Otoendoscopy in the Office and Operating Room.

Since its introduction into the field of otology in 1967 endoscopes are gaining acceptance in evaluation and treatment of middle ear disease. Endoscopes offer a wide field view enabling looking "around the corner" with reduced need for soft tissue and bone removal. Outcomes of middle ear surgery for cholesteatoma and need for second-look procedures are improving because of the addition of endoscopic evaluation. Trainee education using the endoscope improves knowledge of middle ear anatomy. The portability of the endoscopic unit allows performing ear surgery in remote locations.

Fatigue analysis of tympanic membrane after ossiculoplasty.

CONCLUSION: A tentative conclusion was made that the finite element method can be used to investigate the fatigue life of the tympanic membrane after ossiculoplasty; the main reason of the extrusion of the tympanic membrane may not be blamed to the fatigue mechanism under normal sound pressure. OBJECTIVE: This study was to investigate the extrusion of the prosthesis from the tympanic membrane at post-ossiculoplasty by finite element method. METHODS: A finite element model of the human middle ear has been constructed and used as the model of the constructed middle ear at post-ossiculoplasty.

Comparison of the cuff pressure of a TaperGuard endotracheal tube and a cylindrical endotracheal tube after lateral rotation of head during middle ear surgery: A single-blind, randomized clinical consort study.

BACKGROUND: Positional change affects the cuff pressure of an endotracheal tube (ETT) in treacheally intubated patients. We compared the cuff pressure of a TaperGuard ETT and a cylindrical ETT after lateral rotation of head during middle ear surgery. METHODS: Fifty-two patients aged 18-70 years underwent a tympanomastoidectomy under general anesthesia were randomly allocated to receive endotracheal intubation with cylindrical (group C, n = 26) or TaperGuard ETTs (group T, n = 26). After endotracheal intubation, the ETT cuff pressure was set at 22 cmH2O in the neutral position of head. After l

The Tightrope Facial Nerve-An Unsupported Mastoid Segment After Resection of Recidivistic Cholesteatoma.

OBJECTIVES/HYPOTHESIS: Cholesteatomas are epidermal inclusion cysts containing stratified squamous epithelium that arise in the middle ear and mastoid cavities resulting in a persistent inflammatory state. Complications include chronic otorrhea, granulation tissue, and bony erosion. Cholesteatoma growth patterns predict frequent involvement of the Fallopian canal of the facial nerve. Extensive disease may extend to the posterior and middle fossa dura, the otic capsule, the carotid artery, and the jugular bulb. Dehiscence of the Fallopian canal and direct involvement of the facial nerve epineur

Hearing preservation after classical translabyrinthine removal of a vestibular schwannoma: case report and literature review.

The translabyrinthine approach is one of the favoured access routes for removal of vestibular schwannomas; however, total hearing loss in the operated ear is a predictable consequence. Here, we report a case in which a patient maintained serviceable hearing almost six years after classic translabyrinthine surgery. Possible explanations for the hearing preservation are discussed, as well as the feasibility of a modified translabyrinthine approach in attempting preservation of hearing following vestibular schwannoma surgery.

Is the middle fossa approach the treatment of choice for intracanalicular vestibular schwannoma?

OBJECTIVES: To compare the 2 surgical techniques most commonly used during vestibular schwannoma (VS) surgery, i.e., the middle fossa (MF) and the retrosigmoid-transmeatal (RS-TM) routes, when hearing preservation is attempted. STUDY DESIGN: A longitudinal study of a series of consecutive patients operated on with the 2 techniques by the same surgeon was conducted. Selection criteria included tumor confined to the internal auditory canal (IAC) with a length ranging from 4 to 12 mm and hearing class A or B. Patients were alternately assigned to 1 of the 2 groups regardless of auditory class an

[Pathogenesis and treatment of chronic purulent otitis media and its complications: immunological aspects].

Clinicoimmunological, microbiological and cytological examinations were made in 96 patients with otitis media purulenta chronica (OMPC) and otogenic intracranial complications. It was found that in different clinical forms of OMPC nonspecific defense and immunity indices vary. In OMPC and chronic purulent mesotympanitis the indices change insignificantly. In chronic purulent epitympanitis and otogenic intracranial complications there was suppression in T-cell immunity and nonspecific defense. Middle ear secretion contained less SIgA and lysozyme, blood contained more amount of IgA and IgM and

Vestibular surgery for Ménière's disease in the elderly: a review of techniques and indications.

There are several safe and effective surgical options for the treatment of vertigo associated with Ménière's disease in the elderly patient. The choice of procedure depends on the patient's hearing status and overall health. Since 1987, combined retrolabyrinthine-retrosigmoid posterior fossa vestibular neurectomy was performed on 29 older patients who were in good physiologic health and whose hearing was better than 80 dB pure-tone average and 20% speech discrimination. Patients whose hearing is worse are offered transmastoid labyrinthectomy or transmeatal cochleovestibular neurectomy, barring

A modified technique for firm elevation of the reconstructed auricle.

Reconstruction of the external ear for microtia remains one of the most challenging clinical problems in reconstructive surgery. Whether the reconstructive effort replicates a normal ear relates to making a delicate ear cartilage framework and achieving a firm projection of the constructed auricle. In this study, we present a new technique to create a firm elevation. 46 patients with congenital microtia, 30 male and 16 female, are included in this series. We used a modified Nagata's two-stage technique. In the second stage, instead of using a temporoparietal fascial flap to wrap a cartilage we
