Estimating the prevalence of neurosurgical interventions in adults with spina bifida using the Health Facts data set: implications for transition planning and the development of adult clinics.

OBJECTIVE: As the care of patients with spina bifida continues to evolve, life expectancy is increasing, leading to a critical need for tran … ida. The median age of this population has now reached early adulthood, and a significant number of neurosurgical procedures were performed in adults. An abrupt drop in the rate of encounters occurred at 21 years of age, possibly reflecting transition issues such as access-to-care problems and lack of coordinated care.

Daptomycin plus trimethoprim/sulfamethoxazole combination therapy in post-neurosurgical meningitis caused by linezolid-resistant Staphylococcus epidermidis.

Post-neurosurgical infection is a serious complication that occurs in approx. 4% of all patients undergoing neurosurgical procedures and is associated with high morbidity and mortality rates and prolonged length of intensive care unit (ICU) stay. Coagulase-negative staphylococci (CoNS), especially methicillin-resistant Staphylococcus epidermidis (MRSE), are the most frequent pathogens involved in CNS post-neurosurgical meningitis. Treatment is challenging especially in patients with meningitis due to multidrug- resistant (MDR) CONS. Herein, we report a unique case of post-neurosurgical meningi…

First experiences with a 2.0-microm near infrared laser system for neuroendoscopy.

Nd:YAG, argon and diode lasers have been used in neurosurgical procedures including neuroendoscopy. However, many neurosurgeons are reluctant to use these lasers because of their inappropriate wavelength and uncontrollable tissue interaction, which has the potential to cause serious complications. Recently, a 2.0-microm near infrared laser with adequate wavelength and minimal tissue penetration became available. This laser was developed for endoscopic neurosurgical procedures. It is the aim of the study to report the initial experiences with this laser in neuroendoscopic procedures. We have pe…

Robotic systems in cranial surgery: a systematic review of accuracy, clinical outcomes, and adverse events.

Robotic-assisted surgery is becoming more prevalent in neurosurgery. Many studies have classified the advantages and disadvantages, particularly in spinal robotic surgeries. However, the use of robotics in cranial procedures has been limited to individual studies without large-scale review of the different modalities. This study is a comprehensive systematic review of the major robotics utilized in cranial neurosurgical procedures and their respective outcomes. Following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, a systematic review was conducted ov…

Current Neurosurgery.

Neurosurgery is based on neuroscience, physiology and medical physics. Therefore, neurosurgery has also developed along with discoveries and innovations in these fields. The present article outlines the areas of neurosurgery and their development until 2022. Technology for the preservation of the central nervous system and cranial nerve function has made remarkable progress through the integration of diagnostic imaging and functional evaluation capabilities. Endovascular treatment strategies of cerebrovascular disorders have also progressed. The procedures have not only shifted from craniotomy

A Unique Case Report of Leclercia adecarboxylata Causing Monomicrobial Neurosurgical Infections.

Leclercia adecarboxylata is an emerging gram-negative pathogen. Historically, it has primarily affected immunocompromised individuals and has mainly been isolated from polymicrobial infections. Yet, advances in diagnostic techniques have led to increased recognition of its incidence and clinical relevance. In this case series, two instances of monomicrobial L. adecarboxylata infections following neurosurgical procedures in immunocompetent patients are presented. The first case occurred after spinal laminectomy, and the other case caused external ventricular drain ventriculitis. Both patients r…

Neuroprotective effects of propofol following global cerebral ischemia in rats.

Propofol has cerebral vascular and metabolic effects similar to those of barbiturates, and it is used to maintain neurosurgical anesthesia because it reduces cerebral metabolic rate, cerebral blood flow, and intracranial pressure. Although the use of propofol as a cerebral protectant during certain neurosurgical procedures has been advocated, consensus has not been reached as to a protective effect of propofol on cerebral ischemia. In this study we observed the neuroprotective effects of propofol during global cerebral ischemia-reperfusion injury by the use of four-vessel occlusion method in a…

Rupture along with fibrin clot occluding the middle port of the triple lumen catheter: an uncommon complication of central venous catheterisation.

