The most frequent pathogens of opportunistic mycotic infections are Candidae. The clinical manifestation of infection is very varied. With regard to the extent of the affection the following are differentiated: 1. mucocutaneous candidosis, 2. organ affections and 3. disseminated candidosis. The incidence of mycoses caused by Aspergillae, cryptococci, and mucors is of a lower order, these infections are, however, associated with a relatively higher mortality. The authors review briefly different fungi and describe the most important clinical manifestations of infection.

We report an acute myelogenous leukemia patient with mucormycosis who died of massive hemoptysis during antifungal therapy. The diagnosis was made postmortem and autopsy revealed that the pulmonary nodule consisting of mucorales protruded over the luminal surface of the aorta. Microscopic examination showed the invasion of mucor hyphae into the wall of the aortic arch. Surgical treatment may be indicated for patients with pulmonary mucormycosis refractory to amphotericin B therapy.

The histological study with Hotchkiss-Mc Manus staining and Grocott silver impregnation of tissues invaded by opportunistic fungi is a rapid and easy means of diagnosis. The inflammatory reactions around the fungus give the certitude that it is a pathogen and not a contaminant. The major opportunists among Canidida, Aspergillus, Mucor, Absidia and Cryptococcus species are presented in local and disseminated lesions, but all fungi, saprophytic in the normal host, can become pathogens in the immunodepressed patient.

Mucormycotic infections caused by fungi of the families Rhizopus, Mucor or Absidia are rare and usually associated with diabetes or immunosuppression. We describe a patient with invasive necrotizing cutaneous mucormycosis caused by Absidia corymbifera shortly after allogeneic BMT. The infection was successfully treated with surgical debridement and liposomal amphotericin B for 6 weeks. Recognition of these rare infections requires a high index of suspicion. These patients should be evaluated with tissue biopsy and cultures and treated without delay.

The rise in antifungal resistance and changing epidemiology underscores the need to improve antifungal susceptibility testing. This study tested the standard medium RPMI1640 and healthy lung and sinus media against Aspergillus, Mucor, and Candida species. Candida showed greater variability in minimum inhibitory concentrations (MICs) compared with molds, with higher MICs in respiratory tract-mimicking media than in RPMI1640. Aspergillus terreus was the most affected mold, showing higher MICs for certain antifungals in these media.

Mucormycosis is a rare and aggressive fungal infection, most often caused by species of the Mucor, Rhizomucor, Rhizopus, Absidia, and Cunninghamella genera. The condition most commonly affects patients with uncontrolled diabetes, HIV/AIDS, malignancy, and those receiving long-term immunosuppressive therapy. We report the case of a 39-year-old male with biopsy-proven cutaneous mucormycosis of the left axilla 4 months after an orthotopic heart transplant for congenital tricuspid atresia.

The first known case of an intravascular catheter-related primary cutaneous mucormycosis in a heart transplant patient is reported. The patient had corticosteroid-induced hyperglycemia and experienced an acute tissue rejection episode. A necrotic lesion appeared around the insertion site of a peripheral venous catheter. A biopsy revealed typical mucorales hyphae. The lesion continued to spread during the following 24 hours and necessitated amputation of the forearm. The organism was identified as a Mucor species.
