Nasopharyngeal necrosis following intensity-modulated radiation therapy of primary nasopharyngeal carcinoma-incidence rate and predictors of risk.

OBJECTIVES: This study aimed to investigate the incidence of post radiation nasopharyngeal necrosis (PRNN) in primary NPC after intensity modulated radiation therapy (IMRT) and identify the predictors of risk. METHODS: Data of 5798 NPC patients who received IMRT-based treatment between April 2009 and December 2015 were retrospectively reviewed. PRNN was diagnosed by MRI or nasopharyngoscopy. Dosimetric factors were selected by the least absolute shrinkage and selection operator logistic regression and applied to Cox proportional hazards modeling with clinical predictors. RESULTS: Among the 5

Treating radiation‑related nasopharyngeal necrosis with endostar in patient with nasopharyngeal carcinoma: A report of two cases and a literature review.

Radiation-related nasopharyngeal necrosis (RRNN) is a rare and often fatal complication in patients with nasopharyngeal carcinoma (NPC). Currently, no standard treatments are recommended for RRNN. The effects of traditional conservative treatments are suboptimal, and surgery for RRNN cannot be performed by inexperienced doctors. In the present study, the use of Endostar in two patients with RRNN was evaluated. Two patients with RRNN were treated at the Department of Oncology, Panyu Central Hospital (Guangzhou, China). Endostar was administrated (15 mg/day from day 1 to day 7, every three weeks

Nasopharyngeal Large Cell Neuroendocrine Carcinoma Synchronized With Nasopharyngeal Squamous Cell Carcinoma: A Case Report and Review of Literature.

Neuroendocrine carcinomas (NECs) are poorly differentiated neuroendocrine tumors of the upper respiratory tract. We present an extremely rare case of nasopharyngeal large cell neuroendocrine carcinoma (LCNEC) synchronized with nasopharyngeal squamous cell carcinoma (SCC). Both SCC and LCNEC are associated with Epstein-Barr virus (EBV) infection, supported by the positive result of Epstein-Barr encoding region in-situ hybridization. Strong correlation is found between EBV infection and nasopharyngeal malignancies. Furthermore, the EBV status might be a crucial prognostic factor in nasopharyngea

Nasopharyngeal Epstein-Barr virus DNA loads in high-risk nasopharyngeal carcinoma families: Familial aggregation and host heritability.

Nasopharyngeal carcinoma (NPC), the most common head and neck cancer, is characterized by distinct geographic distribution and familial aggregation. Multiple risk factors, including host genetics, environmental factor, and EBV infection, have been linked to the development of NPC, particularly in the familial clustering cases. However, the cause of NPC endemicity remains enigmatic due possibly to the complicated interplay between these risk factors. Recently, positive Epstein-Barr virus (EBV) DNA loads at nasopharyngeal (NP) cavity has been found to reflect NPC development and applied in NPC s

Epstein-Barr virus DNA load in nasopharyngeal brushings and whole blood in nasopharyngeal carcinoma patients before and after treatment.

PURPOSE: Nasopharyngeal carcinoma (NPC) is consistently associated with Epstein-Barr virus (EBV) and highly prevalent in Indonesia. EBV-DNA load can be used for early diagnosis and may have prognostic value. In this study, EBV-DNA load was evaluated in minimal invasive nasopharyngeal (NP) brushings and whole blood for initial diagnosis and therapy assessment against the standard-of-care diagnosis by biopsy with EBV-RISH and standard EBV-IgA serology. EXPERIMENTAL DESIGN: NP brushings and blood samples were collected from 289 consecutive ENT patients suspected of NPCs and 53 local healthy cont

Presence of Epstein-Barr virus latent membrane protein 1 gene in the nasopharyngeal swabs from patients with nasopharyngeal carcinoma.

BACKGROUND: Nasopharyngeal carcinoma (NPC) is the most common head and neck malignancy in southeastern China and Taiwan. Early detection of the local disease followed immediately by proper treatment is essential to increase the cure and survival rates. Because every NPC tumor cell carries Epstein-Barr Virus (EBV) genomes, detection of EBV genomic DNA such as latent membrane protein 1 gene (LMP1) might indicate the presence of NPC. We developed a simple and noninvasive technique of nasopharyngeal swabbing to acquire nasopharyngeal cells for detecting the presence of EBV genome. The aim of this

SEPT9, H4C6, and RASSF1A methylation in nasopharyngeal swabs: A reflection of potential minimally invasive biomarkers for early screening of nasopharyngeal cancer.

The potential value of epigenetic DNA methylation in early cancer screening has been demonstrated. Therefore, in this study, we performed QMS-PCR and quantitative reverse transcription PCR on the genes RASSF1A, H4C6, SEPT9, GSTP1, PAX1, SHOX2, and SOX2, which are common in epithelial cancers. We found hypermethylation in RASSF1A, H4C6 and SEPT9. The mRNA expressions of RASSF1A, H4C6 and SEPT9 in tumor group were significantly different from those in the inflammatory group and healthy group (P < .05). Receiver operating characteristic (ROC) analysis showed that the area under the curve (AUC) of

A model to predict the risk of lethal nasopharyngeal necrosis after re-irradiation with intensity-modulated radiotherapy in nasopharyngeal carcinoma patients.

