Evaluation of Suspected Dementia.

Dementia is a significant and costly health condition that affects 5 million adults and is the fifth leading cause of death among Americans older than 65 years. The prevalence of dementia will likely increase in the future because the number of Americans older than 65 years is expected to double by 2060. Risk factors for dementia include age; family history of dementia; personal history of cardiovascular disease, cerebrovascular disease, diabetes mellitus, or midlife obesity; use of anticholinergic medications; apolipoprotein E4 genotype; and lower education level. The U.S. Preventive Services

Dystonia: A review.

Dystonia is a hyperkinetic movement disorder characterized by sustained or intermittent muscle contractions that cause abnormal repetitive movements, abnormal postures, or both. The new consensus classifies dystonia into two axes to characterize clinical characteristics, and etiology. This system allows correct identification of isolated and combined forms of dystonia and retains the description of generalized and focal dystonia which is very useful in planning investigations and management. The characterization of dystonia for its better identification and a brief overview of its management a

Reviewing the evidence base for the peripheral sensory examination.

BACKGROUND: Many students find the peripheral sensory examination confusing. We set out to summarise the evidence base in order to provide guidance on the most useful manoeuvres. METHODS: We performed a literature review starting with 5 secondary sources, supplemented by a literature search on MEDLINE. RESULTS: A useful approach to neuropathy is to divide these into large fibre sensory neuropathy (LFSN) in which vibration and proprioception are affected, and small fibre sensory neuropathy (SFSN) in which pain and temperature are affected. Positive sensory symptoms such as burning, electric o

Neuromuscular junction disorders.

Disorders of the neuromuscular junction, such as myasthenia gravis and Lambert-Eaton myasthenic syndrome, constitute an important and treatable class of diseases. Both disorders are typically caused by an immunologically mediated attack on discrete components of the neuromuscular junction, compromise the efficacy of neurotransmitter transmission, and produce clinically distinct syndromes of fatigable muscle weakness. Although the history, clinical examination, and routine antibody testing can be diagnostic in many cases, specialized neurophysiological tests, such as repetitive nerve stimulatio

Dynamics of neurological soft signs and its relationship to clinical course in patients with first-episode schizophrenia.

The aim of the study was to assess the dynamics of neurological soft signs (NSS) over four years from the clinical onset of schizophrenia, depending on the clinical course of the disease, and to evaluate the relationship of NSS to symptomatic dimensions in patients with first-episode schizophrenia. Sixty-eight patients with first-episode schizophrenia were included in the trial. The clinical status was assessed using Positive and Negative Syndrome Scale (PANSS) at the same time as the neurological examination, at admission to the hospital for first-episode schizophrenia and at a check-up exami

Neurological aspects of syncope and orthostatic intolerance.

Sudden falling with loss of consciousness from syncope and symptoms of orthostatic intolerance are common, dramatic clinical problems of diverse cause, but cerebral hypoperfusion is the ultimate mechanism in most. Cardiac, reflex, and orthostatic hypotension are important forms to consider. Syncope must be differentiated from seizures, psychiatric events, drop attacks, and other mimics. However, factors such as syncopal induced movements, ictal bradycardia, and insufficient clinical information can confound accurate diagnosis and hamper appropriate treatment. Progress in the diagnosis, treatme

Moyamoya-induced paroxysmal dyskinesia.

Moyamoya disease (MMD) is an uncommon intracranial vasculopathy that typically presents with ischemic or hemorrhagic stroke. Persistent choreoathetosis has been identified as a rare early manifestation of MMD. We present 2 patients with paroxysmal dyskinesia as the initial symptom of MMD, one resembling paroxysmal kinesigenic dyskinesia (PKD) and the other paroxysmal non-kinesigenic dyskinesia (PNKD). We also review the cases of moyamoya-induced chorea reported previously, none of which resembled PKD or PNKD. We hypothesize that both hormonal and ischemic factors may be implicated in the patho

[Neurologic concomitant diseases in Crohn disease].

Disturbances of the central and peripheral nervous system in Crohn's disease can be directly or indirectly caused by the disease itself or by the treatment. The first mentioned disturbances are very seldom. Cerebral arterial and venous thromboses can be attributed to hypercoagulation. Malabsorption of vitamin B 12 or folic acid can lead to a subacute combined degeneration of the spinal cord, sometimes in combination with a polyneuropathy and an encephalopathy. Spinal abscesses very seldom occur. An opticusneuropathy can be caused either by vasculitis or by a lack of vitamin A and/or vitamin B.

[Iridoplegia-areflexia syndrome (Weill/Reys-Holmes-Adie), symptoms and etiology as persistent herpes simplex virus infection].

Looking for additional symptoms and signs of the iridoplegia-areflexia-syndrome referable to its unknown etiology we have found spotlike trigeminal numbness, edema, and/or redness suggestive of herpetic origin and more often common signs of herpes simplex virus-infection (HSVI). Indeed the whole picture of the syndrome as we see and describe it, can be explained by typical features of the HSVI. These include persisting HSVI of only the distal sensible or vegetative neurones and recurrence of infection with further destruction of ganglia-cells. These criteria are also decisive whether symptoms

Effect of early and late syphilis on central nervous system: cerebrospinal fluid changes and neurological deficit.

Neurological examination and investigation of the cerebrospinal fluid (CSF) was performed on 24 patients with early and 180 patients with late syphilis. In 21 (12%) patients with late syphilis positive CSF treponemal test results and neurological deficits suggestive of symptomatic neurosyphilis were found. Concomitantly all but three patients with neurosyphilis showed one or more of the following abnormal CSF variables: CSF concentration of albumin X 10(3)/serum concentration (albumin ratio) greater than or equal to 7.9; mononuclear cells greater than 5 microliters: ratio of CSF to serum IgG c

The clinical examination for motor apraxia.

The signs and symptoms of motor apraxia are described both for ideomotor and ideational apraxia. Typical errors are described. For ideomotor apraxia a list of tasks is given. The many conceptual and psychometric problems that still await an answer are briefly discussed.
