Novel 4D-MRI of tumor infiltrating vasculature: characterizing tumor and vessel volume motion for selective boost volume definition in pancreatic radiotherapy.

BACKGROUND: Pancreatic ductal adenocarcinoma has dismal prognosis. Most patients receive radiation therapy (RT), which is complicated by respiration induced organ motion in upper abdomen. The purpose of this study is to report our early clinical experience in a novel self-gated k-space sorted four-dimensional magnetic resonance imaging (4D-MRI) with slab-selective (SS) excitation to highlight tumor infiltrating blood vessels for pancreatic RT. METHODS: Ten consecutive patients with borderline resectable or locally advanced pancreatic cancer were recruited to the study. Non-contrast 4D-MRI wit

Numerical study of dynamic glottis and tidal breathing on respiratory sounds in a human upper airway model.

BACKGROUND: Human snores are caused by vibrating anatomical structures in the upper airway. The glottis is a highly variable structure and a critical organ regulating inhaled flows. However, the effects of the glottis motion on airflow and breathing sound are not well understood, while static glottises have been implemented in most previous in silico studies. The objective of this study is to develop a computational acoustic model of human airways with a dynamic glottis and quantify the effects of glottis motion and tidal breathing on airflow and sound generation. METHODS: Large eddy simulati

Self-motion perception is sensitized in vestibular migraine: pathophysiologic and clinical implications.

Vestibular migraine (VM) is the most common cause of spontaneous vertigo but remains poorly understood. We investigated the hypothesis that central vestibular pathways are sensitized in VM by measuring self-motion perceptual thresholds in patients and control subjects and by characterizing the vestibulo-ocular reflex (VOR) and vestibular and headache symptom severity. VM patients were abnormally sensitive to roll tilt, which co-modulates semicircular canal and otolith organ activity, but not to motions that activate the canals or otolith organs in isolation, implying sensitization of canal-oto

Infection after farm machine-related injuries in children and adolescents.

Infection played an important role in prolonging hospitalization and increasing morbidity in 68 children injured in farm settings. Predominantly gram-negative enteric organisms, group D streptococci, and anaerobic organisms were isolated in cultures of specimens obtained from wounds. Infection was more often associated with severe injuries of the large bones of the extremities than in amputation injuries of the digits and crush or roll-over injuries when the skin barrier was intact. The occurrence of infection in farm injuries was associated with prolonged hospitalization for parenteral antibi

Coronary microvascular abnormality in the reversible systolic dysfunction observed after noncardiac disease.

Acute reversible left ventricular wall motion abnormalities mimicking myocardial stunning have been reported with noncardiac disease and their coronary angiograms did not demonstrate organic stenosis or vasospasm in the epicardial coronary arteries. Thus, this mechanism has not yet been fully clarified. Two patients are reported as demonstrating acute reversible wall motion abnormalities after noncardiac disease. The electrocardiographic and echocardiographic findings mimicked myocardial stunning and confirmed the previous reports. The coronary angiograms did not show any corresponding coronar

Septic arthritis of the wrist.

Twenty-eight patients (two of them children) with septic arthritis of twenty-nine wrists were treated with early surgical drainage, parenteral antibiotics, and early motion after surgical decompression. The etiology was trauma in seventeen patients, and Staphylococcus aureus was the organism that was most commonly recovered on culture. In twenty-two patients (twenty-three wrists) who were followed for six months to nine years there were no recurrences. The results were evaluated in terms of range of motion, grip strength, and subjective complaints of discomfort and disability. Of the ten wrist

A Soft Robotic Model for Simulating Heart Valve Disease and Cardiac Interventions.

The heart is a dynamic, multisystem organ whose motion is dictated by a complex but organized muscular architecture. Each component is vital to its performance, with the valves being especially critical for ensuring pumping efficiency, yet they are among the most common sites of pathology. Treatment of heart disease is being delivered via minimally invasive procedures to improve patient outcomes and experience. The development of new surgical tools and procedures would greatly benefit from a stable, controllable, and biomimetic simulation platform that accurately replicates the heart's biomech

Magnetic Resonance Imaging-Guided Adaptive Radiotherapy for Colorectal Liver Metastases.

Technological advances have enabled well tolerated and effective radiation treatment for small liver metastases. Stereotactic ablative radiation therapy (SABR) refers to ablative dose delivery (>100 Gy BED) in five fractions or fewer. For larger tumors, the safe delivery of SABR can be challenging due to a more limited volume of healthy normal liver parenchyma and the proximity of the tumor to radiosensitive organs such as the stomach, duodenum, and large intestine. In addition to stereotactic treatment delivery, controlling respiratory motion, the use of image guidance, adaptive planning and

Properties of pedestrians walking in line. II. Stepping behavior.

In human crowds, interactions among individuals give rise to a variety of self-organized collective motions that help the group to effectively solve the problem of coordination. However, it is still not known exactly how humans adjust their behavior locally, nor what are the direct consequences on the emergent organization. One of the underlying mechanisms of adjusting individual motions is the stepping dynamics. In this paper, we present first quantitative analysis on the stepping behavior in a one-dimensional pedestrian flow studied under controlled laboratory conditions. We find that the st

Phenotypic integration in the carnivoran backbone and the evolution of functional differentiation in metameric structures.

Explaining the origin and evolution of a vertebral column with anatomically distinct regions that characterizes the tetrapod body plan provides understanding of how metameric structures become repeated and how they acquire the ability to perform different functions. However, despite many decades of inquiry, the advantages and costs of vertebral column regionalization in anatomically distinct blocks, their functional specialization, and how they channel new evolutionary outcomes are poorly understood. Here, we investigate morphological integration (and how this integration is structured [modula

Reciprocal functional interactions between the respiration/circulation center, the upper spinal cord, and the trigeminal system.

