Brain tissue oxygen plus intracranial pressure monitoring versus isolated intracranial pressure monitoring in patients with traumatic brain injury: an updated meta-analysis of randomized controlled trials.

BACKGROUND: Intracranial pressure (ICP) monitoring plays a key role in patients with traumatic brain injury (TBI), however, cerebral hypoxia can occur without intracranial hypertension. Aiming to improve neuroprotection in these patients, a possible alternative is the association of Brain Tissue Oxygen Pressure (PbtO2) monitoring, used to detect PbtO2 tension. METHOD: We systematically searched PubMed, Embase and Cochrane Central for RCTs comparing combined PbtO2 + ICP monitoring with ICP monitoring alone in patients with severe or moderate TBI. The outcomes analyzed were mortality at 6 month

State of the art of neuromonitoring in thyroid surgery.

Thyroidectomy is a common procedure in endocrine surgery, frequently performed to treat benign and malignant thyroid conditions. Recurrent laryngeal nerve (RLN) injury, a major complication, underscores the necessity for meticulous nerve dissection during surgery. Intraoperative neuromonitoring (IONM) has emerged as a valuable adjunct to visual identification in RLN preservation. This review synthesizes current literature on IONM in thyroid surgery, emphasizing its role in enhancing RLN integrity assessment and reducing surgical complications such as vocal cord paralysis. IONM techniques inclu

Successful transcranial motor evoked potentials monitoring under remimazolam-based anaesthesia: Case reports of two patients.

INTRODUCTION: Intraoperative neurophysiological monitoring is a standard in surgeries with an increased risk of iatrogenic neurological injury development. The gold standard for this monitoring modality is propofol-based total intravenous anaesthesia (TIVA). Although there seems to be no clinically significant complication for propofol administration in patients with soy, egg, or peanut allergies, a whole range of recommendations still prohibit propofol administration in patients with these allergies. Remimazolam is a new, ultra-short-acting benzodiazepine that can potentially be an alternativ…

Intraoperative monitoring for tethered cord surgery: an update.

OBJECT: Intraoperative neurophysiological recording techniques have found increasing use in neurosurgical practice. The development of new recording techniques feasible while the patient receives a general anesthetic have improved their practical use in a similar way to the use of digital recording, documentation, and video technology. This review intends to provide an update on the techniques used and their validity. METHODS: Two principal methods are used for intraoperative neurophysiological testing during tethered cord release. Mapping identifies functional neural structures, namely nerve

Motor Evoked Potential-Based Spinal Cord Function Assessment in Intraoperative Monitoring Using Machine Learning Model.

BACKGROUND: Manual muscle test (MMT) scores have been used to evaluate the muscle strength of patients with spinal surgery, which is performed both preoperatively and postoperatively because surgery sometimes may lead to muscle weakness. Recently, intraoperative spinal cord monitoring (IOM) has been used to assess spinal cord functions during surgery based on motor evoked potentials (MEPs); however, its standardized evaluation method has not been established. Thus, prompt and objective assessment of spinal cord functions during surgery is still challenging. This study aimed to construct a mach

The Utility of Electroencephalography in Guiding General Anesthesia in Children.

Traditional pediatric anesthetic dosing using pharmacokinetic estimates based on age and weight is often imprecise, frequently leading to oversedation. Intraoperative electroencephalography (EEG) allows visualization of the brain's response to anesthetic agents in real time, facilitating precise titration of anesthetic drug doses optimized for the individual child. The goal of EEG-guided anesthesia management is to maintain an optimal state of hypnosis during various stages of the procedure while minimizing hemodynamic instability and other adverse effects of anesthesia. This is especially imp

Development of a new night-time noise index: Integration of neurophysiological theory and epidemiological findings.

BACKGROUND: The effects of environmental noise on sleep are of great interest to public health. Numerous studies have been conducted to investigate these effects; however, these previous studies applied existing sound-level statistics that were not based on neurophysiology. AIMS: This study aimed to develop a new night-time noise index based on neurophysiology and epidemiology. METHODS: First, we derived a formula for predicting the noise effects on sleep based on a neurophysiological model of brainstem sleep regulation, where awakening was associated with greater electrical potentials in th

Microsurgical treatment of spinal epidermoid and dermoid cysts in the lumbosacral region.

The aim of this study was to evaluate the characteristics and surgical outcome of spinal epidermoid and dermoid cysts in the lumbosacral spine. We reviewed 26 patients with spinal epidermoid and dermoid cysts (15 epidermoid and 11 dermoid) treated between October 2000 and December 2010. The latter 15 patients of this series underwent neurophysiological monitoring during surgery. Gross total resection of the tumour was achieved in 46.2% of patients, and 53.8% underwent subtotal resection. In 14 patients, the tumour capsule adhered so tightly to the neural structures that a section of the capsul…

Compound muscle action potentials and spontaneous electromyography can be used to identify and protect the femoral nerve during resection of large retroperitoneal tumors.

