Effects of a Muay Thai Championship on Neuromuscular Parameters and Their Relationship with Competitive Outcome: A Pilot Study.

Background: This study aimed to analyze the acute effects of an official Muay Thai championship on neuromuscular performance, assessing changes in upper- and lower-limb power before and after competition, as well as differences according to competitive outcome (winners vs. losers). Methods: Thirty-three amateur Muay Thai athletes (17 men and 16 women) performed a countermovement jump (CMJ) and a reactive push-up test (RPU) immediately before and after their bouts. Neuromuscular performance changes were analyzed using mixed-design ANOVA, and a binary logistic regression was conducted to examine

Nociception level index variations in ICU: curarized vs non-curarized patients - a pilot study.

PURPOSE: Pain is a major physiological stressor that can worsen critical medical conditions in many ways. Currently, there is no reliable monitoring tool which is available for pain monitoring in the deeply sedated ± curarized critically ill patients. This study aims to assess the effectiveness of the multiparameter nociception index (NOL®) in the critical care setting. We compared NOL with traditionally used neurovegetative signs and examined its correlation with sedation depth measured by bispectral index (BIS®) electroencephalographic (EEG) monitoring. METHODS: This retrospective monocentr

Gut microbiota and neuromuscular disorders: a comprehensive review.

Emerging evidence highlights a bidirectional interaction between the gut microbiota (GM) and the neuromuscular system, suggesting that microbial composition and metabolic activity may play a pivotal role in the pathophysiology of several neuromuscular diseases (NMDs). This comprehensive review synthesizes current evidence on GM alterations across a broad spectrum of NMDs, emphasizing their potential implications for disease mechanisms and management. Gut microbial shifts have been consistently associated with immune dysregulation, systemic inflammation, and metabolic disturbances that may cont

Sugammadex administration in patients with end-stage renal disease: a narrative review with recommendations.

Due to unknown safety concerns, sugammadex should not be administered to patients withend-stage renal disease (ESRD). However, because the supply of benzylisoquinolinium-typeneuromuscular blocking agents (NMBAs) has been discontinued, rocuronium is the onlynon-depolarizing NMBA that can be used in clinical settings in some countries, includingSouth Korea. The administration of sugammadex cannot be avoided to achieve rapid andcomplete neuromuscular recovery in patients with ESRD or renal transplantation after rocuronium administration. Although there has been a limited number of clinical studie

The increasing role of muscle MRI to monitor changes over time in untreated and treated muscle diseases.

PURPOSE OF REVIEW: This review aims to discuss the recent results of studies published applying quantitative MRI sequences to large cohorts of patients with neuromuscular diseases. RECENT FINDINGS: Quantitative MRI sequences are now available to identify and quantify changes in muscle water and fat content. These two components have been associated with acute and chronic injuries, respectively. Studies show that the increase in muscle water is not only reversible if therapies are applied successfully but can also predict fat replacement in neurodegenerative diseases. Muscle fat fraction corre

Electrophysiological characterization of neuromuscular synaptic dysfunction in mice.

Emerging evidence suggests that synaptic dysfunction occurs prior to neuronal loss in neurodegenerative diseases, such as amyotrophic lateral sclerosis (ALS). Therefore, monitoring synaptic activity during early stages of neurodegeneration may provide valuable information for the development of diagnostic and/or therapeutic strategies. Here, we describe an electrophysiological method routinely applied in our laboratory for investigating synaptic activity of the neuromuscular junction (NMJ), the synaptic connection between motoneurons and skeletal muscles. Using conventional intracellular sharp

Rapid arousal from anaesthesia after reversal of deep rocuronium-induced neuromuscular block with sugammadex in a neuroradiological procedure.

We describe the case of a 42-year-old man undergoing elective embolisation of an arteriovenous malformation at the level of the right posterior cerebral artery under propofol anaesthesia. We used the SedLine Brain Function Monitor (Masimo) to titrate the anaesthetic depth with propofol. At the end of the procedure, the patient aroused from anaesthesia very rapidly after the reversal of deep neuromuscular block with sugammadex, despite still significant calculated doses of propofol at the effect site. This rapid arousal was monitored using the SedLine Brain Function Monitor.

Sound-based cough peak flow estimation in patients with neuromuscular disorders.

INTRODUCTION/AIMS: Cough impairment is common in individuals with neuromuscular disorders and is associated with respiratory infections and shorter survival. Cough strength is assessed by measuring cough peak flow (CPF) using a flow meter, but this method requires a complex device setup and trained staff. The aim of the study is to evaluate the reliability of a smartphone app to estimate CPF based on cough sounds in a cohort of individuals with neuromuscular disorders. METHODS: Individuals with neuromuscular disorders underwent CPF measurement with a flow meter and a smartphone app. A CPF <27

Donepezil-related inadequate neuromuscular blockade during laparoscopic surgery: A case report.

BACKGROUND: Donepezil is an acetylcholinesterase inhibitor used to improve cognition and delay disease progression in dementia patients by increasing acetylcholine levels. This drug may potentially interact with neuromuscular blocking agents (NMBAs) that act on muscular acetylcholine receptors during general anesthesia. Herein, we present a case of inadequate neuromuscular blockade with rocuronium, a nondepolarizing NMBA, in a dementia patient who had taken donepezil. CASE SUMMARY: A 71-year-old man was scheduled for laparoscopic gastrectomy. He had been taking donepezil 5 mg for dementia. Ge

Implementation of a neuromuscular clinical trial network: a rare disease model for enhancing clinical trial readiness, capacity, and access in Canada.

BACKGROUND: Rare diseases affect over 300 million people globally, yet clinical trial conduct in rare disease populations remains complex due to small patient numbers, geographic dispersion, heterogeneous phenotypes, and limited trial infrastructure. Neuromuscular diseases (NMDs) exemplify these challenges. Coordinated trial networks have emerged as a strategy to improve feasibility, access, and trial performance. METHODS: Guided by the Consolidated Framework for Implementation Research (CFIR), we conducted a multi-phase qualitative needs assessment involving neuromuscular investigators and c

Severity of Myasthenia Gravis Influences the Relationship between Train-of-four Ratio and Twitch Tension and Run-down of Rat Endplate Potentials.

BACKGROUND: Train-of-four ratio (TOFR) is often used to evaluate muscle relaxation caused by neuromuscular-blocking agents (NMBAs). However, it is unknown whether TOFR reliably correlates with the first twitch tension (T1) in patients with myasthenia gravis (MG). By using rat models of experimental autoimmune MG (EAMG), the authors verified the hypothesis that the severity of MG influences the relationship between TOFR and T1. METHODS: EAMG rats were divided into sham, moderate MG, and severe MG groups. Isometric twitch tension of the hemidiaphragm was elicited by phrenic nerve stimulation wi

Role of neuromuscular blocking agents in the development of polyneuropathy and myopathy in critically ill patients.

BACKGROUND: Acquired critical illness weakness (AWCIP) is the most frequent neuromuscular disease in intensive care medicine departments. Its importance is given by the prolongation of hospital stay and the delayed recovery it causes to patients after hospitalization. The main objective of this study was to investigate the association between neuromuscular blocking agents and the development of acquired weakness in critically ill patients. MATERIAL AND METHODS: We conducted a prospective study of 103 critically ill patients who were periodically monitored with electromyography. RESULTS: The
