Effect of lecithin coating on the pulmonary absorption of furosemide in rats.

The effect of lecithin coating on the pulmonary absorption of furosemide after application by metered dose inhalers (MDI) containing HFA 227 was evaluated in rats. The plasma concentration of furosemide after application of lecithin-coated furosemide was higher than that after application of the un-coated form. Since the disposition in the lung 2 min after application of un-coated furosemide was significantly lower than that after application of the coated form and the adsorption to a polyethylene tube used for the application of the un-coated form was significantly higher than that of the coa…

Recommendations for the management of COPD.

Three sets of guidelines for the management of COPD that are widely recognized (from the European Respiratory Society [ERS], American Thoracic Society [ATS], and British Thoracic Society [BTS]) are reviewed and compared. None of the documents uses classic evidence-based documentation, and, in many instances, the recommendations are empiric because of a lack of scientific evidence. Overall, there is strong agreement between the documents. All three guidelines recommend inhaled bronchodilators as first-line therapy. Anticholinergics are noted to be well tolerated, although potential problems wit

Prescribers' and users' perspective of CFCs: a market research survey.

Focus groups were conducted with physicians, pharmacists and patients prior to semi-structured interviews with physicians and patients. The survey was conducted in Spain, Germany and the UK. There were high levels of awareness of environmental issues, with ozone layer damage and global warming being of particular concern. Most respondents were aware of chlorofluorocarbon (CFC) use in aerosols and in refrigerators. However, their presence in asthma metered dose inhalers (MDIs) was much less widely known. German physicians had the highest awareness (78%) while only 1 in 10 of UK patients was awa…

In vitro characterisation of fluticasone propionate and ciclesonide delivered with and without valved holding chambers using cascade impaction and an adult simulated breathing profile.

BACKGROUND: Valved holding chambers (VHCs) are not interchangeable. Because VHC design and formulation type influence aerosol behaviour, each VHC-drug combination should be evaluated to inform clinical use and device selection. AIM: To compare the aerodynamic particle size distribution (APSD) of two inhaled corticosteroids delivered from pressurised metered dose inhalers with and without commonly used VHCs under realistic, adult-representative breathing. METHODS: We assessed fluticasone propionate (FP, suspension) and ciclesonide (CIC, solution) using no VHC and three VHCs (AeroChamber, Easy…

Consistent aerodynamic performance and content uniformity of mometasone-magnesium stearate co-processed aerosols for asthma therapy.

Pressurized metered-dose inhalers (pMDIs) are a common technology for delivering respiratory medications, but the stability of suspended formulations remains a significant challenge. Physical instability can lead to dosing inaccuracies and reduced therapeutic effectiveness. While various stabilization strategies exist, these may entail high costs, complex processing and/or high excipient loading. This study evaluated the impact of magnesium stearate (MgSt) incorporation on delivered dose content uniformity and aerodynamic performance of mometasone-based pMDI suspensions. Different processing s

Albuterol delivery during noninvasive ventilation.

HYPOTHESIS: Albuterol delivered during noninvasive positive-pressure ventilation is affected by use of a nebulizer or metered-dose inhaler (MDI) and whether the leak port is in the hose or the mask. METHODS: A lung model that simulated spontaneous breathing at 20 breaths/min was used. A bi-level positive-airway-pressure ventilator (Respironics S/T30) was set for an inspiratory positive airway pressure of 15 cm H(2)O and an expiratory positive airway pressure of 5 cm H(2)O. The tidal volume delivered was 0.4 L. Two mask types were used: one in which the leak port was incorporated into the circ

Inhaled salbutamol delivery in small children with disposable and reusable spacers: an in vitro study.

BACKGROUND: Research on spacers for paediatric breathing patterns is limited, especially for disposable types, which may be a practical alternative to reusable valved holding chambers (VHC) in certain clinical settings. METHODS: In vitro, the fine particle dose (FPD) of salbutamol from a pressurised metered-dose inhaler (pMDI) was tested using two paperboard spacers-DispozABLE (Diz) and LiteAire (LA)-and three reusable VHCs: AeroChamber Plus Flow-Vu (AC), EasyChamber (EC) and OptiChamber Diamond (OC). The pMDI+VHC setup was connected to a child throat model without a facemask. Salbutamol avai

Comparing 6-Year Carbon Footprint Between GINA Track 1 and 2 Asthma Management for Adults: A Real-World Primary Care Report from Singapore to Advocate for Policy Change in Inhaler Procurement.

