Urothelial carcinoma (UC) is a significant global health challenge with heterogeneous clinical presentations from nonmuscle-invasive to metastatic disease. Circulating tumor DNA (ctDNA) has emerged as promising noninvasive biomarker for risk classification, treatment monitoring and recurrence detection. Systematic searches of PubMed identified 390 articles; 61 met inclusion criteria for plasma ctDNA analysis in UC. Independent dual screening, Joanna Briggs Institute assessment, and stratification by disease stage (nonmuscle-invasive bladder cancer [NMIBC], muscle-invasive bladder cancer [MIBC]

Non-muscle-invasive bladder cancer (NMIBC) is characterized by high recurrence rates and necessitates lifelong cystoscopic surveillance, underscoring the need for minimally invasive biomarkers to improve early detection and risk stratification. Therefore, this study aimed to investigate the role of trimethylamine-N-oxide (TMAO) and its precursors as diagnostic biomarkers for NMIBC. A total of 50 male patients with NMIBC (25 pTa and 25 pT1) were included in this study. Additionally, 52 age-matched healthy individuals were included as controls. Serum TMAO and its dietary precursors were quantifi

BACKGROUND: Intravesical Bacillus Calmette-Guérin (BCG) therapy is an established treatment for non-muscle-invasive bladder cancer, involving direct instillation of a solution containing attenuated Mycobacterium bovis into the bladder. Although generally well-tolerated, rare vascular complications, including mycotic aneurysms and aortoenteric fistulas, have been reported. CASE PRESENTATION: We report the case of a 76-year-old Japanese man who developed an aortoesophageal fistula three years after completing intravesical BCG therapy for superficial bladder carcinoma. The patient initially prese

Urothelial carcinoma (UC) remains a persistent clinical challenge in the post-Bacillus Calmette-Guérin (BCG) era, particularly because a substantial proportion of patients with high-risk non-muscle-invasive bladder cancer (NMIBC) experience recurrence or progression despite standard intravesical immunotherapy. Although immune checkpoint inhibitors (ICIs) and antibody-drug conjugates (ADCs) have expanded the therapeutic armamentarium, widely studied biomarkers such as tumor mutational burden (TMB) and PD-L1 expression show limited and inconsistent predictive performance in UC, which may partly

Mitomycin C (MMC) is a widely employed chemotherapeutic agent, particularly in non-muscle invasive bladder cancer (NMIBC), where it functions by inducing DNA cross-linking and promoting tumor cell apoptosis. However, the tumor microenvironment (TME) significantly influences the therapeutic efficacy of MMC. Among the key regulators within the TME, the complement system and the coagulation pathway play a crucial role in modulating immune responses to cancer therapies, including MMC. This article explores the interaction between platinum nanoparticles (PtNPs) with human serum (HS) of NMIBC patien

IMPORTANCE: Due to the ongoing bacillus Calmette-Guérin (BCG) shortage, sequential intravesical gemcitabine and docetaxel has been increasingly used as first-line therapy for high-risk non-muscle-invasive bladder cancer (NMIBC). However, data directly comparing these 2 therapies are lacking.
OBJECTIVE: To compare the outcomes of patients with high-risk NMIBC treated with gemcitabine and docetaxel vs BCG.
DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study was conducted from January 1, 2011, to December 31, 2021. The median (IQR) duration of follow-up was 23 (12-33) months for

PURPOSE: The advantages of en bloc resection of bladder tumors (ERBT) over transurethral resection of bladder tumors (TURBT) in terms of patient prognosis are not yet clear, and there are some technical limitations. We aimed to compare the tumor recurrence in non-muscle invasive bladder cancer (NMIBC) patients with tumor diameter ≥ 3 cm undergoing either TURBT or thulium laser ERBT.
METHODS: The patients included were those diagnosed with NMIBC based on pathological confirmation and underwent TURBT or modified thulium laser ERBT in the Department of Urology at Tongji Hospital from 2019 to 202

