The broadening scope of oral mucositis and oral ulcerative mucosal toxicities of anticancer therapies.

Oral mucositis (OM) is a common, highly symptomatic complication of cancer therapy that affects patients' function, quality of life, and ability to tolerate treatment. In certain patients with cancer, OM is associated with increased mortality. Research on the management of OM is ongoing. Oral mucosal toxicities are also reported in targeted and immune checkpoint inhibitor therapies. The objective of this article is to present current knowledge about the epidemiology, pathogenesis, assessment, risk prediction, and current and developing intervention strategies for OM and other ulcerative mucosa

The mucosal immune system of the upper respiratory tract and recent progress in mucosal vaccines.

The mucosal immune system prevents microorganism invasion through mucosal surfaces and consists of inductive and effector sites. Nasopharynx-associated lymphoid tissue (NALT) functions as an inductive site, inducing mucosal immune responses in the upper respiratory tract. It follows that intranasal vaccines may prevent upper respiratory infections. To induce and enhance the immune response by administering inactivated antigens intranasally, mucosal adjuvants have been developed, including mutant cholera toxin and cationic cholesteryl pullulan nanogel, which do not accumulate in the central ner

Mucosal delivery of fish vaccines: Local and systemic immunity following mucosal immunisations.

The mucosal organs of fishes are directly exposed to their aquatic environment, which is suited to the colonization and growth of microorganisms, and thus these barriers are considered to play an important role in maintaining homeostasis and preventing entry of invasive pathogens. Research on fish mucosal immunity have shown that mucosal organs such as gills, skin, intestines and olfactory organs harbor lymphoid cells, including T and B cells as well as dendritic-like cells. Findings related to immune responses following direct administration of antigens into the mucosal organs could help to s

[Treatment of rectocele and rectal mucosal prolapse with mucosal flaps excision and with manual reconstruction].

BACKGROUND: Rectocele is defined as an erniation of the anterior rectal and vaginal wall in the vaginal lumen due to the loss of rectovaginal septum. Rectal mucosal prolapse was characterized by the protrusion of the mucosa alone in the rectal lumen. The Authors, herein, reported the results of a new surgical technique with the excision of one or more mucosal or muco-muscolar flaps and reconstruction with Sequential Transfixed Stitch Technique (STST) using a anal retractor designed by them. MATERIALS AND METHODS: The new surgical technique was applied to 40 consecutive patients with rectal mu

Heterologous mucosal vaccine boosting enhances mucosal and systemic immunity by distinct mechanisms.

Seasonal booster vaccination is the primary intervention for protection from respiratory viral infections, such as influenza virus or SARS-CoV-2. However, efficacy is often limited because immune exposure to prior strains impairs development of new responses. In this study, we sought to determine how this issue could be overcome in a mouse model of heterologous immunization against WT and omicron strains of SARS-CoV-2. Intranasal booster immunization circumvented the shortcomings of intramuscular immunization, resulting in superior systemic and mucosal T and B cell immunity and better viral co

GV1001, hTERT Peptide Fragment, Prevents 5-Fluorouracil-Induced Mucositis by Inhibiting Mitochondrial Damages.

Chemotherapy-induced mucositis (CIM) is a dose-limiting toxicity of cancer therapy that is mainly associated with mitochondrial dysfunction in epithelial cells. We investigated whether GV1001, a mitochondrial protective peptide from human telomerase reverse transcriptase (hTERT), attenuates 5-fluorouracil (5-FU)-induced mucositis in a murine model. 5-FU induced notable mortality, leukopenia, and mucositis in the gastrointestinal (GI) tract, including tongue, esophagus and small intestine. It promoted epithelial-mesenchymal transition (EMT), nuclear factor kappa-B (NF-κB) activation, systemic a

Pembrolizumab-induced pan-airway mucositis with a cobblestone-like appearance: a case report.

Immune checkpoint inhibitors (ICIs) can induce diverse immune-related adverse events (irAEs), but pan-airway mucosal involvement is rare and may mimic common infectious laryngopharyngitis. A 76-year-old woman receiving long-term pembrolizumab for lung squamous cell carcinoma developed asthma-like symptoms ~1 year after treatment initiation, which were partially controlled with inhaled corticosteroid. Approximately 3 years after pembrolizumab initiation, she presented with persistent hoarseness and sore throat. Laryngoscopy showed diffuse erythema and a characteristic cobblestone-like appearanc

Novel Microbiome-Driven Approaches to Counteract Chemotherapy-Induced Mucositis in Breast Cancer Patients.

Chemotherapy-induced mucositis (CIM) is a clinically important toxicity in breast cancer care that adversely affects oral and gastrointestinal mucosa, quality of life, nutritional status, and treatment tolerability. While CIM is less frequently dose-limiting in contemporary standard regimens (eg, AC-T, TC) compared with hematologic and neurotoxicities, it remains a significant supportive care challenge, particularly in dose-dense schedules, older cytotoxic protocols, and with selected targeted agents and antibody-drug conjugates. Emerging evidence indicates that chemotherapy-associated dysbios

FLASH reduces radiation-induced oral mucositis in a mouse model of Fanconi anemia.