BACKGROUND: Resection of large retroperitoneal neoplasms may injure the femoral nerve, thereby causing a permanent neurological deficit. We used electrical neurophysiological monitoring to identify, map, and preserve the femoral nerve during surgical resection to reduce the risk of neurological deficit. METHODS: Seven patients with retroperitoneal neoplasms underwent eight resections. Compound muscle action potentials (CMAPs) were recorded from needle electrodes placed in the iliacus, quadriceps, and sartorius muscles. Spontaneous electromyography (EMG) was continuously monitored from the sam…

Near-complete tongue transection during spinal surgery with transcranial MEP monitoring: A case report.

RATIONALE: IONM, especially transcranial MEPs, is a cornerstone in preventing neural injury during spinal surgery. However, transcranial stimulation may result in forceful jaw closure, causing oral trauma such as tongue lacerations, dental injury, or rarely, airway compromise. This case underscores the importance of recognizing and preventing rare but serious complications related to intraoperative neuromonitoring. PATIENT CONCERNS: A 55-year-old female with progressive lower limb weakness and thoracic kyphotic deformity underwent multi-level posterior spinal fusion and vertebral column resec

Spinal Cord Stimulation: Neuromodulation for Intraoperative Neuromonitoring Personnel.

Intraoperative neuromonitoring (IONM) has emerged as a transformative tool in spinal cord stimulation (SCS) and dorsal root ganglion (DRG) procedures, enabling precise lead placement under general anesthesia. It also addresses the safety aspect of placement in an asleep patient while also mitigating the challenges of traditional awake techniques. This paper outlines the protocols and benefits of IONM, with a focus on electromyography (EMG) and somatosensory evoked potentials (SSEP) for real-time guidance during thoracic, cervical, and DRG stimulator placement. It explores how IONM optimizes my

Use of technologies in thyroid surgery: Latin American Thyroid Society Surgical Affairs Committee Expert Opinion. Part 1.

Thyroidectomyis the most frequent endocrine surgical treatment for problems such as goiter, thyroid cancer, and Graves' disease. The global incidence of goiter ranges from 5%-20%, with a notably high frequency in less wealthy countries, and the incidence of thyroid cancer is on the rise due to the greater use of diagnostic imaging. Despite medical options, surgery remains essential. Surgical advancements such as blood vessel sealing technology, intraoperative laryngeal nerve neuromonitoring (IONM), remote access surgery, and parathyroid fluorescence have transformed thyroid surgery. Vessel sea

Retromastoid Craniotomy: An Adaptable and Panoramic Approach to the Cerebellopontine Angle.

There are many approaches to the cerebellopontine angle, all of which involve different degrees of disruption of the normal anatomy. The retromastoid craniotomy has stood the test of time for the efficiency, panoramic exposure, and flexibility it provides while allowing excellent control over the vital cerebrovascular structures including the brainstem. This approach allows the surgeon not to get distracted by how to get there but instead to focus on what to do when he or she is there. Herein, I discuss the nuances of this simple approach with its "extended" variations.

The Diagnostic Properties of Intraoperative Ultrasound in Glioma Surgery and Factors Associated with Gross Total Tumor Resection.

OBJECTIVE: In glioma operations, we sought to analyze sensitivity, specificity, and predictive values of intraoperative 3-dimensional ultrasound (US) for detecting residual tumor compared with early postoperative magnetic resonance imaging (MRI). Factors possibly associated with radiologic complete resection were also explored. METHODS: One hundred forty-four operations for diffuse supratentorial gliomas were included prospectively in an unselected, population-based, single-institution series. Operating surgeons answered a questionnaire immediately after surgery, stating whether residual tumo

Evaluating Spelling in Glioma Patients Undergoing Awake Surgery: a Systematic Review.

A main goal of awake surgery is to preserve language in order to facilitate return to work and maintain quality of life. Although spelling has become crucial in daily life, it has received little attention in awake surgery practice. We review assessments of spelling carried out in awake surgery studies, to inspect how current neurofunctional theories of spelling may guide pre-, intra- and post-operative neurosurgical practice. A systematic database search in Embase, Medline, PubMed and Web of Science identified studies reporting on spelling assessment in glioma patients undergoing awake surger

Bilateral vocal cord palsy post thyroidectomy: lessons learnt.

Bilateral vocal cord paralysis is a known possible complication following thyroid surgery. It owes to the close relationship between the recurrent laryngeal nerve and the thyroid gland. The most feared complication of bilateral vocal cord paralysis is airway compromise. We report the case of a 39-year-old woman who underwent total thyroidectomy for multinodular goitre. The surgery was uneventful. However she developed stridor in the recovery bay needing intubation. We postulate that the cause was attributed to bilateral vocal cord paresis due to the use of the intraoperative nerve monitoring (
