Saccadic velocity measurements in strabismus.

Traditional evaluation of strabismus has included cover test measurements, evaluation of the range of ocular rotations, and an array of subjective sensory tests. These studies could not always differentiate paresis of an extraocular muscle from restrictions and from various neuro-ophthalmic motility disorders. The measurement of horizontal and vertical saccadic movements can provide an objective test of rectus muscle function. Using EOG, saccades can be recorded easily, inexpensively, and repeatably at any age. In ocular muscle paresis or paralysis, saccadic speed is reduced mildly to markedly

Oculomotor disorders and nystagmus.

The precision of visual perception depends on accurately directing the fovea to objects of interest, a process mediated by six supranuclear oculomotor systems that all converge in the brainstem: saccadic and smooth pursuit, vestibular optokinetic, fixation, and vergence. This chapter provides a clinically oriented overview of brainstem neuroanatomy relevant to eye movement control and links it to characterisitic patterns of gaze disturbance. After outlining the organization and functions of the oculomotor (III), trochlear (IV), and abducens (VI) nuclei and their connections via the medial long

Supranuclear Disorders of Eye Movements.

OBJECTIVE: This article reviews the basic anatomy and pathophysiology of the supranuclear ocular motor pathways, emphasizing clinical correlations. LATEST DEVELOPMENTS: More advanced neuroimaging, particularly functional MRI, has yielded insight into previously unclear mechanisms of eye movement control with potential clinical implications. A better understanding of the mechanisms of eye movement control has implications for translational research, resulting in improved and early diagnosis and better treatment options. ESSENTIAL POINTS: Dysfunction of the supranuclear ocular motor pathways t

Acute ventriculomegaly and gaze deviation after evacuation of subdural hematoma: report of an unusual case.

Subdural hematomas (SDHs) in infants present diagnostic challenges due to their diverse aetiologies ranging from non-accidental injury to metabolic or coagulopathic disorders. Clinical presentation includes vomiting, irritability, and bulging fontanelles. There are multiple surgical and non-surgical options available for management aimed at relieving mass effect. We report the unusual development of acute ventriculomegaly and downward gaze palsy in a clinically well 9-week-old boy following evacuation of bilateral subacute subdural hematomas. Neuroimaging revealed ventricular enlargement witho

Intrarater and interrater reliability of the dynamic gait index in post stroke patients with eye movement disorders.

BACKGROUND: The Dynamic Gait Index (DGI) is a useful tool that has been evaluated for its reliability in patients with vestibular disorders, elderly people and, in chronic stroke population. Present study was aimed to evaluate the intrarater and interrater reliability of the DGI to measure dynamic balance and gait performance in stroke patients with eye movement disorders. METHODS: A sample of 30 stroke patients suffering from eye movement disorders were recruited. Two Physical therapists assessed the intrarater and interrater reliability of the DGI in two testing sessions three days apart. I

Contrast sensitivity, optotype acuity and fixation eye movement abnormalities in amblyopia under binocular viewing.

INTRODUCTION: Visual function deficits are seen in amblyopic subjects during fellow and binocular viewing. The purpose of the study was to examine the relationship between Fixation Eye Movement (FEM) abnormalities and binocular contrast sensitivity and optotype acuity deficits in amblyopia. METHODS: We recruited 10 controls and 25 amblyopic subjects [Anisometropic = 6, Strabismic = 10, Mixed = 9]. We measured binocular contrast sensitivity at spatial frequencies 1,2, 4, 8, 12 and 16 and binocular and monocular optotype acuity using a staircase procedure. We recorded FEMs using high-resolution

Reading Difficulty Due to Oculomotor Dysfunction following Mild Traumatic Brain Injury: A Case Report.

Reading involves adequate coordination of the oculomotor system. As interlink consists of neuronal control, an insult to the brain might affect the signal processing and lead to oculomotor dysfunction that can affect reading performance. Appropriate training to enhance the oculomotor coordination is effective in such scenarios. The purpose of this case report is to highlight the role of neuro-optometric vision therapy as a management option in oculomotor-based reading difficulty.

Reading difficulties in amblyopia: Consequence of visual sensory and oculomotor dysfunction.

INTRODUCTION: Reading is a vision-reliant task, requiring sequential eye movements. Binocularly discordant input results in visual sensory and oculomotor dysfunction in amblyopia, which may contribute to reading difficulties. This study aims to determine the contributions of fixation eye movement (FEM) abnormalities, clinical type and severity of amblyopia to reading performance under binocular and monocular viewing conditions. METHODS: Twenty-three amblyopic patients and nine healthy controls were recruited. Eye movements elicited during fixation and reading of preselected passages were coll

The prevalence of internuclear ophthalmoparesis in a population-based cohort of individuals with multiple sclerosis.

