Measuring sensory and pain thresholds by Semmes-Weinstein monofilaments in patients with leg ulcers: a pilot study.

OBJECTIVE: Pain is a common and disabling symptom in patients with leg ulcers. Clinical quantification of pain mostly depends on subjective pain reports, which do not reveal underlying mechanisms. The aim of this pilot study is to identify mechanisms underlying the pain in patients with leg ulcers by documenting alterations in pain processing using quantitative sensory testing. METHODS: In nine ulcer patients the mechanical sensory thresholds and the mechanical pain thresholds were determined by Semmes-Weinstein monofilaments (SWM) at three different sites: on the contralateral (unaffected) l

Validity of clinical measures of smooth pursuit eye movement control in patients with idiopathic neck pain.

BACKGROUND: Electrooculography is useful in detecting smooth pursuit neck torsion (SPNT) abnormalities in patients with neck pain, however, a validated, clinically relevant measure is lacking. OBJECTIVES: To explore the validity of visual assessment of formal and clinical videotaped SPNT tests in comparison to electrooculography. DESIGN: Cross-sectional observational study. METHOD: Twenty patients with idiopathic neck pain (INP) and twenty healthy controls performed the electrooculography SPNT test: first in neutral, then 45° trunk-under-head torsion to the left then right. The formal video

Estimation of clinically important change for visual analog scales measuring chronic temporomandibular disorder pain.

AIMS: To estimate the clinically important change (CIC) on a 100-mm visual analog scale for pain intensity (VAS-PI) by relating it to the patient's global impression of change (PGIC) in patients with chronic temporomandibular disorder (TMD) pain and to assess the dependency of the CIC on their baseline pain scores. METHODS: Data from a prospective cohort study with 588 patients with chronic TMD pain were analyzed. The CIC was estimated over a 3-month period, and receiver operating characteristic methods were used to assess the optimal cut-off point. The PGIC category of "much improved" served

Long-term skin temperature measurements - a practical diagnostic tool in complex regional pain syndrome.

Despite the development of the IASP criteria, diagnosing complex regional pain syndrome (CRPS) remains a challenge because all symptoms vary interindividually, including the vascular abnormalities. Previous studies showed that skin temperature asymmetries between the affected and contralateral extremity around 2 degrees C are useful for diagnosing CRPS. However, they were either assessed only at one single point in time or during specific investigations including controlled thermoregulatory modulation of sympathetic activity which limits their practicability. The present study evaluated long-t

Development of a multidimensional measure for recurrent abdominal pain in children: population-based studies in three settings.

OBJECTIVE: Recurrent abdominal pain (RAP) is a common problem in children and adolescents. Evaluation and treatment of children with RAP continue to challenge physicians because of the lack of a psychometrically sound measure for RAP. A major obstacle to progress in research on RAP has been the lack of a biological marker for RAP and the lack of a reliable and valid clinical measure for RAP. The objectives of this study were (1) to develop and test a multidimensional measure for RAP (MM-RAP) in children to serve as a primary outcome measure for clinical trials, (2) to evaluate the reliability

Effectiveness of protective measures in reducing risk of radiation exposure in interventional pain management: a prospective evaluation.

It is estimated that over 50% of approximately 4 million interventional procedures performed annually in the United States are performed utilizing fluoroscopy. Fluoroscopy offers validity to interventional techniques by providing precise localization of anatomic target areas and facilitating accurate delivery of injectate. Exposure to ionizing radiation is an unavoidable consequence while performing fluoroscopic procedures. The primary source of radiation to the physician during interventional procedures is from scatter reflected back from the patient. Multiple investigators have prospectively

Phantom limb telescoping in individuals with limb loss: links to anxiety, depression, and pain-related measures.

Phantom limb pain (PLP) and residual limb pain (RLP) have been widely studied following limb loss; however, the role of telescoping, the perceived shortening of the phantom limb, remains poorly understood in pain and psychosocial outcomes. Using a cross-sectional observational design, this study examined whether PLP and RLP intensity, pain interference, and psychosocial functioning differ between individuals who report telescoping and those who do not. Fifty-one adults with limb loss (mean age = 49.5 years, SD = 15.4) completed measures of PLP and RLP intensity (0-10 numeric rating scale), tel

Establishing clinical significance thresholds for common pediatric pain-related patient-reported outcome measures.

