Focal EEG abnormalities in Heidenhain's variant of Jakob-Creutzfeldt disease.

Serial EEGs obtained during a six-week period from a patient with Heidenhain's variant of Jakob-Creutzfeldt disease demonstrated periodic complexes confined to the occipital regions that at no time became generalized. The focal character of the discharges correlated with the site of maximal disease in the occipital cortex, suggesting that cortical damage is a necessary substrate for the production of periodic complexes in Jakob-Creutzfeldt disease.

Mapping the changed hubs and corresponding functional connectivity in idiopathic restless legs syndrome.

OBJECTIVE: The hubs of the brain network play a key role in integrating and transferring information between different functional modules. However, whether the changed pattern in functional network hubs contributes to the onset of leg discomfort symptoms in restless legs syndrome (RLS) patients remains unclear. Using resting-state functional magnetic resonance imaging (rs-fMRI) and graph theory methods, we investigated whether alterations of hubs can be detected in RLS. METHODS: First, we constructed the whole-brain voxelwise functional connectivity and calculated a functional connectivity st

[Primary hypereosinophilia syndrome manifesting as encephalopathy and vision disorders].

We report the case of a 60-year-old man who developed visual and cognitive disorders. Investigations confirmed the diagnosis of idiopathic hypereosinophilic syndrome even though the patient had a history of rectal cancer. The olinical course was favorable after treatment. We discuss the different clinical forms, imaging data and treatments of eosinphilic syndrome.

Diesel Aspiration Pneumonitis: A Rare but Serious Occupational Hazard.

Chemical pneumonitis is frequently encountered in patients following the siphoning of diesel fuel, which can lead to severe complications, including acute respiratory distress syndrome (ARDS). This is a 33-year-old male patient who presented with a history of dyspnea, a non-productive cough, and heartburn. His symptoms commenced five days earlier following an inadvertent aspiration of diesel while refueling vehicles. This patient was admitted to our intensive care unit as a case of ARDS related to chemical pneumonitis following siphoning of diesel fuel. This patient was treated with intravenou

Hutchinson-Gilford progeria syndrome complicated with stroke: A report of 2 cases and literature review.

BACKGROUND: Hutchinson-Gilford Progeria Syndrome (HGPS) is a ultrarare, fatal autosomal dominant disorder. The pathogenesis of the disease is a mutation in LMNA, which leads to the accumulation of progerin in cells, impairing the normal physiological functions. Stroke and transient ischemic attack seriously affect the survival rate and quality of life of HGPS children, although the literature of this aspect is limited. This study summarizes the clinical manifestations and related imaging features of HGPS children with stroke to improve pediatric clinicians' understanding of this disease. CASE

Differentiation of sarcoidosis-lymphoma syndrome lesions: a case report on the use of two different positron emission tomography tracers.

BACKGROUND: Sarcoidosis-lymphoma syndrome (SLS) is a rare disease in which both entities coexist. We aimed to study the role of (18)F-fluorodeoxyglucose (FDG) and L-[3-(18)F] α-methyltyrosine (FAMT) positron emission tomography (PET)/computed tomography (CT) in differentiating between these two lesions. CASE PRESENTATION: A 54-year-old female with large liver tumors was referred to our Nuclear Medicine Department for staging using FDG PET/CT. She had a history of primary biliary cirrhosis (PBC) for 15 years and developed lung and mediastinal sarcoidosis 1 year before the liver tumors were not

Benson's Disease or Posterior Cortical Atrophy, Revisited.

BACKGROUND: D. Frank Benson and colleagues first described the clinical and neuropathological features of posterior cortical atrophy (PCA) from patients in the UCLA Neurobehavior Program. OBJECTIVE: We reviewed the Program's subsequent clinical experience with PCA, and its potential for clarifying this relatively rare syndrome in comparison to the accumulated literature on PCA. METHODS: Using the original criteria derived from this clinic, 65 patients with neuroimaging-supported PCA were diagnosed between 1995 and 2020. RESULTS: On presentation, most had visual localization complaints and r

Aberrant patterns of default-mode network functional connectivity associated with metabolic syndrome: A resting-state study.