Central venous catheter (CVC) insertion is a commonly done procedure but associated with some potential complications. In our case, intraoperatively we placed a CVC into the right subclavian vein of the patient for a neurosurgical procedure. Subsequently, in the neurosurgical intensive care unit, on checking the patency of the CVC, only the distal and proximal lumens were working and no backflow of blood was detected from the middle port. A chest X-ray and ultrasound were done immediately, which did not reveal why the middle port was blocked. Later CVC was removed, and on examination of the ca

[Treatment of trigeminal neuropathic pain by motor cortex stimulation].

The neurosurgical procedures currently available for the treatment of trigeminal neuralgia can induce trigeminal neuropathic pain. Severe forms of trigeminal neuropathic pain correspond to the classical facial anesthesia dolorosa, whose treatment is known to be very difficult. Chronic stimulation of the ventral posterolateral nucleus (VPL) of the thalamus was, in the past, the only neurosurgical therapy available to treat this complication. The long-term results have been disappointing, which opened the field to the development of other techniques, including stimulation of the motor cortex for…

Trigeminal trophic syndrome of all three nerve branches: an underrecognized complication after brain surgery.

The authors report a case of trigeminal trophic syndrome (TTS) that occurred as a complication of a neurosurgical procedure. Three years after a second surgical treatment for a meningioma of the cerebellopontine angle, this 32-year-old woman developed TTS with progressive skin ulcers on the left ala nasi and the left side of the forehead and chin. Trigeminal trophic syndrome is an extremely rare cause of facial ulceration. It occurs as a consequence of trigeminal nerve damage or impaired central sensory connections. To the authors' knowledge, this is the first report of lesions in the dermatom

Remote cerebellar haemorrhage from site of craniotomy: A report of two cases.

Remote cerebellar haemorrhage (RCH) is a rare complication of neurosurgical procedures seldom requiring intervention. We report two cases of RCH. The first unilateral RCH is asymptomatic, the other is bilateral and associated with supratentorial haemorrhage and hydrocephalus requiring intervention. We propose multiple foci of haemorrhage as an adverse prognostic marker in RCH.

Alternative hybrid access in endovascular neurosurgery: Scoping review and technical considerations.

BackgroundThe common femoral artery and radial artery constitute the most common access sites in contemporaneous endovascular neurosurgery. Oftentimes, it may be impossible to reach the vascular target safely due to different circumstances, necessitating alternative approaches. We aim to review these "last resource" access sites described in the literature, focusing on the technical aspects as a convenient reference.MethodsWe performed a comprehensive literature search of PubMed capturing articles from 1958 to 2022.ResultsWe identified nine alternative approaches for endovascular access, after

Introduction: Endovascular Neurosurgery.

Remarkable developments in the field of endovascular neurosurgery have been witnessed in the last decade. The success of endovascular therapy for ischemic stroke treatment is now irrefutable, making it an accepted standard of care. Endovascular treatment of cerebral aneurysms is no longer limited to primary coiling but now includes options such as stent or balloon assistance and flow diversion and applications utilizing neck reconstruction, intrasaccular, and bifurcation-specific devices. Balloons, liquid embolic agents, and flow-directed catheters have revolutionized the treatment of arteriov

Obesity related complications in surgery.

PURPOSE OF REVIEW: This article aims to review relevant implications of obesity and associated comorbid conditions for the otology and neurotology surgical subspecialties. RECENT FINDINGS: Both the incidence and prevalence of obesity are rising worldwide. This condition, along with its many associated comorbidities, can adversely affect the perioperative management of patients undergoing otologic and neurotologic procedures. Preoperative assessment of the cardiopulmonary system and airway are imperative as compromise of either may lead to death. Physiologic changes found in patients with obes
