Identification of pressor regions activated by central cholinergic stimulation in rat brain.

The acetylcholinesterase inhibitor neostigmine (2 microg) was microinjected into the lateral cerebral ventricle (i.c.v.) of unanesthetized rats to activate central cholinergic receptors. Changes in arterial blood pressure were correlated with changes in Fos-like immunoreactivity in the hypothalamus and forebrain following cholinergic stimulation. Neostigmine increased mean arterial pressure by 39 +/- 3 mmHg at peak (P < 0.05) from a pretreatment level of 104 +/- 4 mmHg. Blood pressure remained elevated for more than 30 min. Distinct Fos-like immunoreactivity was found in the posterior hypothal…

Reciprocal changes of plasma glucose and ketone bodies in fasted and acutely diabetic rats after CNS stimulation.

To assess the effect of chemical stimulation of the central nervous system (CNS) on ketogenesis, we injected neostigmine (5 x 10(-8)mol) into the third cerebral ventricle in normal rats fasted for 48 h and fed rats with diabetes induced by streptozotocin (STZ, 80 mg/kg). The hepatic venous plasma levels of ketone bodies (3-hydroxybutyrate and acetoacetate), free fatty acids (FFA), and glucose were measured for 120 min after the injection of neostigmine under pentobarbital anesthesia. In the normal rats, plasma glucose levels were significantly increased but neither ketone bodies nor FFA were a…

Morphine: a dual effect at the canine choledochoduodenal junction.

The effects of i.v. bolus injections of morphine sulfate on resistance to flow through the choledochoduodenal junction were studied in acutely prepared dogs anesthetized with pentobarbital. Dose-response studies indicate that the spasmogenic effect of 0.1 mg/kg of morphine is greater than that of 1.0 mg/kg of morphine. These and other data indicate a combination of stimulatory and inhibitory activity with the relaxing activity occurring at higher doses. Inhibitory activity was potentiated by neostigmine, given 1 hr before morphine. Prior administration of atropine or vagotomy enhanced the stim…

Sugammadex vs neostigmine in post-anesthesia recovery: A systematic review and meta-analysis.

Residual neuromuscular blockade (RNB) is linked to an increased risk of perioperative adverse events. This study systematically evaluates the impact of neuromuscular blockade antagonists on postoperative complications and quality of recovery in surgical patients. We conducted a systematic review and meta-analysis to compare the efficacy of sugammadex and neostigmine. Comprehensive searches were performed across medical databases, including Web of Science, PubMed, Embase, and the Cochrane Library, with a final search date of April 6, 2025. A total of thirty-five randomized controlled trials (RC…

Pancuronium Bromide for Chemical Immobilization of Adult Nile Crocodiles (Crocodylus niloticus): A Field Study.

(1) Background: Pancuronium bromide is a neuromuscular blocker used for immobilizing crocodiles that can be reversed with neostigmine. A recommended drug dose has only been established for saltwater crocodiles (Crocodylus porosus), mostly based on trials in juveniles and subadults. After trialing a dose recommendation in a small cohort of nine Nile crocodiles (Crocodylus niloticus), we developed and applied a new dose recommendation for large adult Nile crocodiles. (2) Methods: we trialed and adapted a pancuronium bromide (Pavulon 4 mg/2 mL) dose in Nile crocodiles originally established for s…

How robust are the STRONGER and STIL-STRONGER studies?

In 2020, the Sugammadex vs Neostigmine for Reversal of Neuromuscular Blockade and Postoperative Pulmonary Complications (STRONGER) study provided evidence for the first time that use of sugammadex is associated with fewer postoperative pulmonary complications than use of neostigmine. In a recent publication in the British Journal of Anaesthesia, a secondary analysis of the same data, the Association Between Neuromuscular Blockade Reversal Agent Choice and Postoperative Pulmonary Complications (STIL-STRONGER) study, has produced similar evidence of the advantages of sugammadex over neostigmine …

Reversal with sugammadex in the absence of monitoring did not preclude residual neuromuscular block.

BACKGROUND: In Japan, routine clinical care does not normally involve the use of a monitoring device to guide the administration of neuromuscular blocking drugs or their antagonists. Although most previous reports demonstrate that sugammadex offers more rapid and reliable antagonism from rocuronium-induced neuromuscular blockade, this advantage has not been confirmed in clinical settings when no neuromuscular monitoring is used. In this multicenter observational study, we sought to determine whether sugammadex reduces the incidence of postoperative residual weakness compared with neostigmine w…

[Influence of a central cholinergic mode of action on the regulation of the intact and disturbed cerebral blood flow (author's transl)].