UNLABELLED: WHAT'S KNOWN ON THE SUBJECT? AND WHAT DOES THE STUDY ADD?: Intravesical bacille Calmette-Guérin (BCG) is generally considered to be contraindicated in immunologically compromised patients with bladder cancer because it may be ineffective and potentially toxic. Therefore, there is little experience with BCG in individuals with impaired immune systems. The present study provides evidence that intravesical BCG is safe and effective in the short term against non-muscle-invasive bladder cancer affecting patients who were receiving immunosuppressive medications. This included anti-reject

BACKGROUND AND OBJECTIVE: Sequential intravesical gemcitabine/docetaxel (Gem/Doce) is a potential alternative to bacillus Calmette-Guérin (BCG) for treating non-muscle-invasive bladder cancer (NMIBC). Intermediate-risk NMIBC group (IR-NMIBC) includes both low-grade (LG) and high-grade tumors (HG). This study investigates the response of HG and LG IR-NMIBC to Gem/Doce compared to BCG therapy.
METHODS: We included patients with IR-NMIBC who received either BCG or Gem/Doce between 2013 and 2023. Maintenance regimens were administered to patients without recurrence after induction for 1 year. Fol

The study aimed to explore the distribution of circulating tumor cells (CTCs), circulating tumor endothelial cells (CTECs), and their subtypes in non-cancer and bladder cancer individuals, focusing on their prognostic value in high-risk non-muscle invasive bladder cancer (NMIBC). Researchers analyzed 59 fresh peripheral blood samples using subtraction enrichment and immunostaining fluorescence in situ hybridization (SE-iFISH). Samples were collected from healthy individuals (n = 18), patients with benign urinary conditions (n = 2), newly diagnosed bladder cancer patients (n = 20), and NMIBC pa

BACKGROUND: Despite the high recurrence rate of non-muscle-invasive bladder cancer (NMIBC), there are limitations in accurately predicting recurrence after transurethral resection of bladder tumor (TURBT) based on clinicopathological factors alone. However, prediction of recurrence using biomolecular characteristics of bladder tumors has not been applied to clinical practice. The objective of this study was to establish a new gene expression scoring system for identifying patients at high risk of recurrence.
METHODS: NMIBC and normal bladder samples were subjected to microarray analysis to ob

OBJECTIVE: Recent preclinical and retrospective clinical evidence shows that androgen receptor (AR)-mediated signals have significant roles in development of non-muscle invasive bladder cancer (NMIBC). Here, we conducted a single-center, phase I study to assess the feasibility and efficacy of enzalutamide in patients having recurrent NMIBC with marker tumors. Patients with NMIBC who cannot achieve complete transurethral resection (TUR) or with recurrence within a year after the TUR, were enrolled. The patients were administered oral enzalutamide at 160 mg dose, once daily for four weeks. Clini

AIM: Patients with NMIBC frequently experience recurrences. Yet, the timing and characteristics of subsequent recurrences are understudied. We aim to describe subsequent recurrences in a large, contemporary, population-based cohort.
METHODS: We included 1915 patients from the UroLife study and the Nijmegen Bladder Cancer Study, diagnosed with primary NMIBC between 2011 and 2021. The conditional 1-, 3- and 5-year risks of first to fourth recurrence were calculated using Kaplan-Meier risks, stratified by clinicopathological factors. Patterns of subsequent tumors were described and visualized.

INTRODUCTION: Mitomycin C or bacillus Calmette-Guerin (BCG) intravesical instillations are the more widespread adjuvant treatments following transurethral resection of bladder tumor for intermediate and high-risk non-muscle invasive bladder cancer, but both are regularly in short supply worldwide. This Belgian survey assessed the changes of management during shortage period.
METHODS: On behalf of the two national societies of urology in 2023, an anonymous questionnaire was sent to 340 Belgian urologists exploring modifications treatments' practices in shortage period.
RESULTS: In case of Mit