The interplay of neural discharge patterns involved in "respiration", "circulation", "opening movements in the mandible", and "locomotion" was investigated electrophysiologically in a decerebrate and arterially perfused in situ rat preparation. Sympathetic tone increased with increases in perfusion flow rate. All nerve discharges became clearly organized into discharge episodes of increasing frequency and duration punctuated by quiescent periods as the perfusion flow rate increased at 26°C. The modulated sympathetic tone at 10× total blood volume/ min activated the forelimb pattern generator a

Technological strategies to estimate and control diffusive passage times through the mucus barrier in mucosal drug delivery.

In mucosal drug delivery, two design goals are desirable: 1) insure drug passage through the mucosal barrier to the epithelium prior to drug removal from the respective organ via mucus clearance; and 2) design carrier particles to achieve a prescribed arrival time and drug uptake schedule at the epithelium. Both goals are achievable if one can control "one-sided" diffusive passage times of drug carrier particles: from deposition at the mucus interface, through the mucosal barrier, to the epithelium. The passage time distribution must be, with high confidence, shorter than the timescales of muc

Versatility of the Novalis system to deliver image-guided stereotactic body radiation therapy (SBRT) for various anatomical sites.

Stereotactic radiosurgery (SRS) and fractionated stereotactic radiotherapy (FSRT) programs to treat brain tumors were implemented when we first acquired the Brainlab Novalis system in 2003. Two years later, we started an extra-cranial stereotactic radio-ablation or more appropriately a stereotactic body radiation therapy (SBRT) program using the Brainlab Novalis image-guided system at The Methodist Hospital in Houston, Texas. We hereby summarize our initial experience with this system in delivering image-guided SBRT to a total of 80 patients during our first year of clinical implementation, fr

Asymmetry-induced chiral dynamics in coupled self-propelled robots: Spinning and circular motion.

Motivated by the chiral motility of microswimmers, we investigate how geometric asymmetry in a system of two self-propelled active Brownian robots coupled by a spring gives rise to rich emergent dynamics. We demonstrate that asymmetry in the robots' propulsion directions generates net torques that induce persistent rotational motion. Depending on the choice of propulsion angles α_{1} and α_{2}, the system exhibits three distinct dynamical regimes-run-and-tumble motion, circular trajectories, and spinning-with the geometric configuration primarily determining the realized regime. We further sho

In silico investigation of conformational motions in superfamily 2 helicase proteins.

Helicases are motor proteins that play a central role in the metabolism of DNA and RNA in biological cells. Using the energy of ATP molecules, they are able to translocate along the nucleic acids and unwind their duplex structure. They have been extensively characterized in the past and grouped into superfamilies based on structural similarities and sequential motifs. However, their functional aspects and the mechanism of their operation are not yet well understood. Here, we consider three helicases from the major superfamily 2--Hef, Hel308 and XPD--and study their conformational dynamics by u

Biomimetic Janus supramolecular colloidal motors for bioenergy ATP synthesis and storage.

Precise control of motion is fundamental to living organisms, enabling the execution of complex behaviours. Although previous supramolecular colloidal motors driven by asymmetrically assembled FOF1-ATP synthase have achieved biomimetic motion, their homogeneous surface properties and random enzyme distribution hinder directional propulsion and complicate mechanistic analysis. Here, we present Janus rotary biomolecular motor-powered supramolecular (RBMS) colloidal motors fabricated via a controlled assembly approach. These motors feature a polyethylene glycol (PEG)-gold hemispherical shell on o

Crystals on the move: mechanical effects in dynamic solids.

When exposed to external stimuli such as heat or light, certain single crystals can acquire momentum and undergo motion. On a molecular scale, the motility of such dynamic single crystals is triggered by a phase transition or chemical reaction without gaseous products, and macroscopically manifests as either slow (reversible or irreversible) deformation, or as rapid, almost instantaneous propulsion of the crystals that is oftentimes accompanied by disintegration. While the elastic energy of the slow reconfiguration processes such as bending, twisting and coiling can be utilized for actuation o

Modeling Normal Mouse Uterine Contraction and Placental Perfusion with Non-invasive Longitudinal Dynamic Contrast Enhancement MRI.

The placenta is a transient organ critical for fetal development. Disruptions of normal placental functions can impact health throughout an individual's entire life. Although being recognized by the NIH Human Placenta Project as an important organ, the placenta remains understudied, partly because of a lack of non-invasive tools for longitudinally evaluation for key aspects of placental functionalities. Non-invasive imaging that can longitudinally probe murine placental health in vivo are critical to understanding placental development throughout pregnancy. We developed advanced imaging proces

Experimental pathogenesis of hydrops.

1. The best method for inducing hydrops in guinea pigs is obliteration of the endolymphatic duct. 2. The location of hydrops produced in the guinea pig is comparable to that of Menière's disease. 3. The major histopathological finding in the guinea pig is atrophy of sensorineural elements in the apical turns of the cochlea. 4. The vestibular sensory cell population is rarely decreased, though ultrastructural changes are noted. 5. Acute hyperglycemia is related to gastrointestinal symptoms in motion sickness: an experimental study.

Motion sickness is caused by exposure to unfamiliar motions and typical symptoms of motion sickness include nausea and vomiting. To observe the metabolic and hormonal differences between nausea/vomiting (NAV) subjects and non-nausea/vomiting (NNV) ones, and to understand how the differences in metabolites and hormones affect the tolerance of organism to acceleration, 60 volunteers were exposed to repetitive acceleration using a 6-degree-of-freedom ship motion simulator. Meanwhile, 36 rats were randomly divided into three groups: an acceleration model group (n=14, exposed to acceleration), insu