Pediatric patient-reported outcome measures (PROMs) are essential in pain intervention research, yet their changes often lack clear clinical significance. The authors estimated minimally important differences (MIDs) for commonly used and freely available PROMs in pediatric pain intervention research. Using data from a longitudinal multicenter study (N = 453) on children and adolescents aged 8 to 17 years with chronic pain in Germany, the authors applied equipercentile linking to align pre-post intervention changes in PROMs with global ratings of change. The MID for improvement in the Functiona

The association between diverse dietary quality measures and the presence of acute or chronic pain in the UK Biobank.

Chronic pain affects approximately 20% of adults worldwide and is a leading cause of disability. Emerging evidence suggest that nutrition may influence pain perception. However, the association between dietary quality and various pain conditions remains underexplored. This study examines associations between multiple dietary indices and the presence of acute and chronic pain analysing data from 55,721 UK Biobank participants. Multinomial logistic regression models were used to assess associations between diet quality (Dietary Inflammatory Index [DII], Healthy Diet Index [HDI], Healthful Plant-

The VISA-C Questionnaire: A Self-Administered Assessment to Measure Finger/Hand/Wrist Pain in Climbers.

BACKGROUND: Rock climbing places high loads through the hands, wrists, and fingers, leading to high injury rates, with the highest proportion in the fingers. Until now, there has been no attempt to categorize pain in the forearm to assess readiness to train. The purpose of this study was to create a questionnaire, the VISA-C (Victorian Institute of Sports-like Assessment; C for climbing), to measure forearm pain and determine how pain limits training. METHODS: We asked rock climbers aged 18 to 50 years old, who met the participation criteria and gave consent, to complete a survey containing 8

How is recovery defined and measured in patients with low back pain? A mixed study systematic review.

BACKGROUND: Despite the burden of low back pain (LBP) there is no currently accepted definition for its recovery, nor is there a gold standard for measurement. In addition, it is currently unclear how the perspective of patients are used in making recovery determinations. The purpose of this mixed study systematic review across both quantitative and qualitative literature was to (1) explore how recovery has been defined and measured for patients experiencing LBP, and (2) examine how the perspectives of patients and providers for recovery of LBP align or differ. METHODS: This was a mixed study

The development and cognitive testing of a patient-reported experience measure for patients with chronic pain of temporomandibular disorders.

INTRODUCTION: The importance of the patients' clinical experience has been reinforced several times over the last decade by healthcare organisations and policy makers. Routine gathering of experience data can help in enhancing patient-centred care and provide guidance to quality improvement schemes. Patient-reported experience measures can help to that end. The aim of this study was to develop a patient-reported experience measure to evaluate the experience of patients with temporomandibular disorders while receiving healthcare. METHODS: Input from several sources was utilised to develop the

The Influence of Personality Type on Patient Outcome Measures and Therapeutic Alliance in Patients with Low Back Pain.

BACKGROUND: Low back pain (LBP) has been shown to have various biological, psychological, and social factors that affect prognosis. However, it is unclear how personality may influence self-reported outcome measures and therapeutic alliance (TA). METHODS: Eysenck's personality inventory was used to assess personality, while the numeric pain rating scale (NPRS), Oswestry Disability Index (ODI), Tampa Scale of Kinesiophobia (TSK), Global Rating of Change (GROC), and the Working Alliance Inventory (WAI) measured patient progress and relationship strength. All outcome measures were formulated in

Measuring Effects on Pain and Quality of Life after Abobotulinum Toxin A Injections in Children with Cerebral Palsy.

Sixty-seven percent of children with cerebral palsy (CCP) experience pain. Pain is closely interrelated to diminished quality of life. Despite this, pain is an overlooked and undertreated clinical problem. The objective of this study was to examine the analgesic effect of a single lower extremity intramuscular injection of Abobotulinum toxin A/Dysport in CCP. Twenty-five CCP with at least moderate pain (r-FLACC ≥ 4) during passive range of motion were included. Localized pain and pain in everyday living were measured by r-FLACC and the Paediatric Pain Profile (PPP), respectively. Functional im
