Diffusion tensor imaging of white matter integrity and pain-related outcomes in chronic nonspecific neck pain.

Chronic neck pain is associated with neuroplastic changes in brain structure, but its alterations in white matter integrity remain unclear. This study aimed to investigate white matter microstructural changes in patients with chronic nonspecific neck pain and their relationships with pain-related outcomes (i.e., pain duration, intensity, disability, extent and pressure pain thresholds (PPTs) over the neck). Using diffusion tensor imaging, fractional anisotropy (FA) and mean diffusivity (MD) were analyzed in 30 individuals with neck pain and 30 pain-free controls through whole-brain and region-

Rethinking neck-related arm pain: hypothetical clinical scenarios to differentiate the underlying IASP-defined pain mechanisms.

Neck-related arm pain is frequently encountered in clinical settings, yet its underlying pain mechanisms remain elusive. While such pain radiating from the neck to the arm is often attributed to injuries or diseases of the nervous system (neuropathic pain), it can also arise from nociceptive (referred) or nociplastic sources. Regrettably, patients exhibiting this specific pain distribution are frequently diagnosed with varying terms, including 'cervicobrachialgia', 'cervicobrachial neuralgia', 'cervicobrachial pain syndrome', and 'cervical radiculopathy'. The ambiguity surrounding these diagno

Shoulder muscle co-ordination during chronic and acute experimental neck-shoulder pain. An occupational pain study.

Little is known about the mechanisms leading to chronic neck-shoulder musculoskeletal disorders (MSD). The aim of the present study was to investigate and compare motor function during controlled, low load, repetitive work together with chronic or acute experimental neck-shoulder pain. The clinical study was performed on workers with (n = 12) and without (n = 6) chronic neck-shoulder pain. In the experimental study, experimental muscle pain was induced in healthy subjects by intra-muscular injection of hypertonic saline into the trapezius muscle (n = 10). The assessed parameters related to mot

Effect of pillow on pain, disability and sleep quality in patients with chronic neck pain: A systematic review.

Chronic neck pain impairs physical function and sleep quality. Appropriate pillow use may support spinal alignment, reduce muscle strain, and improve sleep. This systematic review evaluated the effectiveness of various pillow characteristics in alleviating pain, disability, and sleep disturbances among individuals with chronic neck pain. A comprehensive literature search was conducted in PubMed, Scopus, PEDro, and Cochrane Library for interventional studies published from 2015 to June 2024. Following PRISMA guidelines, the methodological quality was assessed using the PEDro scale, GRADE system

Global trends in low back pain and neck pain in the working population: implications for occupational health.

This study systematically evaluated the global burden of low back pain (LBP) and neck pain (NP) among individuals aged 20-65 from 1990 to 2021, utilizing data from the Global Burden of Disease (GBD) database. We analyzed incidence, prevalence, years lived with disability (YLDs), and age-standardized rates across 204 countries and regions, stratified by socio-demographic index (SDI). Key findings indicate a slight decline in LBP metrics but a mild increase in NP trends, with higher growth rates observed in females. Socioeconomic status significantly influenced these patterns, with developed cou

Estimating EQ-5D values from the Neck Disability Index and numeric rating scales for neck and arm pain.

OBJECT: The Neck Disability Index (NDI) and numeric rating scales (0 to 10) for neck pain and arm pain are widely used cervical spine disease-specific measures. Recent studies have shown that there is a strong relationship between the SF-6D and the NDI such that using a simple linear regression allows for the estimation of an SF-6D value from the NDI alone. Due to ease of administration and scoring, the EQ-5D is increasingly being used as a measure of utility in the clinical setting. The purpose of this study is to determine if the EQ-5D values can be estimated from commonly available cervical

Referrals to the specialist health service for patients with neck or back pain.

BACKGROUND: National guidelines recommend that patients with neck or back pain who show no signs of improvement after 4-6 weeks be referred to the specialist health service. The referral should include details of medical history, clinical examination and treatment. Limited use of diagnostic imaging is recommended for neck and back pain with no red flags. The aim of the study was to assess the quality of referrals from primary care to the specialist health service for neck and back pain. MATERIAL AND METHOD: We reviewed 632 referrals to the Neck and Back Outpatient Clinic at the Department of

Reduction of neck pain severity in patients with medication-overuse headache.

BACKGROUND: Neck pain and primary headache disorders are highly prevalent in populations and clinical cohorts. Medication-overuse headache (MOH) is a treatable secondary headache, mainly developing in migraine sufferers, that accounts for the majority of patients presenting to headache clinics. Nevertheless, the association between neck pain and MOH has not been reported. This study evaluated the prevalence and clinical course of neck pain in patients with MOH before and after MOH treatment. METHODS: We analyzed 635 MOH patients enrolled in a nationwide, prospective, multicenter MOH registry.

Alterations in kinematics of temporomandibular joint associated with chronic neck pain.

BACKGROUND: Temporomandibular disorder (TMD) is an umbrella term for pain and dysfunction of the temporomandibular joint (TMJ) and its associated structures. Patients with TMD show changes in TMJ kinematics and masticatory muscle activation. TMD is commonly comorbid with non-specific chronic neck pain (NCNP), which may be one of the risk factors for TMD. OBJECTIVES: This study aimed to investigate whether patients with NCNP have altered TMJ kinematics and masticatory muscle activity. METHODS: This was a cross-sectional exploratory study including 19 healthy participants and 20 patients with

Transdermal Buprenorphine for Pain Management Following a Neck of Femur Fracture.

INTRODUCTION: Pain management in patients with hip fractures can be challenging. Poor pain control is associated with reduced mobility and increased morbidity. Inadequate analgesia in patients with dementia is a concern. After using several different alternatives, transdermal buprenorphine was chosen as a standardised approach for analgesia in patients with fragility fracture in our hospital. There is limited evidence on the use of buprenorphine in this population. Our aim was to investigate the safety and effectiveness of transdermal buprenorphine in patients with hip fractures. METHODS: A r
