Brackets, epitopes and flash memory cards: a futuristic view of clinical orthodontics.

Orthodontics continues to be a profession anchored in traditional technology using appliances that cause inflammatory periodontal ligament (PDL) responses. Existing concepts of biological tooth movement based largely on histological tissue observations and the application of physical principles require major reassessment. In the next millennium, the genome revolution and knowledge of protein production and control could lead to the genetic correction of dentofacial anomalies and pain-free, biomolecular methods of malocclusion correction and long-term stability. A fundamental change is likely t…

Interest of decompression in orthodontics: Case report of a keratocyst during childhood.

Keratocysts, also known as keratocystic odontogenic tumors (KCOT), make up 7% of all odontogenic cysts and develop asymptomatically in most cases. Enucleation is the benchmark treatment. However, there are surgical alternatives. Marsupialization and decompression are necessary in some cases, mainly when the KCOT is large, thus causing an increased risk of bone fracture, or if it has engulfed important elements such as the inferior alveolar nerve or teeth, consequently causing alveolar bone growth failure. The authors describe the case of a nine-year-old child treated for a large keratocyst sit

Minimal presurgical orthodontics for a skeletal Class III patient with mandibular asymmetry.

A 19-year-old man with a skeletal Class III malocclusion was treated using minimal presurgical orthodontics. Orthodontic appliances and miniscrews were placed at the beginning of treatment, and the double-jaw-surgery was performed once the maxillary right and left first premolars were intruded, without worsening the concave profile and facial asymmetry presurgically. Different from the traditional combined orthodontic-orthognathic surgery, the jaw discrepancy was corrected first, followed by the orthodontic tooth movement. Miniscrews were used to intrude the premolars presurgically because of …

Evolution of occlusion and temporomandibular disorder in orthodontics: Past, present, and future.

Occlusion has been an important consideration in orthodontics since the beginning of the discipline. Early emphasis was placed on the alignment of the teeth, the stability of the intercuspal position, and the esthetic value of proper tooth positioning. These factors remain important to orthodontists, but orthopedic principles associated with masticatory functions must also be considered. Orthopedic stability in the masticatory structures should be a routine treatment goal to help reduce risk factors associated with developing temporomandibular disorders.

Experimental evidence of pharmacological management of anchorage in Orthodontics: A systematic review.

INTRODUCTION: Orthodontic anchorage is one of the most challenging aspects of Orthodontics. Preventing undesired movement of teeth could result in safer and less complicated orthodontic treatment. Recently, several reviews have been published about the effects of different molecules on bone physiology and the clinical side effects in Orthodontics. However, the effects of local application of these substances on the rate of orthodontic tooth movement have not been assessed. OBJECTIVES: The aim of this research was to analyze the scientific evidence published in the literature about the effects…

Components of overbite correction in lingual orthodontics: molar extrusion or incisor intrusion?

OBJECTIVE: The aim of this study was to evaluate the components of overbite correction with the lingual Incognito(®) technique. MATERIALS AND METHODS: The study was based on 45 files of patients with overbite treated with the Incognito(®) technique. The evaluation of overbite correction is assessed by comparing lateral cephalograms before and after treatment using QuickCeph 2000(®) software. All the statistical calculations were performed using Statview II software for Mac(®). RESULTS: The results revealed the efficiency of the Incognito(®) technique in overbite correction despite the absenc

Psychological aspects of orthodontics in clinical practice. Part two: general psychosocial wellbeing.

Orthodontists tend to treat/see their patients on a systematic, recurrent basis, often during crucial stages of psychological development. Therefore, they have a pivotal role in identifying a number of psychological as well as of psychiatric disorders. Effective communication is crucial and unfortunately, it is often underestimated in a busy clinical practice. Aim of part two of this article it to review the role clinical orthodontics and the orthodontist-patient relationship have on the patients' psychosocial wellbeing, including effects on self-esteem, bullying and harassment by peers, and e…

A European Board of Orthodontics case report. Case category: severe skeletal discrepancy.

OBJECTIVES: this 18.1 year-old girl presented with a chief complaint of progressive worsening of facial and dental esthetics, crowding, headache and facial pain. MATERIALS AND METHODS: clinically, she was at the end of her growth and exhibited a severe facial asymmetry, but with normal sagittal and vertical cranial relationships. Clicking in the right TMJ was evident. This was accompanied by a deviation upon opening, and pain in the joint. The pain she experienced during jaw movement, and upon palpation, was significant. There was a shift to the right from centric relation to intercuspal posi

External root resorption in orthodontics: avoiding high risk teeth. A case report.

External root resorption is one of the most exasperating and least understood clinical problems in orthodontics. Numerous studies have been conducted, attempting to establish the factors causing root resorption of permanent teeth. Considerable variation exists however, in the extent and distribution of root resorption, seen in the orthodontic treatment of apparently similar cases. After an excellent review of the literature, Brezniak and Wassertein (1993) concluded that the most significant disgnostic aid in predicting whether root resorption would occur during orthodontic treatment, is the ra…

Orthodontic root resorption.

External apical root resorption (EARR) is one of the most frequently reported iatrogenic side effects of orthodontic movement. Nevertheless, … rds an earlier detection without radiographs, will mark future developments in the field of EARR in orthodontics.

[Respiration in orthodontic practice].

In this paper, the author presents his method of dealing with breathing problems when he conducts an orthodontic consultation. Because they can interfere with growth, the author believes that ventilation disorders should be dealt with promptly so that they cannot cause skeletal discrepancies. After having reviewed the literature, the author presents his clinical procedures for uncovering and treating breathing disorders and then shows treated cases to illustrate his techniques, one of the most important of which, palate splitting, is simple, effective and virtually without risk. In addition to
