Neonatal Health Outcomes Before and After Introduction of a Midwifery-Led Care Model in Embu County, Kenya: A Randomized Controlled Trial.

INTRODUCTION: Neonatal health is a global concern, particularly in Sub-Saharan Africa where the neonatal mortality ratio exceeds Sustainable Development Goals targets. To address this, midwifery-led care models have been proposed as safe and cost-effective solutions. However, their use and impact in Kenya remain under-researched. This study aimed to evaluate the effects of one of these models on neonatal health outcomes. METHODS: Two hospitals, Siakago and Runyenjes, were chosen for a Randomized Controlled Trial involving 164 mothers and 60 midwives. Data were collected using questionnaires, …

Enablers and barriers of domiciliary midwifery practice using the socio-ecological model: a qualitative study exploring the perspectives of students and lecturers in Uganda.

BACKGROUND: Early postnatal discharge is increasingly common worldwide, yet the World Health Organization’s recommended 3-3-6 schedule for postnatal visits and assessments is often not adhered to by mothers and healthcare workers. The early neonatal period is critical for both mother and baby, accounting for a significant proportion of maternal and neonatal deaths, especially in developing countries. According to the recently released Midwifery Accelerator, achieving universal coverage of care provided by midwives can avert two-thirds of maternal and newborn deaths, including stillbirths. Domi…

The effect of Midwifery continuity of care on maternal outcomes among women with psychosocial risk factors in Sweden: A registry-based cohort study.

BACKGROUND: Women identified with psychosocial risk factors are at higher risk of interventions during labour and birth and adverse maternal outcomes. Midwifery Continuity of Care (MCoC) reduces interventions and improves maternal outcomes among low-risk women; however, evidence on MCoC's impact among women with psychosocial risk factors is limited. This study aimed to investigate whether an MCoC model targeting women with psychosocial risk factors improved maternal outcomes in Sweden. METHODS: We conducted a retrospective cohort study using data from the Swedish Pregnancy Register. The cohor…

How good are we at implementing evidence to support the management of birth related perineal trauma? A UK wide survey of midwifery practice.

BACKGROUND: The accurate assessment and appropriate repair of birth related perineal trauma require high levels of skill and competency, with evidence based guideline recommendations available to inform UK midwifery practice. Implementation of guideline recommendations could reduce maternal morbidity associated with perineal trauma, which is commonly reported and persistent, with potential to deter women from a future vaginal birth. Despite evidence, limited attention is paid to this important aspect of midwifery practice. We wished to identify how midwives in the UK assessed and repaired peri…
