mistreatment at Two Years Among Older Adults: A Preliminary Prediction Model.

Low rates of screening for elder mistreatment preclude early detection and harm reduction. No validated tools currently exist to identify elder mistreatment with high sensitivity. We developed, internally validated, and tested a preliminary prediction model to improve elder mistreatment screening of older adults. This retrospective observational study used 20% national Medicare Fee-for-Service claims data for the logistic regression and machine learning approaches (random forest, gradient boosted decision tree, and multilayer perceptron classifier). We included beneficiaries aged 66 and older

Air Pollution and Cardiovascular and Thromboembolic Events in Older Adults With High-Risk Conditions.

Little epidemiologic research has focused on pollution-related risks in medically vulnerable or marginalized groups. Using a nationwide 50% random sample of 2008-2016 Medicare Part D-eligible fee-for-service participants in the United States, we identified a cohort with high-risk conditions for cardiovascular and thromboembolic events (CTEs) and linked individuals with seasonal average zip-code-level concentrations of fine particulate matter (particulate matter with an aerodynamic diameter ≤ 2.5 μm (PM2.5)). We assessed the relationship between seasonal PM2.5 exposure and hospitalization for e…

Thrombopoietin receptor agonist discontinuation rates and reasons among patients with immune thrombocytopenia: a study of administrative claims linked with medical chart review.

Administrative claims provide a rich data source for retrospective studies of real-world clinical practice, yet some important data may be inconsistent or unavailable. This study explored factors influencing discontinuation of thrombopoietin receptor agonists (TPO-RAs) among patients with immune thrombocytopenia (ITP), by adding medical chart abstraction for additional details. Adult (≥ 18 years) patients with continuous commercial or Medicare Advantage with Part D health insurance coverage were included. Inclusion criteria were ≥ 1 claim for eltrombopag or romiplostim and ≥ 2 diagnoses of ITP

Healthcare resource utilization patterns among patients with Parkinson's disease psychosis and dementia: analysis of US Medicare beneficiaries treated with pimavanserin versus other-atypical antipsychotics or versus quetiapine.

BACKGROUND: Pimavanserin (PIM) is the only FDA approved atypical antipsychotic treatment (AAP) for hallucinations and delusions associated with Parkinson's disease psychosis (PDP) among patients with or without coexisting dementia; however, Other-AAPs (i.e. quetiapine (QUE), risperidone, olanzapine, aripiprazole) are commonly prescribed off-label. Healthcare resource utilization (HCRU) patterns among patients with PDP and coexisting dementia (PDP+D) who newly initiate PIM versus (vs.) Other-AAPs (i.e. other AAP-mix) or QUE in real-world settings is limited. METHODS: A retrospective analysis o

Evaluating Cost-Effectiveness in Using High-Kidney Donor Profile Index Organs.

A more granular donor kidney grading scale, the kidney donor profile index (KDPI), has recently emerged in contradistinction to the standard criteria donor/expanded criteria donor framework. In this paper, we built a Markov decision process model to evaluate the survival, quality-adjusted life years (QALY), and cost advantages of using high-KDPI kidneys based on multiple KDPI strata over a 60-month time horizon as opposed to remaining on the waiting list waiting for a lower-KDPI kidney. Data for the model were gathered from the Scientific Registry of Transplant Recipients and the United States

Computer-aided diagnosis: the emerging of three CAD systems induced by Japanese health care needs.

The aim of this paper is to describe three emerging computer-aided diagnosis (CAD) systems induced by Japanese health care needs. CAD has been developing fast in the last two decades. The idea of using a computer to help in medical image diagnosis is not new. Some pioneer studies are dated back to the 1960s. In 1998, the first U.S. FDA (Food and Drug Administration) approved commercial CAD system, a film-digitized mammography system, was launched by R2 Technologies, Inc. The success was quickly repeated by a number of companies. The approval of Medicare CAD reimbursement in the U.S. in 2001 fu

Spending by the Centers for Medicare & Medicaid Services Before and After Confirmation of Benefit for Drugs Granted US Food and Drug Administration Accelerated Approval, 2012 to 2017.

IMPORTANCE: Accelerated approval by the US Food and Drug Administration (FDA) grants market authorization for drugs based on clinical trials using surrogate end points likely to anticipate a clinical benefit. The FDA requires postapproval trials to confirm benefit, after which the accelerated approval is converted to a standard approval or is withdrawn. However, trials frequently fail to assess clinical benefit, and expenditure for these drugs may be substantial. OBJECTIVE: To evaluate spending by the Centers for Medicare & Medicaid Services (CMS) on drugs granted FDA accelerated approval bef
