Lesser metatarsophalangeal joint (MTPJ) instability is a common cause of metatarsalgia and is associated with plantar plate pathology. The instability is often due to chronic attenuation and attritional changes of the plantar plate and collateral ligaments, leading to eventual rupture. The plantar plate and collateral ligaments serve as the primary stabilizers of the MTPJ, and rupture or attenuation of these ligaments leads to instability. Surgical intervention is often necessary for plantar plate tears, with various approaches and techniques available.

Hallux rigidus can be treated with a variety of surgical procedures, including joint preserving techniques, arthrodesis, and arthroplasty. The most commonly reported complications for joint preserving techniques consist of progression of arthritis, continued pain, and transfer metatarsalgia. Although good outcomes have been reported for arthrodesis overall, careful attention must be paid to technique and positioning of the toe to avoid nonunion or malunion. Arthroplasty preserves motion but in the case of failure can present the additional challenge of bone loss. In these scenarios, the author

BACKGROUND: Plantar pressure distribution after the first metatarsal proximal crescentic osteotomy (FMPCO) with lesser metatarsal proximal shortening osteotomy (LMPSO) for hallux valgus with metatarsalgia has not been previously described.
METHODS: The pre- (Pre) and postoperative (Post) groups comprised of 18 patients who underwent unilateral FMPCO with LMPSO; fifteen healthy volunteers constituted the control (C) group. For each of the 10 regions, peak pressure (Peak-P), maximum force (Max-F), contact time (Con-T), contact area (Con-A), and force-time integral (FTI) were measured.
RESULTS:

Brachymetatarsia involves a reduction in length of one or more metatarsals. The affected metatarsal is shortened by 5 mm or more, altering the normal metatarsal parabola. In addition to being an aesthetic deformity, it can present with pain due to transfer metatarsalgia. A possible association with genetic disorders needs to be investigated during clinical evaluation. Surgical treatment may involve a one-stage lengthening procedure or progressive distraction, each having its advantages and limitations.

In order to provide proper treatment intervention, the foot and ankle surgeon must develop a further understanding of the pathoanatomy and pathomechanics leading to specific surgical complications of central metatarsal osteotomies. In addition to providing the authors' experiences and potential solutions with regard to these complications, a clear definition of the progression of the complication course is presented. The specific complications of floating toe deformity, metatarsalphalangeal joint stiffness, recurrent metatarsalgia, transfer lesions, malunions, and nonunions are discussed.

As an extra-articular osteotomy, the distal metatarsal minimally invasive osteotomy (DMMO) affords the surgeon the ability to treat both static and propulsive metatarsalgia with predictable healing and a reduced risk of stiffness, scarring, and wound healing complications. The development of more advanced variations of this procedure expands the range of conditions that can be successfully treated by the surgeon. The percutaneous osteotomy is an alternative to open surgery that offers immediate weight bearing. The efficacy of the DMMO has been demonstrated by growing evidence, with satisfactor

BACKGROUND: Since metatarsal osteotomy was first used to treat metatarsalgia in the early twentieth century, many techniques have been described to accomplish the basic aim of reduction of load transmission through the operated metatarsal and reduction of localized high pressure on the plantar surface of the metatarsal. Our study examined two popular distal metatarsal neck osteotomies used for the relief of central metatarsalgia and the biomechanical changes that result from their use in a cadaver forefoot model.
METHODS: After applying 445 N (100 lbs) of axially directed force, we measured p

BACKGROUND: Hallux valgus deformities with large intermetatarsal angles require a more proximal metatarsal procedure to adequately correct the deformity. Due to the relative ease of a closing wedge osteotomy, this technique was adopted but with concern over first metatarsal shortening. In this study, we primarily evaluated angular correction and first metatarsal shortening.
MATERIALS AND METHODS: We evaluated 70 feet in 57 patients (average age, 54 years) with 52 female and five male. The average followup was 14 (range, 6 to 45) months. The charts were reviewed for the presence of metatarsalg

The Mitchell distal metatarsal stepcut osteotomy for the treatment of hallux valgus is a technically demanding procedure with a high complication rate. Reported complications include metatarsal shortening with lateral metatarsalgia, partial avascular necrosis of the distal fragment, and loss of position with incomplete correction. Nonunion, however, has only rarely been reported. We describe our experience with the treatment of three nonunions of Mitchell osteotomies reconstructed using a tricortical iliac crest bone graft.