BACKGROUND: Internuclear ophthalmoparesis (INO) occurs in 15-52% of individuals with multiple sclerosis (MS) and is reliably detected by infrared oculography. Methods for diagnosing INO with infrared oculography and the association between INO and MS characteristics need confirmation. We aimed to describe INO prevalence and the clinical characteristics of individuals with MS and INO in a population-based cohort of individuals with MS born in the year 1966 (Project Y). METHODS: Previously described thresholds for the versional dysconjugacy index (VDI), assessed with standardized infrared oculo

Paroxysmal tonic upgaze accompanied by occipital discharge on electroencephalography: a case report and literature review.

BACKGROUND: Paroxysmal tonic upgaze (PTU) is an infantile-onset paroxysmal neurological disorder that is characterized by episodes of sustained conjugate upward eye deviation. The paroxysmal abnormal eye movements need to be differentiated from seizures. We report a case of PTU with occipital discharge on electroencephalography (EEG), which made the diagnosis more complicated. CASE PRESENTATION: A 6-month-old girl presented with paroxysmal upward deviation or left strabismus of the eyes, with a bowed head, lowered jaw, raised eyebrows, closed lips, and slight grin. Each episode lasted for a f

Acute vestibular syndrome: is skew deviation a central sign?

OBJECTIVE: Skew deviation results from a dysfunction of the graviceptive pathways in patients with an acute vestibular syndrome (AVS) leading to vertical diplopia due to vertical ocular misalignment. It is considered as a central sign, however, the prevalence of skew and the accuracy of its test is not well known . METHODS: We performed a prospective study from February 2015 until September 2020 of all patients presenting at our emergency department (ED) with signs of AVS. All patients underwent clinical HINTS and video test of skew (vTS) followed by a delayed MRI, which served as a gold stan

Wall-Eyed Monocular Internuclear Ophthalmoplegia Syndrome in Midbrain Infarction.

Wall-eyed monocular internuclear ophthalmoplegia (WEMINO) is a rare variant of internuclear ophthalmoplegia (INO), consisting of unilateral INO and ipsilateral exotropia. This distinctive syndrome is probably associated with damage to the medial longitudinal fasciculus. However, WEMINO caused by a midbrain lesion has not been previously reported. We herein report a 50-year-old man presenting with WEMINO and vertical gaze dysfunction resulting from infarction of the midbrain tegmentum.

Congenital Cranial Dysinnervation Disorders: A Literature Review.

Congenital cranial dysinnervation disorders (CCDD) is a new term describing a collection of non-progressive neurogenic syndromes. Initially referred to as congenital fibrosis syndrome, it was thought that the primary problem was extraocular muscular maldevelopment. Recent advancements in genetics and neuro-radiology have now determined the initial observation of fibrotic muscles is secondary to a primary lack of innervation from deficient, absent, or misguided cranial nerves. This presentation provides an overview of the known genes and phenotypes currently recognized within the CCDD domain. I

[Characteristic asymmetric abnormal eye movement and dystonic posture as the first symptoms of alternating hemiplegia of childhood].

A 3-month-old girl exhibited asymmetric abnormal eye movement and unilateral dystonic posture intermittently after the first few days of life. Unilateral ocular deviation or nystagmus were the main signs of abnormal eye movements. She was suspected to have alternating hemiplegia of childhood (AHC) despite the absence of apparent alternating hemiplegic episodes. Gene analysis revealed a de-novo missense mutation (Asp801Asn) of ATP1A3. AHC is a rare neurodevelopmental disorder characterized by recurrent transient attacks of hemiplegia affecting the unilateral or bilateral side of the body; in mo

Parinaud's syndrome due to an unilateral vascular ischemic lesion.

A 59-year-old man who complained of binocular vertical diplopia after an exploratory laparotomy, complicated by cardiorespiratory arrest during anesthetic induction, was found to have Collier's sign, anisocoria, complete paralysis of upward vertical gaze associated with convergence-retraction nystagmus on attempted upgaze and skew deviation with hypertropia in the left eye without ptosis, and an absent Bielschowsky sign. Magnetic resonance imaging of the brain showed a small lesion in the left paramedian midbrain compatible with microvascular ischemic sequelae. This patient was diagnosed with
