A double-blind clinical trial of the analgesic effects of R & S 218-M, a new potent analgesic for the relief or pain following abdominal surgery: comparison with morphine sulphate.

The analgesic effects of R & S 218-M, administered in doses of 0.56 mg/70 kg and 0.35 mg/70 kg, were compared with those of morphine sulphate 10.5 mg/70 kg for the relief of abdominal pain following surgery in healthy adults. The drugs were given as the first potent analgesic after operation and the subjects were interviewed at 30-min intervals until the pain became severe again. All the interviews were conducted by the one observer. R & S 218-M 0.56 mg/70 kg was as effective as morphine sulfate 10.5 mg/70 kg, while R & S 218-M 0.35 mg/70 kg was inferior. No evidence was found to support the c

Molecular dynamics study of M-Trifluoromethyl diphenyl diselenide binding to the μ-opioid receptor: A computational perspective on morphine-induced tolerance.

Chronic pain is a maladaptive state where pain signals persist beyond the expected resolution of injury or illness. Morphine and related compounds, acting as µ-opioid receptor (µOR) agonists, are effective analgesics for managing this condition. However, chronic morphine administration can disrupt µOR trafficking and activate β-arrestin-mediated pathways, leading to opioid tolerance. The role of µOR in mood disorders is less well-defined. The organoselenium compound m-trifluoromethyl diphenyl diselenide (TFDD) has shown promising antinociceptive and antidepressant-like effects in experimental

NCP, a Dual Kappa and Mu Opioid Receptor Agonist, Is a Potent Analgesic Against Inflammatory Pain without Reinforcing or Aversive Properties.

While agonists of μ (MOR) and κ (KOR) opioid receptors have analgesic effects, they produce euphoria and dysphoria, respectively. Other side effects include respiratory depression and addiction for MOR agonists and sedation for KOR agonists. We reported that 17-cyclopropylmethyl-3,14β-dihydroxy-4,5α-epoxy-6β-{[4'-(2'-cyanopyridyl)]carboxamido}morphinan (NCP) displayed potent KOR full agonist and MOR partial agonist activities (58%) with 6.5x KOR-over-MOR selectivity in vitro. Herein, we characterized pharmacological effects of NCP in rodents. In mice, NCP exerted analgesic effects against infl

Sinomenine regulated gut-joint axis for alleviating rheumatoid arthritis.

BACKGROUND: Gut microbiota is considered as a new target for the treatment of rheumatoid arthritis (RA). However, the mechanism by which Sinomenine (SIN) alleviates RA by modulating gut microbiota has not been fully explored. METHODS: The collagen-induced arthritis (CIA) rat model was established, and the therapeutic effect of SIN on CIA rats was evaluated. Alterations in gut microbiota and serum metabolites in CIA rats after SIN administration were assessed by 16S rRNA and non-targeted metabolomics methods. Then the CIA pseudo-sterile model was established by the combination of broad-spectru

Application of hybrid exosomes-mediated targeted delivery system for sinomenine hydrochloride in rheumatoid arthritis therapy.

Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by synovial inflammation and joint destruction, whose pathogenesis is closely associated with dysregulated macrophage polarization. Recent evidence highlights the inherent anti-inflammatory properties of exosomes derived from M2-macrophages (M2-Exo), positioning them as promising bioinspired vehicles for targeted delivery to inflammatory sites. Here, we present an innovative hybrid exosome system (SH@M2-Exo-Lip) for targeted RA therapy. This system was constructed via the fusion of sinomenine hydrochloride (SH)-loaded lipo

Sinomenine Liposomes Alleviate Neuropathic Pain in a Spared Nerve Injury Model by Regulating Astrocyte Reactivity Associated with Inhibition of the JAK2/STAT3 Pathway.

Neuropathic pain (NP) is chronic pain caused by injury or disease affecting the nervous system. SIN@Lip-HA was prepared and characterised for particle size, zeta potential, encapsulation efficiency and morphology. Sprague-Dawley rats with spared nerve injury (SNI) were randomly divided into the six groups. Analgesic effects were assessed via behavioural tests. Histopathological changes were examined using H&E staining. Astrocyte polarisation was detected using immunofluorescence, and Western blotting was used to assess related protein expression. qRT-PCR measured mRNA level of genes, while ELI

Targeted neural stem cell-derived extracellular vesicles loaded with Sinomenine alleviate diabetic peripheral neuropathy via WNT5a/TRPV1 pathway modulation.

BACKGROUND: Diabetic peripheral neuropathy (DPN) is one of the most prevalent and debilitating complications of diabetes, marked by chronic neuroinflammation, immune dysregulation, and progressive neuronal degeneration. Current treatments offer limited efficacy, largely focusing on symptomatic relief rather than addressing the underlying disease mechanisms. There is a critical need for disease-modifying therapies that target the molecular basis of DPN. RESULTS: In this study, we developed a novel targeted nanotherapeutic system-ZH-1c-EVs@SIN-composed of neural stem cell-derived extracellular

Novel Oral Therapies for Opioid-induced Bowel Dysfunction in Patients with Chronic Noncancer Pain.