Patients with Fanconi anemia (FA) are particularly susceptible to developing squamous cell carcinoma of the head and neck due to impaired DNA repair pathways. However, their hypersensitivity to DNA damaging agents can limit effective treatment with standard radiotherapy due to severe side effects and complications. In pre-clinical models, ultra-rapid FLASH radiotherapy (FLASH) reduces radiation-induced toxicity in normal tissues while maintaining similar tumor control compared to conventional dose rate radiotherapy (CONV). Here, we investigated the safety of FLASH for treatment of the head and

Laryngeal stenosis as a late complication of plasma cell mucositis of the larynx.

Plasma cell mucositis (PCM) is a rare inflammatory condition of the upper aerodigestive tract with a heterogeneous clinical course. Predominately seen in the oral cavity, laryngeal involvement and the requirement for tracheostomy have been rarely described. We describe the case of a man diagnosed with airway-threatening PCM isolated to the larynx requiring a tracheostomy. He responded to oral glucocorticoids and was decannulated only to present 3 years later with severe laryngeal stenosis requiring repeat tracheostomy insertion. Medical management in the form of immunosuppression is the mainst

Dual-functional guanosine-based hydrogel: high-efficiency protection in radiation-induced oral mucositis.

Radiation-induced oral mucositis (RIOM) is the most common adverse effect experienced by cancer patients following radiotherapy; however, effective clinical treatments remain insufficiently recognized. In this study, a guanosine-polyvinyl alcohol (G-PVA) supramolecular hydrogel was developed using a one-pot synthesis method. The G-PVA hydrogel demonstrated remarkable wet adhesion properties measuring 74.16 kPa (±3.53 kPa), biocompatibility and shape adaptability, making it suitable for the dynamic conditions of the oral cavity. Under radiation exposure, the G-PVA hydrogel not only mitigated ox

Management of Oral Mucositis in Pediatric Patients: A Practical Overview of Available Treatment.

Oral mucositis (OM) is an inflammatory condition associated with breakdown of the protective mucosal barrier. Clinically, it can range from mucosal erythema to painful ulcerations that can lead to malnutrition, secondary infection, and increased medical costs due to hospitalizations. Existing literature regarding OM primarily investigates antineoplastic-induced OM in the adult population; however, many non-oncologic disease processes can present with OM, including autoimmune and infectious conditions such as pemphigus vulgaris, recurrent aphthous stomatitis, oral lichen planus, and reactive in

Exploring the Influence of Sex on the Development of Radiation-Induced Oral Mucositis.

Oral mucositis is a significant side effect of chemotherapy, radiotherapy, or their combination in cancer patients. Current clinical management focuses primarily on symptom control, with few effective therapeutic agents. Understanding the factors influencing susceptibility to oral mucositis is essential for a comprehensive grasp of its pathophysiology. This study investigates sex as a biological variable in radiation-induced oral mucositis. Oral mucositis was induced via X-ray ionizing radiation using a fractionated dose of 8 Gy/d for 5 consecutive days, targeting only the head and neck area u

Patient-, implant- and prosthetic-related factors on peri-implant mucositis and bone loss.

Peri-implant diseases, including peri-implant mucositis (PIM) and peri-implantitis, are a chronic inflammatory disorder triggered by bacterial biofilm in susceptible hosts. Potential risk factors for peri-implant diseases include smoking, dental plaque accumulation, poor oral hygiene, genetics, and absence of peri-implant keratinized mucosa. This cohort study aimed to evaluate the influence of patient-, implant-, and prosthetic-related factors on PIM and peri-implant bone loss (PBL) around dental implants after 1 year of loading. A total of 54 subjects (22 males and 32 females) were included i

Microbiological and functional traits of peri-implant mucositis and correlation with disease severity.

Osseointegrated dental implants replace missing teeth and create an artificial surface for biofilms of complex microbial communities to grow. These biofilms on implants and dental surfaces can trigger infection and inflammation in the surrounding tissue. This study investigated the microbial characteristics of peri-implant mucositis (PM) and explored the correlation between microbial ecological imbalance, community function, and disease severity by comparing the submucosal microflora from PM with those of healthy inter-subject implants and intra-subject gingivitis (G) within a group of 32 indi

Dehydrocostus lactone alleviates irinotecan-induced intestinal mucositis by blocking TLR4/MD2 complex formation.

BACKGROUND: Irinotecan (CPT-11) is used as chemotherapeutic drug for treatment of colorectal cancer. However, without satisfactory treatments, its gastrointestinal toxicities such as diarrhea and intestinal inflammation severely restrained its clinical application. Roots of Aucklandia lappa Decne. are used as traditional Chinese medicine to relieve gastrointestinal dysfunction and dehydrocostus lactone (DHL) is one of its main active components. Nevertheless, the efficacy and mechanism of DHL against intestinal mucositis remains unclear. PURPOSE: The present study aimed to investigate the pro

Gold nanoparticles as innovative therapeutics for oral mucositis: A review of current evidence.

Oral mucositis (OM) remains a debilitating side effect in patients undergoing cancer therapy. DNA damage and oxidative stress generated by radiation and/or chemotherapy activate key inflammatory pathways, ultimately resulting in the destruction of the epithelial barrier, leading to microbial colonization, and ulceration. These ulcerative lesions are often extremely painful, compromising nutrition and oral hygiene, requiring intravenous nutritional support, resulting in longer periods of hospitalization and increased cost. Ulcers often increase the risk of secondary infection, disrupting cancer
