Clinical chorioamnionitis and histologic placental inflammation: association with early-neonatal sepsis.

OBJECTIVE: Chorioamnionitis and fetal inflammatory response syndrome (FIRS) are significant risk factors for early onset sepsis (EOS). Recently, the use "Intrauterine Inflammation or Infection or both" or triple I has been proposed, classifying cases into an isolated maternal fever, suspected triple I, or confirmed chorioamnionitis. Evidence suggests that the association between suspected chorioamnionitis and confirmed histological chorioamnionitis (HCA) is not consistent, as well as the impact of HCA on the development of EOS.We aimed to evaluate the association between suspected chorioamnion

Neonatal sepsis in Sana'a city, Yemen: a predominance of Burkholderia cepacia.

BACKGROUND: Neonatal sepsis is a global concern with increasing morbidity and mortality. The burden of neonatal sepsis is highest in developing countries, especially in those lacking proper surveillance systems. The causative pathogens and their drug-resistance levels vary between countries with emergence of multidrug resistance organisms. Thus, accurate records on the recent trends of organisms causing neonatal sepsis will provide vital information for appropriate intervention. We aimed to investigate neonatal sepsis, identify its associated factors and causative pathogens and to assess the a

Niche specialization and spread of Staphylococcus capitis involved in neonatal sepsis.

The multidrug-resistant Staphylococcus capitis NRCS-A clone is responsible for sepsis in preterm infants in neonatal intensive care units (NICUs) worldwide. Here, to retrace the spread of this clone and to identify drivers of its specific success, we investigated a representative collection of 250 S. capitis isolates from adults and newborns. Bayesian analyses confirmed the spread of the NRCS-A clone and enabled us to date its emergence in the late 1960s and its expansion during the 1980s, coinciding with the establishment of NICUs and the increasing use of vancomycin in these units, respectiv

Adaptive response of neonatal sepsis-derived Group B Streptococcus to bilirubin.

Hyperbilirubinemia is so common in newborns as to be termed physiological. The most common bacteria involved in early-onset neonatal sepsis are Streptococcus agalactiae, commonly called Group B Streptococcus (GBS). Whilst previous studies show bilirubin has antioxidant properties and is beneficial in endotoxic shock, little thought has been given to whether bilirubin might have antibacterial properties. In this study, we performed a transcriptomic and proteomic assessment of GBS cultured in the presence/absence of bilirubin. Our analysis revealed that increasing levels of bilirubin (>100 µmol/

Preterm labor and neonatal sepsis caused by intrauterine Helicobacter cinaedi infection.

Helicobacter cinaedi is a rare pathogen but known to cause bacteremia, cellulitis and enterocolitis. Recently, cases of involving various organs are increasingly reported such as endocarditis, meningitis, and kidney cyst infection. We report a case of intrauterine H. cinaedi infection leading preterm birth and neonatal sepsis. A 29-year-old pregnant women who was no underlying disease hospitalized due to threatened preterm labor at 22 weeks of gestation. Clinical findings showed uterine tenderness, fever, leukocytosis and elevated C-reactive protein. H. cinaedi was isolated from amniotic fluid

Prediction models for neonatal health care-associated sepsis: a meta-analysis.

BACKGROUND AND OBJECTIVES: Blood culture is the gold standard to diagnose bloodstream infection but is usually time-consuming. Prediction models aim to facilitate early preliminary diagnosis and treatment. We systematically reviewed prediction models for health care-associated bloodstream infection (HABSI) in neonates, identified superior models, and pooled clinical predictors. DATA SOURCES: LibHub, PubMed, and Web of Science. METHODS: The studies included designed prediction models for laboratory-confirmed HABSI or sepsis. The target population was a consecutive series of neonates with susp

Superficial cultures in neonatal sepsis evaluations. Impact on antibiotic decision making.

The authors performed a retrospective analysis of neonatal superficial cultures and their effect on antimicrobial decision making during a nine-month period at Nashville General Hospital. They obtained and reviewed charts of infants (n = 66) having paired superficial (skin and/or gastric aspirate) and deep (blood and cerebrospinal fluid) cultures for the evaluation of early-onset sepsis. Superficial cultures were positive for pathogens (any streptococcus or enteric gram-negative) in 15% (10/66) of cases. Antimicrobial decision making was affected in only one of these cases, and in a seemingly

Neonatal group B streptococcal sepsis: effects of late treatment with dazmegrel.

Neonatal group B streptococcal (GBS) sepsis produces pulmonary arterial hypertension and hypoxemia that are preventable by pretreatment with the selective thromboxane A2 synthase inhibitor, dazmegrel. In the present experiment we administered dazmegrel (8 mg/kg) 2 h after the initiation of a 2 1/2 h infusion of 5 X 10(8) GBS/kg/h in ten 2- to 3-wk-old piglets. The multiple inert gas elimination technique was used to measure intrapulmonary shunt and alveolar ventilation to pulmonary perfusion mismatching. Thromboxane B2, the stable metabolite of thromboxane A2, and 6-keto-prostaglandin F1 alpha

Escherichia coli as a Presentation of Fulminant Early-Onset Neonatal Sepsis.

Background: Neonatal sepsis is a bloodstream infection and constitutes a major cause of morbidity and mortality among newborns. The diagnosis is primarily based on a positive blood culture result. An additional diagnostic and therapeutic challenge may be congenital infection, particularly in preterm infants. This is associated with the lack of routine screening of the maternal genital tract, except for testing for group B streptococci during earlier stages of an uncomplicated pregnancy. Case presentation: This paper presents a case of a neonate with early-onset sepsis (EONS) caused by Escheric

Antibiotic strategies for neonatal sepsis: navigating efficacy and emerging resistance patterns.

UNLABELLED: Neonatal sepsis is a critical global health challenge, resulting in high morbidity and mortality. This systematic review and meta-analysis was designed to evaluate the effectiveness of different treatments with antibiotics on neonatal sepsis, as well as resistance patterns affecting treatment outcomes. PubMed, Scopus, and Web of Science articles from January 2005 to December 2024 were included in the analysis. Studies focused on mortality, treatment failure rates, and pathogen-specific resistance profiles. The review involved data from 37 articles, which included 8954 neonates. Fin

Development of a mouse model for Klebsiella pneumoniae-associated neonatal sepsis.

Klebsiella pneumoniae is a major cause of neonatal sepsis in low- to middle-income countries. With the proportion of multidrug-resistant K. pneumoniae strains increasing globally and the lack of novel antibiotics in the pipeline, vaccination to prevent infections due to K. pneumoniae is an alternative strategy to antibiotics. Several K. pneumoniae vaccines are currently in development; however, there are no small animal models of K. pneumoniae-associated neonatal sepsis that can be used to evaluate vaccine efficacy. Therefore, we developed and characterized a neonatal murine model of K. pneumo
