The treatment of minimal change nephrotic syndrome (lipoid nephrosis): cooperative studies of the Arbeitsgemeinschaft für Pädiatrische Nephrologie (APN).

Minimal change nephrotic syndrome (MCNS) is the most frequent type of nephrotic syndrome (NS) in childhood. During the last decade only little progress has been made in the understanding of the etiology and pathogenesis of MCNS. However, treatment has been improved pragmatically by controlled cooperative studies, which have lead to an unexpected degree of standardization in pediatric nephrology. The Arbeitsgemeinschaft für Pädiatrische Nephrologie (APN) is conducting its own cooperative studies in close contact with the International Study of Kidney Diseases in Children (ISKDC). In Steroid Stu

T-cell lineage acute lymphoblastic leukemia with chromosome 5 abnormality in a patient with Crohn's disease and lipoid nephrosis.

We describe a 17-year-old patient with a documented history of Crohn's disease (CD) and of minimal-change nephrotic syndrome (MCNS) in whom a diagnosis of T-cell acute lymphoblastic leukemia (ALL) was made. The diagnosis of ALL was established by morphological examination of bone-marrow aspirates and confirmed by means of immunophenotypic analysis showing the involvement of T-cell lineage leukemic cells. The lymphoid clone showed a karyotypic abnormality involving the long arm of chromosome 5 in a translocation (5;6). Few cases of CD complicated by ALL have been previously reported. The presen

Interleukin 12 upregulates the release of vascular permeability factor by peripheral blood mononuclear cells from patients with lipoid nephrosis.

The vascular permeability factor (VPF) is a lymphokine that has been shown to play a role in lipoid nephrosis (LN). Prior studies have shown that interleukin (IL) 12 promotes T helper type 1 differentiation and enhances production of T helper type 1 cytokines such as gamma interferon and IL-2. We, therefore, investigated the effects of recombinant human IL-12 on the release of VPF by peripheral blood mononuclear cells (PBMC) from LN patients. The VPF activity was measured according to the method of Ovary, with minor modifications. The goal of the present study was to examine the importance of

Recombinant granulocyte-macrophage colony-stimulating factor modulates in vitro function of the peripheral blood mononuclear cells in lipoid nephrosis.

The effect of recombinant human granulocyte-macrophage colony-stimulating factor (GM-CSF) on the in vitro proliferation of peripheral blood lymphocytes (PBL) was evaluated in patients with lipoid nephrosis (LN). The cytokine increased the proliferation of LN PBL in response to phytohemagglutinin (PHA), measured by the [3H]thymidine uptake. The effects were abrogated by antibody against human GM-CSF. We then investigated the effect of GM-CSF on the release of interleukin-1 (IL-1) from peripheral blood monocytes (PBM) in LN patients and normals. In vitro IL-1 production by LN PBM treated with li

Interleukin 10 and interleukin 13 synergize to inhibit vascular permeability factor release by peripheral blood mononuclear cells from patients with lipoid nephrosis.

It has been proposed that a vascular permeability factor (VPF) is involved in the pathogenesis of lipoid nephrosis (LN). There is now increasing evidence that interleukin 10 (IL-10) and interleukin 13 (IL-13) have regulatory effects on cytokine production by activated macrophages. These results prompted us to study the effects of recombinant human IL-10 and IL-13 on VPF secretion in LN. In the present study, we demonstrate that the regulatory cytokines IL-10 and IL-13 are potent inhibitors of the VPF activity of activated peripheral blood mononuclear cells. Each cytokine was found to suppress

[Morphology of the kidneys in children with primary nephrotic syndrome (lipoid nephrosis) before and after hormone therapy, studied by biopsy (light and electron microscopy)].

Light and electron microscopies were employed to examine biopsy specimens from 15 children suffering from the primary nephrotic syndrome with minimal alterations in renal bodies. The studies were carried out at the height of the disease and in the stage of a clinical remission after prednisolone therapy. The author describes the development of ultrastructural changes in the glomerular filter, primarily of those in podocytes, under the conditions of highly selective proteinuria as well as during hormonal therapy that favours reparative processes and discontinuation of protein "leakage". It is a
