PURPOSE OF THIS REVIEW: This review will focus on the neglected side of metabolic support in ICU survivors: nutritional therapy after critical illness. Knowledge of the evolution of the metabolism of patients that survived critical illness will be bundled, and current practices will be investigated. We will discuss some studies conducted to determine resting energy expenditure in ICU survivors and which identified barriers that cause interruptions in the feeding process based on published data between January 2022 and April 2023.
RECENT FINDINGS: Resting energy expenditure can be measured usi

The major and most well-known function of vitamin D is to maintain calcium and phosphorus homeostasis and promote bone mineralization. However, recent evidence suggests that vitamin D may be important for a variety of nonskeletal outcomes. The review synthesizes the available evidence for the role of vitamin D in skeletal health as well as its novel roles in medical conditions such as muscle function, falls, immunity, glucose homeostasis, and cardiovascular diseases. The article reviews methods for assessing vitamin D status and suggests strategies to restore vitamin D status in patients requi

Although there are no official recommendations for specific nutrient intakes in premature infants after hospital discharge, it is agreed that the goal should be to achieve the body composition and rate of growth of that of a normal fetus of the same postmenstrual age during the entire first year of life. A general recommendation to use the special formulas designed for preterm infants after hospital discharge in place of the formulas for term infants cannot be made from the available evidence at this time. Infants fed human milk after discharge are of the greatest concern as human milk does no

32 out of 111 patients with major abdominal trauma were studied to evaluate the effects of total parenteral nutrition (TPN). There were 24 men and 8 women with average age of 31 years. 25 cases were recorded as blunt trauma. The others had penetrating wounds. An Abdominal Trauma Index (ATI) was used to score the injury severity. 68% (22 32 ) of the patients had a score of more than 15, and 25% (8 32 ) had a score over 25. TPN was carried out in 11 patients with complicated injuries of the pancreas and duodenum, in 15 cases with small bowel fistula secondary to extended intestinal trauma, and i

Outcome data for patients malnourished due to gastrointestinal problems requiring surgery show that morbidity and mortality are improved when nutrition support is provided. The effect is greatest for those patients whose prognostic nutrition index is high. For these individuals in particular, this support is mandatory for the other therapies provided to be effective. The principle underlying this support is the fact that protein deficit cannot be repaired by carbohydrates or fat, whether from body stores or an external source. Instead, protein has to be provided, either enterally or parenteral

Introduction: chronic kidney disease (CKD) is a major risk factor for end-stage renal disease, and owing to its increasing global prevalence, poses a serious public health challenge. Patients with CKD frequently experience protein-energy wasting, frailty, sarcopenia, and cachexia, necessitating complex dietary restrictions, including protein, salt, and potassium limitation. Although a nutrition support team (NST) can improve nutritional status in hospitalised patients, additional interventions such as swallowing evaluation, oral care, and medication adjustments are crucial for effective outcom

Nutritional support therapy is one of the extremely important treatment methods for patients in the intensive care unit. Timely and effective nutritional support regimens can improve patients' immune function, reduce complications, and optimize clinical outcomes. Energy expenditure is influenced by multiple factors, including patients' baseline characteristics (such as physical condition, gender, age) and dynamic changes in indicators (such as body temperature, nutritional support regimens, and therapeutic interventions). The currently recognized "gold standard" for accurately assessing energy…

BACKGROUND: Nutritional support is essential for the recovery of critically ill patients in intensive care units (ICUs). However, the clinical impact of dietitian-guided interventions, particularly on ICU length of stay and nutritional risk scores, remains underexplored. This study evaluated the effect of personalized, dietitian-guided nutritional support (DGNS) versus physician-directed care on ICU length of stay and nutritional status.
MATERIALS AND METHODS: In this quasi-experimental study conducted at two centers in Tehran between June 2022 and February 2023, adult ICU patients with an ex…

BACKGROUND: Acute diseases responsible for hospitalization negatively affect protein-calorie balance, as well as the capacity of patients to cope with it. In those who still retain spontaneous feeding, oral nutritional supplements (ONSs) administration is an effective support strategy, but evidence on the timing they should be provided along with nutritional counseling during hospitalization is lacking. Hence, we evaluated the efficacy of the systematic use of ONSs since hospital admission.
METHODS: In a single-site, open-label, randomized, controlled trial (NCT02763904; July 2016-July 2024),

Over the past 10 years, several clinical and experimental studies report the potential benefit of enteral nutrition as primary therapy after multiple system trauma. In this study, 98 patients sustaining blunt and penetrating trauma were randomised to receive either enteral or parenteral feeding for 15 days. There were significantly fewer infectious complications in patients randomised to receive enteral feeding with particular benefit shown in the most severely injured patients. Serum protein concentrations correlated with the clinical outcome with an increase in constitutive protein and decre