BACKGROUND: For patients with nasopharyngeal carcinoma (NPC) who undergo re-irradiation with intensity-modulated radiotherapy (IMRT), lethal nasopharyngeal necrosis (LNN) is a severe late adverse event. The purpose of this study was to identify risk factors for LNN and develop a model to predict LNN after radical re-irradiation with IMRT in patients with recurrent NPC. METHODS: Patients who underwent radical re-irradiation with IMRT for locally recurrent NPC between March 2001 and December 2011 and who had no evidence of distant metastasis were included in this study. Brain Metastasis in Nasopharyngeal Carcinoma: A Case Report.

INTRODUCTION: Nasopharyngeal carcinoma (NPC) is endemic in Southeast Asia. Despite the close proximity of the nasopharynx to the brain, true brain metastasis is rarely observed in NPC. CASE PRESENTATION: We present an interesting case of NPC with brain metastasis which to our knowledge is the first case reported in Southeast Asia. The patient was asymptomatic, and brain metastasis was detected during surveillance PET scan. He underwent craniectomy and excision of brain metastasis followed by Gamma Knife to the tumour cavity and remains well 2 years post resection. CONCLUSION: We explore seve

Nasopharyngeal classical Hodgkin lymphoma: a case report.

BACKGROUND: Hodgkin lymphoma is a malignant lymphoid neoplasm that most commonly originates in lymph nodes, particularly within the cervical, mediastinal, and axillary regions. Extranodal involvement is relatively uncommon, and primary nasopharyngeal Hodgkin lymphoma defines a rare clinical phenomenon. Diagnosis may be delayed because its clinical presentation resembles more common nasopharyngeal conditions. CASE PRESENTATION: A 51-year-old Palestinian woman presented with progressive nasal obstruction and snoring. Endoscopic examination revealed a mass in the nasopharynx. Histopathological a

Neutrophils in nasopharyngeal carcinoma: from mechanisms to therapeutics.

BACKGROUND: Neutrophils are the most abundant circulating leukocytes and are raipidly recruited to inflammatory sites as key effectors of innate immunity. Nasopharyngeal carcinoma (NPC), an Epstein-Barr virus (EBV)-associated epithelial malignancy endemic in Southeast Asia and North Africa, develops within a chronically inflamed and immunologically specialized tumor microenvironment (TME). Current standard therapies (concurrent chemoradiotherapy and induction chemotherapy) face challenges of recurrence and metastasis, highlighting the need to explore the role of neutrophils in NPC progression

Prediction of treatment outcome in nasopharyngeal carcinoma patients.

BACKGROUND: Currently, the American Joint Committee on Cancer staging systems do not reliably predict outcomes in nasopharyngeal carcinoma as patients with similar tumour stages and treatments often experience divergent outcomes. This study aimed to develop prediction models for overall survival (OS), progression-free survival (PFS), locoregional control (LRC), and distant control (DC) based on routinely collected clinical parameters in nasopharyngeal carcinoma patients treated with (chemo)radiation to support personalised treatment strategies. METHODS: Data from 956 patients treated with inte

The current role of immunotherapy in treating nasopharyngeal carcinoma.

Immunotherapy has emerged as a cornerstone in the management of both recurrent/metastatic nasopharyngeal carcinoma (R/M NPC) and locoregionally advanced NPC (LANPC). Evidence indicates that programmed death-1/programmed death ligand-1 (PD-1/PD-L1) inhibitors, whether administered as monotherapy or in combination with chemotherapy or antiangiogenic agents, can significantly improve outcomes in R/M NPC. For LANPC, incorporating immunotherapy into induction, concurrent chemoradiotherapy, and adjuvant phases has demonstrated potential benefits in enhancing antitumor immune responses and reducing t

Induction Paclitaxel-Cisplatin-Capecitabine for Nasopharyngeal Carcinoma.

BACKGROUND: We previously reported higher failure-free survival (FFS) with induction paclitaxel, cisplatin, and capecitabine (TPC) compared with cisplatin and fluorouracil (PF) in high-risk locoregionally advanced nasopharyngeal carcinoma (LA-NPC). We now report 5-year FFS, with prespecified secondary outcomes. METHODS: In this multicenter, randomized trial, patients with high-risk LA-NPC (T4N0-2M0 or TanyN3M0) received two 21-day cycles of induction chemotherapy with TPC or PF, followed by concurrent chemoradiotherapy. The primary endpoint was FFS; secondary endpoints included distant metast

Metastasis of nasopharyngeal carcinoma to the spleen: a case report.

BACKGROUND: Splenic metastasis from nasopharyngeal carcinoma (NPC) is exceptionally rare, accounting for only about 1% of all splenic metastases from solid tumors. Fewer than 30 cases have been documented worldwide. NPC is a common head and neck malignancy in certain regions, with a tendency to metastasize to regional lymph nodes in approximately 70% of cases, while distant metastases occur in about 30%. The spleen is an unusual site for secondary involvement, possibly due to its anatomical and immunological characteristics. CASE DESCRIPTION: We report a 40-year-old male with a significant sm