INTRODUCTION: Metabolic syndrome (MetS) is a clustering of three or more cardiovascular risk factors (RF), including hypertension, obesity, high cholesterol, or hyperglycemia. MetS and its component RFs are more prevalent in older age, and can be accompanied by alterations in brain structure. Studies have shown altered functional connectivity (FC) in samples with individual RFs as well as in clinical populations that are at higher risk to develop MetS. These studies have indicated that the default mode network (DMN) may be particularly vulnerable, yet little is known about the overall impact o

[Parkinsonism-ALS-dementia complex: case report].

We are presenting a case of Parkinson-ALS-Dementia complex in 53 years old male. Outstanding bulbar signs with muscular atrophy of tongue, muscular atrophy of shoulder girdle together with massive fasciculations (myoclonus type descending from spinal cord) have been observed. Clear Parkinson's syndrome coexisted--bradykinesia, rigidity with cogwheel resistance, "masking" of the face, disturbances of body balance and significant vegetative signs. Moreover psychological testing has shown psycho-organic syndrome of frontal type, MRI scans proved the existence of slight brain general atrophy that

Bilateral atrophy of the frontal and temporal lobes in schizophrenic patients with Capgras syndrome: a case-control study using computed tomography.

The computed tomographic scans of 12 patients with Capgras syndrome and 12 matched controls were blindly assessed. A variety of statistical comparisons between the two groups consistently showed that patients with Capgras syndrome had significantly more bilateral frontal and temporal lobe atrophy. Capgras syndrome may be associated with dysfunction of these areas. Interestingly, all of the index patients had paranoid schizophrenia, and these results suggest the hypothesis that in some schizophrenic patients the presence of Capgras syndrome may be a marker for the coexistence of a focal idiopat

[Space-occupying cerebral process as a clinical misdiagnosis in subacute spongioid encephalopathy (Jakob-Creutzfeldt syndrome). Clinical-morphological report on 3 cases].

This is a report on three cases of subacute spongioid encephalopathy (Jakob-Creutzfeldt syndrome) where an extended cerebral process had been diagnosed clinically. However, a final and correct diagnosis could not be established until after the postmortem examination. The patients were 1 woman and 2 men; the duration of disease was between 5 weeks and 2 1/2 years. In 2 of the patients, initial pseudoneurasthenic complaints were followed, in the further course of disease, by manifestations of an organic psychosyndrome accompanied by neurological and, moreparticularly, coordination disorders. [Relationship between SPECT and pathological alterations in Alzheimer's disease--a study of a case with left-hemisphere dominant lesions].

123I-IMP SPECT (SPECT) has been widely used in clinical neuropsychiatry for establishing the clinical diagnosis, and evaluating the course of the disease. However, little is known about the significance of alterations in SPECT. In this paper, we present comparative study between alterations in SPECT and neuropathological findings in the case of Alzheimer's disease (AD). The patient, a 59-year-old female, began to show memory disturbance and the left hemisphere disturbances, non-fluent aphasia, but right hemisphere disturbances, constructional apraxia, visuo-spatial dysfunctions were not notabl

A proposal for a new clinical entity: transient epileptic amnesia complex syndrome (TEACS).

Transient epileptic amnesia (TEA) is a special type of temporal lobe epilepsy, the main symptom of which is recurrent amnesia attacks. In the late 1990s, Zeman et al. developed the following diagnostic criteria for TEA: (i) recurrent, witnessed episodes of amnesia; (ii) other cognitive functions remain intact during attacks; and (iii) evidence of epilepsy. It was subsequently reported that patients with TEA almost always demonstrate two other types of memory symptoms: accelerated long-term forgetting (ALF) and autobiographical amnesia (AbA). As a result, it has been recognised that TEA causes

Papillary thyroid carcinoma presenting as paraneoplastic acral vascular syndrome in a 37-year-old female: A case report.

BACKGROUND: Paraneoplastic acral vascular syndrome is a rare condition associated with various malignancies, most commonly adenocarcinomas. It manifests as digital ischemia, including Raynaud's phenomenon, acrocyanosis, and digital gangrene. Reported cases typically show improvement after treatment of the underlying cancer. We describe the first known case of paraneoplastic acral vascular syndrome secondary to papillary thyroid carcinoma (PTC), with complete resolution following thyroidectomy. CASE PRESENTATION: A 37-year-old previously healthy woman was referred from the vascular surgery cli