Authors discuss, presenting case reports, the operative indications of Helal's metatarsal serial osteotomy. In their first case this kind of operation was performed because of metatarsalgia, following traumatic amputation of the hallux. In their second case, in a young female with rheumatoid arthritis, considering the age of the patient and the destruction in the metatarsophalangeal joints too, Helal's metatarsus osteotomies were completed with the resection of the basis of the ground phalanx. In both cases good correction was reached and without any complaints.

Patients with inflammatory arthritic disease of the feet invariably require conservative office management. The simplest method of obtaining relief has been to use commercially available extra-depth shoes combined with custommade, removable, closed-celled polyethylene foam arch supports. These supports are soft, light, and can be accurately adjusted to each patient's pathology. The technique and modifications used for relieving the commonest problems, such as anterior metatarsalgia and plantar heel pain, are discussed.

Primary care physicians often see patients who have foot pain. Although foot disorders may have many diagnostic possibilities, the majority can be explained via the pathologic biomechanics of hyperpronation and the resulting changes in the kinetic chain. Four common problems often associated with hyperpronation are plantar fasciitis, posterior tibial tendon dysfunction, metatarsalgia, and hallux valgus. Interventions that seek to reduce hyperpronation and strengthen foot muscles are often recommended for treating foot pain.

The authors have analysed a special disturbance of the statics and dynamics of the forefoot - deficient weight-bearing on the first metatarsal ray whilst walking. From the clinical point of view, the condition resulted in the development of a dorsal bunion and metatarsalgia. Forty-nine cases were treated and seven causative factors were discovered: Club foot, Paralysis, Congenital deformity, Fracture-dislocation of Lisfranc's joint, Sequelae of surgery for hallux valgus, Sequelae of subtaler arthrodesis, "Functional" abnormality.

A peculiar clinical presentation of post-traumatic complex left foot fracture deformity is presented in this report as the result of a motorbike accident. Notwithstanding the significant deformity following forefoot fractures, the patient complained only of the recent onset of metatarsalgia. Of particular interest, is that this severe foot injury as well following deformity was overlooked, probably because patient had sustained head injury that was the main problem to treat due to life risk.

A study of lower-extremity stress fractures is presented with emphasis placed on metatarsal stress fractures. The metatarsals were found to be the second most common site in the study. The study documents not only the number of metatarsal stress fractures found, but also the location within the affected metatarsal. Because stress fractures are a common problem, especially for the military recruit or athlete, metatarsal stress fractures can represent a common cause of metatarsalgia.

A review of the literature attests to a place for arthrodesis of the metatarsophalangeal joint of the great toe in the management of hallux valgus and rigidus. The predictability and consistency of the result of arthrodesis is noted with good results in approximately 90% of cases. The functional excellence and durability of such a result are impressive. The procedure is particularly indicated in patients with hallux rigidus and hallux valgus associated with metatarsalgia. There are a number of operative techniques, but the cone arthrodesis procedure described by Wilson is probably nearest to t

Subluxation or dislocation of the second metatarsophalangeal joint may be commonly associated with crossover toe, metatarsalgia, and painful calluses. This retrospective study aims to evaluate the clinical and functional results in patients with irreducible second metatarsophalangeal joint dislocation treated by double percutaneous osteotomy in one step: Haspell's osteotomy and Distal Metatarsal Mini-Invasive Osteotomy. A total of 39 patients were included in this study. 31 patients had a simultaneous procedure on the first ray for hallux valgus correction. The American Orthopaedic Foot and An

Rocker profile shoes (rocker shoes) are one of the treatment options of metatarsalgia and forefoot stress fractures. The efficacy of rocker shoes in unloading the forefoot pressure has been shown in walking. In running, however, the effect of rocker shoes on forefoot pressure is unknown. Eighteen healthy female runners participated in this study. In-shoe plantar pressures were recorded during running with the standard running shoes and rocker shoes. Shoe comfort was assessed after each shoe measurement. Peak pressure (PP), maximum mean pressure (MMP) and force-time integral (FTI) were determin