Cerebral chemical vasomotor reactivity and autoregulation were tested in normal baboons before and after intravertebral, intravenous and (or) intracarotid infusion of atropine and neostigmine. Furthermore, disordered cholinergic neurotransmission and dysautoregulation after acute experimental cerebral infarction have also been investigated. Intravertebral injection of atropine suppressed the increase of CBF by inhalation of 5% CO2 and enhanced the decrease of CBF induced by hyperventilation, but did not appreciably affect autoregulatory response. On the other hand, cerebral autoregulatory vaso…

Maintenance of atrial fibrillation in anesthetized and unanesthetized sheep using cholinergic drive.

Atrial fibrillation (AF) was induced electrically and the duration of AF was measured in six isoflurane-anesthetized sheep (weight range 54.5-72.7 kg), and in five unanesthetized sheep (weight range 60-75 kg). In the anesthetized sheep, AF was induced by direct electrical stimulation of the right atrium with a catheter electrode and the duration of AF was determined. Intravenous neostigmine (10 micrograms/kg IV) was administered and the duration of AF was again measured. Then cholinergic drive was increased by bilateral electrical vagal stimulation; AF was induced and the duration of AF was me…

Urinary Retention Following Inguinal Herniorrhaphy: Role of Neuromuscular Blockade Reversal.

BACKGROUND: Nondepolarizing neuromuscular blockade is reversed with neostigmine/glycopyrrolate or sugammadex. Anticholinergic glycopyrrolate decreases bladder detrusor muscle contractility, potentially leading to postoperative urinary retention (POUR). POUR commonly complicates inguinal herniorrhaphy. In this study we assess association between reversal technique and POUR. METHODS: Records of adult patients undergoing unilateral inguinal herniorrhaphy with neuromuscular blockade from January 2013 to September 2020 were reviewed for POUR (unplanned postoperative insertion of urinary catheter).…

A miracle that accelerates operating room functionality: sugammadex.

BACKGROUND: Sugammadex offers a good alternative to the conventional decurarisation process currently performed with cholinesterase inhibitors. Sugammadex, which was developed specifically for the aminosteroid-structured rocuronium and vecuronium neuromuscular blockers, is a modified cyclodextrin made up of 8 glucose monomers arranged in a cylindrical shape. METHODS: In this study, the goal was to investigate the efficacy of sugammadex. Sugammadex was used when there was insufficient decurarisation following neostigmine. This study was performed on 14 patients who experienced insufficient dec…

Potentiation of a functional autoantibody in narcolepsy by a cholinesterase inhibitor.

We have recently reported the presence of an immunoglobulin G (IgG) autoantibody (Ab) in patients with narcolepsy with cataplexy that abolishes spontaneous colonic migrating motor complexes (CMMCs) and increases smooth muscle tension and atropine-sensitive phasic contractions in a physiological assay of an isolated colon. In this study, we used the cholinesterase inhibitor, neostigmine, to explore the mechanism of the narcoleptic IgG-mediated disruption of enteric motor function in four patients with narcolepsy with cataplexy and to identify a pharmacological mimic of the Ab. Neostigmine poten…

[Comparison of two methods of pre-emptive infiltration anaesthesia for inguinal hernia repair].

BACKGROUND: Intraarticular injection of neostigmine has been reported to create long-lasting analgesia due to the blockade of muscarinic receptors and neuronociceptive transmitters. The aim of the study was to determine if similar effects could be achieved after adding neostigmine to bupivacaine during infiltration of the surgical area before inguinal hernia surgery. METHODS: Ninety patients (82 males and 8 females), aged 20-88 (mean 54.8) years, scheduled for Lichtenstein hernia surgery under spinal anaesthesia, were randomly allocated to three groups. The surgical area was infiltrated with …

Train-of-four as an index of neuromuscular block in cats: changes induced by atropine.

1. The present study reevaluates the effect of atropine on the rate of recovery from tetanic fade caused by intraarterial administration of neostigmine or antinicotinic agents in cat anterior tibial muscle preparations submitted to a train-of-four (TOF) pattern of nerve stimulation. The study also compares the sensitivity of the TOF and tetanic responses as indices of residual nondepolarizing block. 2. Neostigmine, hexamethonium and d-tubocurarine all produced short-lived TOF fade. Both single and TOF twitches were increased by neostigmine and depressed by the antinicotinic agents. 3. Prior ad…