Opioid analgesics are frequently prescribed and play an important role in chronic pain management. Opioid-induced bowel dysfunction, which includes constipation, hardened stool, incomplete evacuation, gas, and nausea and vomiting, is the most common adverse event associated with opioid use. Mu-opioid receptors are specifically responsible for opioid-induced bowel dysfunction, resulting in reduced peristaltic and secretory actions. Agents that reverse these actions in the bowel without reversing pain control in the central nervous system may be preferred over traditional laxatives. The efficacy

Sinomenine induces the generation of intestinal Treg cells and attenuates arthritis via activation of aryl hydrocarbon receptor.

Sinomenine (SIN), an anti-arthritis drug, has previously been proven to exert immunomodulatory activity in rats by inducing intestinal regulatory T-cells (Treg cells). Here, we assessed the effect of SIN on the generation and function of Treg cells in autoimmune arthritis, and the underlying mechanisms in view of aryl hydrocarbon receptor (AhR). The proportions of Treg cells and IL-17-producing T-cells (Th17 cells) differentiated from naive T-cells were analyzed by flow cytometric analysis. The AhR agonistic effect of SIN was tested by analyzing the activation of downstream signaling pathways

Drug effects on multiple and concurrent schedules of ethanol- and food-maintained behaviour: context-dependent selectivity.

BACKGROUND AND PURPOSE: Drugs that more potently or effectively reduce ethanol-maintained behaviour versus an alternative are considered selective and are considered promising pharmacotherapies for alcoholism. Such results are often obtained using separate groups or multiple schedules where ethanol and the alternative are available alone or sequentially. Recently, we observed that when ethanol and food were available sequentially under a multiple schedule, fluvoxamine and varenicline were selective; yet this selectivity disappeared when ethanol and food were concurrently available. EXPERIMENT

Pyrrolomorphinans as delta opioid receptor antagonists. The role of steric hindrance in conferring selectivity.

A series of 2',3'-disubstituted pyrrolomorphinans (5a-i) were synthesized to determine the role of steric hindrance at mu and kappa receptors in promoting delta opioid receptor antagonist selectivity. In smooth muscle preparations, five members of the series (5a-c,e,f) possessed Ke values in the range 2-15 nM and were delta selective. Since the unsubstituted analogue 4 possessed delta antagonist potency of similar magnitude, but was not delta selective, it is suggested that the 2',3'-substitution confers delta selectivity by hindering the interaction of the pharmacophore at mu and kappa recept…

Role of spacer and address components in peptidomimetic delta opioid receptor antagonists related to naltrindole.

A series of heterocyclic analogues 2-5 related to naltrindole (1) (NTI) and 6-arylnaltrexone derivatives 6-8 were synthesized in order to determine the role of the spacer and the address moieties in conferring delta opioid receptor antagonist activity. The benzofuran (NTB), quinoxaline, and quinoline analogues (2, 3, and 4, respectively) were delta-selective opioid antagonists in vitro and bound selectively to delta receptors. The tetrahydroindole derivative 5, while delta-selective, was considerably less potent than its indole analogue 13. The data for 2-4 indicate that heterocycles other tha

"Paradoxical" analgesia and aggravated morphine dependence induced by opioid antagonists.

Chronic treatment with naloxone (Nx) or naltrexone (Ntx) induces paradoxical analgesia. In the present study, the effects of chronic treatment with opioid receptor antagonists, such as nor-binaltorphimine (nor-BNI) for kappa and naltrindole (NTI) for delta receptors, on analgesic response using the hot plate test and on morphine physical dependence in rats were examined. The hot plate latency was significantly increased by pretreatment with Nx (5 mg/kg, s.c.), nor-BNI (20 mg/kg, i.p.) or NTI (20 mg/kg, i.p.) for 5 days. After chronic pretreatment with these antagonists, the rats were treated w

Design of peptidomimetic delta opioid receptor antagonists using the message-address concept.

Highly selective nonpeptide ligands with potent delta opioid receptor antagonist activity have been developed using the message-address concept. This approach envisaged the delta opioid receptor to contain two major recognition subsites; a message subsite which recognizes the pharmacophore, and an address subsite that is unique for the delta receptor type and confers selectivity. The message and address components of the delta-selective enkephalins were postulated to be Tyr1 and Phe4, respectively, with Gly2-Gly3 functioning as a spacer. The message component of the target compounds in this st

Adrenalectomy and the anorectic effects of benzodiazepine inverse agonists and opiate antagonists in rats fed a palatable diet.

The benzodiazepine (BZ) receptor inverse agonists, FG 7142 (1.25-10.0 mg/kg, IP) and CGS 8216 (2.5-20.0 mg/kg, IP), significantly attenuated the consumption of a palatable sweetened diet by non-deprived male rats in a 30 min test. Adrenalectomy failed to affect the reduction in food intake produced by these two drugs. Similarly, the anorectic effects of the opiate antagonists, naltrexone (0.3-3.0 mg/kg, SC) and diprenorphine (0.3-3.0 mg/kg, SC) in the same feeding paradigm were unaffected by adrenalectomy. So far as palatability-induced feeding in concerned, anorectic effects of BZ inverse ago
