Myositis-associated antibodies predict the severity of lung involvement in adult patients with inflammatory myositis - a cohort study of 70 adult patients with myositis in a single center.

INTRODUCTION: Idiopathic inflammatory myopathies (IIMs) encompass a diverse group of diseases characterized by considerable variability in clinical manifestations, antibody profiles, and responsiveness to immunosuppressive therapies. This study aimed to investigate the association between organ involvement and distinct myositis autoantibodies in individuals with IIM in a single-center cohort. METHODS: Patients with ICD diagnoses M33.1, M33.2, M33.9, or M609 who (1) had been tested with Euroline blot assay for myositis autoantibodies and (2) met the classification criteria of definite/probable…

Myositis Ossificans of the Soleus Muscle.

Myositis ossificans (MO) is a rare condition in which abnormal calcification and bone formation develop within a muscle. It appears most often in adolescents but can also occur in younger children. Although it usually affects a single muscle, it can sometimes involve different parts of the body. We report a case of post-traumatic MO. A 16-year-old girl presented with progressive left knee pain and swelling 15 days after minor trauma, with a visual analog scale (VAS) for pain of 8/10 and nocturnal exacerbation. Examination showed a soft, mobile, tender medial knee mass with reduced range of mot…

New treatments in myositis on the horizon.

The idiopathic inflammatory myopathies (IIM), or briefly myositis, are a heterogeneous group of rare autoimmune diseases with high morbidity and mortality. Current treatment is based on high doses of glucocorticoids over long periods of time in combination with immunosuppressive drugs, with varying and often disappointing results. Therefore, there is a high unmet need for new therapies. Subgrouping patients with IIM based on clinical, histopathological and autoantibody profiles has facilitated identification of predominant molecular pathways, and this knowledge has been applied in recent succe…

Diagnostic Evaluation for Myositis and Their Pitfalls.

Myositis is a systemic autoimmune disorder that affects multiple organ systems, including the skeletal muscles, lungs, skin, and joints. The clinical presentation can vary significantly, and physicians must tailor the diagnostic evaluation for each patient, reflecting a personalized approach rather than relying on a standardized routine order set. Each diagnostic test has its own advantages, limitations, and pitfalls, requiring careful interpretation of results to make appropriate clinical decisions. In this article, we discuss the clinical applications of laboratory tests, myositis antibodies…

Autoantibody testing in myositis: an update.

PURPOSE OF REVIEW: This review aims to provide an update on myositis autoantibody testing strategies. We have focussed on the reliability and usefulness of different myositis autoantibody detection methods, including commonly used solid phase immunoassays and newer discovery techniques. RECENT FINDINGS: Several studies have highlighted the limitations of currently available immunoassays, particularly when used in populations with low pretest probability and without supporting clinical evidence. While many autoantibodies, such as anti-Jo1, are detected with high sensitivity and specificity, th…

Update on muscle imaging in myositis.

PURPOSE OF REVIEW: Imaging techniques such as MRI, ultrasound and PET/computed tomography (CT) have roles in the detection, diagnosis and management of myositis or idiopathic inflammatory myopathy (IIM). Imaging research has also provided valuable knowledge in the understanding of the pathology of IIM. This review explores the latest advancements of these imaging modalities in IIM. RECENT FINDINGS: Recent advancements in imaging of IIM have seen a shift away from manual and qualitative analysis of the images. Quantitative MRI provides more objective, and potentially more sensitive characteriz…

An Atypical Presentation of Sarcoid Myositis.

Sarcoidosis is a well-characterized inflammatory disease that affects multiple organ systems and can have long-term devastating outcomes if … dosis as granulomatosis mucositis in the salivary gland and concurrent rare complication of sarcoid myositis in the rectus femoris in a patient with no evidence of pulmonary involvement throughout the duration of their clinical course. Further, we discuss differential diagnoses related to this patient's presentation as well as the efficacy of treatment mod …

Update on outcome assessment in myositis.

The adult and juvenile myositis syndromes, commonly referred to collectively as idiopathic inflammatory myopathies (IIMs), are systemic autoimmune diseases with the hallmarks of muscle weakness and inflammation. Validated, well-standardized measures to assess disease activity, known as core set measures, were developed by international networks of myositis researchers for use in clinical trials. Composite response criteria using weighted changes in the core set measures of disease activity were developed and validated for adult and juvenile patients with dermatomyositis and adult patients with…

Myositis in a child with murine typhus.

A 12-year-old boy presented with fever, lower extremity pain and weakness. Examination revealed paraparesis, thigh and calf tenderness. Labs showed high creatinine phosphokinase and Rickettsia typhi titers. This case illustrates endemic typhus should be considered in the differential diagnosis of myositis especially in areas with high prevalence of the disease. To our knowledge, this is the first reported case of myositis and paraparesis associated with a case of murine typhus.

Myositis or dystrophy? Traps and pitfalls.

Some hereditary myopathies can mimic acquired myositis especially when they first present in adulthood with a limb-girdle distribution of weakness. Although inherited myopathies are generally painless and progress very slowly there are exceptions, which can further add to diagnostic confusion. The diagnosis is made even more difficult when inflammatory infiltrates are present on muscle biopsy. This is common in certain dystrophies in particular e.g. dysferlinopathies or facioscapulohumeral dystrophy. On the other hand, acquired (and treatable with immunosuppressants) necrotizing myopathies wit…

[Myositis ossificans traumatica of the palm].

We present a 27-year-old female patient who developed a post-traumatic myositis ossificans with pain and restricted function after trigger finger release of the middle finger. After tumour resection an early recurrence made a re-resection necessary, followed by radiotherapy. Three years postoperatively the patient is free of complaints.

[Rehabilitation for myositis in acute phase].

Polymyositis and dermatomyositis (PM/DM) are representative inflammatory muscle diseases. In treatment of PM/DM, drug therapies are cardinal but rehabilitation may be another option. Since muscles with PM/DM are fragile for muscle exercise, rehabilitation has been recommended mainly in chronic phase. Some researchers considered that rehabilitation for PM/DM patients in acute phase may improve their functional prognosis without major sideeffect. However, there are controversies about rehabilitation for PM/DM patients from acute phase. To consider advisability, I reviewed literatures concerning …

Myositis and malignancy: is there a true association?

There may be an association between polymyositis/dermatomyositis and malignant disease. Cancer occurs in patients with polymyositis/dermatomyositis with a frequency estimated between 2.5% and 29% (relative risk 1.0 to 6.5). We present two such cases, associated with colorectal carcinoma and non-Hodgkin's lymphoma respectively, together with an overview of existing controlled studies in the area.
