Sensitization to Cat: When Is Nasal Challenge Needed?

INTRODUCTION: Although the skin prick test (SPT) is a reliable diagnostic tool in perennial allergic rhinitis (PER) for patients allergic to cats, the minimum necessary SPT wheal size required to distinguish cat sensitization from true allergy remains controversial. The cat nasal challenge test (cNCT) could be considered the gold standard for detecting true cat allergy. AIMS: To assess the difference in the frequency of cNCT positivity between cat owners and non-owners and to determine an appropriate cut-off level for SPT wheal size in detecting positive cNCT in PER patients who are candidate

[Food-induced anaphylaxis in latex allergy].

Contact uticaria and anaphylactic reactions to latex-containing rubber products are being recognized with increasing frequency in all kinds of medical disciplines. Recently a number of reports have been published describing anaphylactic reactions to food items in patients with latex allergy. The cases of three patients who developed anaphylactic reactions to both latex and food items are presented, and the importance of the association of latex and cross-reactivity with food items is stressed. The food items that led to anaphylactic reactions were banana and avocado; banana, avocado and buckwh

Determining the Threshold Concentration of Birch Pollen for Inducing Allergic Symptoms Using an Allergen Challenge Chamber.

UNLABELLED: <p>Introduction: To determine threshold concentrations of pollen inducing symptoms in seasonal allergic rhinitis patients has been a challenge for decades. Allergen challenge chambers (ACC) allow a controlled, reproducible experimental design to address this problem. Hitherto, ACCs were only run with high pollen concentrations. METHODS: The Fraunhofer ACC was technically modified to deploy very low pollen concentrations. Then, adults with birch pollen-induced allergic rhinitis were challenged with varying birch pollen concentrations using a patient-blinded, sham challenge-controll

The influence of terfenadine and pseudo-ephedrine alone and in combination on allergen-induced rhinitis.

Nasal biopsies to naturally occurring allergic rhinitis demonstrate ultrastructural evidence of mast cell degranulation. Consistent with this, H1 antihistamines are clinically efficacious in this disorder. The nasal symptoms of itch, sneeze and discharge are, however, more susceptible to this mode of therapy than is nasal blockage. As nasal blockage is vascular, the combination of an H1 antihistamine and a decongestant has a logical rationale. To investigate the efficacy of such a combination, the influence of pretreatment with terfenadine, 60 mg, pseudo-ephedrine, 120 mg, Seldane-d (a registe

Objective assessment of lower airway involvement in chronic rhinosinusitis.

BACKGROUND: When associated with lower airway involvement, the morbidity and the cost of chronic rhinosinusitis (CRS) can increase. The aim of this prospective study was to assess the clinical and radiological characteristics of the upper airways in CRS patients scheduled for functional endoscopic sinus surgery and to correlate these findings with the functional changes of the lower airways. METHODS: Twenty-five adult patients with CRS were subjected to clinical and endoscopic assessments, computed tomography scan, and allergic assessments using validated scoring systems. Lower airways were a

Budesonide and nasal histamine challenge.

The possible mode of action of the lately demonstrated steroid effect on the immediate type allergic reaction was investigated. The influence of a topical steroid, budesonide, on the effects that released mediators have on the nasal mucosa was also studied. A nasal histamine challenge study was performed in a double-blind, cross-over fashion, a 1-week pretreatment with budesonide or placebo preceding the challenge. Symptoms were recorded by means of symptom score as well as objectively via rhinomanometry. In contrast to a previous allergen challenge study, the steroid was found to have minimal

Bronchospasm and rhinitis induced in allergic patients by in vitro released mediators. I. Methodology.

This paper reports the development of an in vitro model of mediator production to be applied to patients with allergic asthma or rhinitis. All the selected patients were atopics and exhibited type I hypersensitivity to Candida alb., D. pter., or both. Heparinized blood was taken. The lymphomonocytes were separated and challenged in vitro with the specific antigen. After two hours of incubation and careful washing, the cells were further cultured for 24 hours and the supernatants removed. Skin tests and bronchial provocation tests were done with supernatants with known concentration of proteins

[Occupational rhinitis].

Occupational rhinitis (OR) is an inflammatory disease of the nose, which is characterized by intermittent or persistent symptoms, arising out of causes and conditions attributable to a particular work environment and not to stimuli encountered outside the workplace. Its clinical symptoms (nasal congestion, sneezing, rhinorrhea, itching, nasal airflow limitation) are very similar with the symptoms of the allergic rhinitis caused by other (classical) agents. According to the 27/1996 NM Departmental Order, OR in Hungary is a notifiable disease. Despite, between year 1997 and 2009, not even a sing

Histamine metabolism in nasal polyps.

We attempted to determine the relationship of nasal polyps to histamine (HA) metabolism. Compared to that in allergy-related nasal polyps and infection-related nasal polyps, the level of HA in aspirin-induced asthma-related polyps was significantly lower. Large differences between the groups were not observed in HA-synthesizing enzyme activity, but degradative enzyme activity was much higher in aspirin-induced asthma-related polyps than in other types of nasal polyps tested. These findings suggest the possibility that the amount of HA in polyps associated with aspirin-induced asthma was less b

A case of occupational rhinitis caused by rice powder in the grain industry.

Rice is the major staple food in a large part of the world, especially in Asia. Hypersensitivity reactions to rice are rare. Moreover, cases of occupational allergies induced by inhalation of rice powder are uncommon. We report a 31-year-old male with work-related rhinitis and conjunctivitis symptoms caused by occupational exposure to rice powder in the grain industry. He showed positive responses to rice extracts on a skin prick test, and a high level of serum specific IgE to rice was detected by ELISA. Occupational rhinitis was confirmed by a nasal provocation test with rice extracts. An IgE