BACKGROUND: Global Initiative for Asthma (GINA) has recommended inhaled corticosteroid (ICS)-Formoterol as Track-1 treatment for patients. Using ICS and SABA (Short-Acting Beta-Agonist) as a reliever is an alternative Track-2 option. Both modalities are tied to type of inhaler use, dry powder inhalers (DPI) and propellent-containing metered-dose inhalers (pMDI). Compared to propellent-free DPI, pMDI have much higher carbon footprint (CF) detrimental to the environment. Leveraging on dispensed inhaler data from the electronic medical records of patients managed in primary care, the study aimed

Inhaler Devices as Potential Bacterial Reservoirs: A 30-Day Microbiota Analysis in Non-Cystic Fibrosis Bronchiectasis.

Background/Objectives: Handheld inhalers are essential for the management of bronchiectasis, yet their role as potential microbial reservoirs remains poorly understood. We aimed to characterize and compare the bacterial communities in pressurized metered-dose inhalers (pMDIs) and Respimat® devices after 30 days of clinical use. Methods: In this 30-day prospective study of eight adults with non-cystic fibrosis bronchiectasis, inhaler devices (six pMDIs and four Respimat® devices) were analyzed using 16S rRNA gene sequencing of the V4 region. Total bacterial loads (quantitative PCR), α- and β-di

Evaluation of asthma control after reinforcement of proper inhaler techniques in a tertiary care center in northern India.

Asthma is an obstructive lung disease with high prevalence. Symptom control in asthma remains poor despite significant advancements in treatment guidelines and inhalational devices. This is often attributed to inadequate patient adherence to the inhaler technique and insufficient education on its long-term use. Through this study, we aimed to assess the impact of reinforcement of proper inhaler technique on asthma symptom control and quality of life. A prospective cohort of 400 asthma patients using dry powder inhalers and metered dose inhalers was recruited in the outpatient clinic of our hos…

[New aspects of use inhaled steroids in treatment of children with bronchial asthma].

The development of potent glucocorticoids (GC) delivered topically via metered dose inhalers (MDI) have revolutionized the way we care for asthmatic children. Steroids can improve asthma symptoms, baseline pulmonary function, and reduce bronchial hyperresponsiveness (BHR) to the extent no other asthma medication can. In addition, the fact that small quantities of GC are delivered topically, the incidence of adverse effects is greatly diminished. Inhaled GC therapy has become first line therapy for all children even with mild persistent asthma, and their early use in life is known as "early int…

[Inhalation chambers].

Since the last 30 years metered-dose pressurised aerosols have largely contributed to the treatment of asthma with inhaled beta-2 stimulants and corticosteroids. Two problems, however, are associated with these aerosols: (1) small amounts of active substance are deposited in the lungs and larger amounts in the mouth and pharynx; (2) aerosols are uneasy to use because of lack of coordination between actuation and inhalation and absence of apnoea at the end of inspiration. An elegant solution of these problems is the inhalation chamber: a reservoir inserted between the patient's mouth and the ae

Disposition of inhaled 1,1,1,2-tetrafluoroethane (HFA134A) in healthy subjects and in patients with chronic airflow limitation. Measurement by 18F-labeling and whole-body gamma-counting.

HFA134a (1,1,1,2-tetrafluoroethane) is a nonozone-depleting candidate to replace the chlorofluorocarbons used as propellants in metered-dose inhalers (MDIs) for pharmaceuticals that are widely used in the treatment of respiratory tract disease. As a means for ensuring the safety of such a compound for human use, it is necessary to establish that there is no excessive or unexpected accumulation in the body and in selected regions. A sensitive whole-body gamma-counting technique has been used with 18F-labeled HFA134a to measure the whole-body and regional absorption, distribution, and retention

Advances in asthma and COPD management: delivering CFC-free inhaled therapy using Modulite technology.

Inhaled corticosteroids (ICS) and long-acting beta-agonists (LABA) are currently used in the management of asthma and chronic obstructive pulmonary disease (COPD). Localized targeted delivery of these drugs into the lungs is achieved by means of two types of inhalation devices; pressurized metered-dose inhalers (pMDIs) and dry powder-inhalers (DPIs). For environmental reasons, the chlorofluorocarbon (CFC) propellants used in pMDIs are now being replaced by ozone friendly hydrofluoroalkanes (HFAs). These new generation HFA-based pMDIs, developed to provide effective lung deposition of the activ
