Rehabilitation program for children with brachial plexus and peripheral nerve injury.

An aggressive and integrated physical and occupational therapy program is essential in the treatment of congenital brachial plexus injuries and other severe upper extremity nerve injuries. This article addresses the evaluation, identification of needs, establishment of goals, and the approaches to rehabilitation treatment for patients with brachial plexus palsy and other peripheral nerve injuries. Rehabilitative therapy can preserve and build on gains made possible by medical or surgical interventions; however, therapy is vital to these children regardless of whether surgery is indicated. The

Diaphragmatic paralysis in children: a review of 11 cases.

We reviewed 11 pediatric cases of diaphragmatic paralysis related to nonspinal-cord injury which were managed in our Intensive Care Unit over the past 10 years. Three cases were secondary to birth trauma, 7 followed surgical procedures for congenital heart disease, and 1 occurred in association with injuries sustained in a motor vehicle accident. The paralysis was bilateral in 8 children. The diagnosis was initially suspected on clinical grounds because of respiratory distress, impossibility of weaning from the ventilator, and paradoxical abdominal respiratory movements. Confirmatory investiga

[Obstetrical injuries of the newborn infant. Retrospective study apropos of 20,409 births].

We have been able to find 291 cases of obstetric trauma when we analysed the 20,409 deliveries which took place in the Pitié maternity between 1971 and 1980. It has been shown that bony lesions did well in the long and short term but nerve lesions had a far less good prognosis. This pathology should become far less when it is possible to recognize better when the fetus is large and when such traumatising manoeuvres as high forceps and assisted deliveries of large breech babies are stopped.

Rare case of a newborn baby with left-sided Erb's palsy and a contralateral/right-sided paralysis of the diaphragm.

Brachial plexus birth injury (BPBI) and phrenic nerve injury can sometimes occur concurrently in neonates following difficult deliveries like breech presentation, shoulder dystocia, forceps or vacuum extraction. Phrenic nerve palsy should be suspected in a newborn with respiratory distress and an elevated hemidiaphragm on the imaging studies in presence of the associated risk factors. The right side is affected more often than the left side and most of it is associated with BPBI. We present here a rare case of a newborn baby with a left-sided Erb's palsy and a contralateral/right-sided diaphra

Severe brachial plexus palsy in women without shoulder dystocia.

OBJECTIVE: Neonatal brachial plexus palsy frequently is described in conjunction with shoulder dystocia complicating a vaginal delivery. In this study, we present a series of cases of severe brachial plexus palsy that occurred without shoulder dystocia. METHODS: Cases were identified from deliveries at the Los Angeles County and University of Southern California Medical Center. Various maternal and neonatal characteristics were analyzed. RESULTS: Eight cases without shoulder dystocia but with severe brachial plexus palsy requiring neonatal intensive care unit admission were identified. None

The value of preoperative and intraoperative electromyography in the management of obstetric brachial plexus injury.

The treatment of obstetric brachial plexus palsy (OBPP) with neuroma-in-continuity is controversial. The recent literature advocates excision of neuroma-in-continuity in OBPP and repair with nerve graft irrespective of its neurophysiological conductivity. This approach risks sacrificing the regenerating axons, and the result has not yet been proven to be superior to neurolysis alone. In this case report, the authors aim to outline their strategy of using the combination of preoperative and intraoperative clinical and neurophysiological findings to aid their decision making. The lack of upper t

Intraoperative nerve action and compound motor action potential recordings in patients with obstetric brachial plexus lesions.

OBJECT: A typical finding in supraclavicular exploration of infants with severe obstetric brachial plexus lesions (OBPLs) is a neuroma-in-continuity with the superior trunk and/or a root avulsion at C-5, C-6, or C-7. The operative strategy in these cases is determined by the intraoperative assessment of the severity of the lesion. Intraoperative nerve action potential (NAP) and evoked compound motor action potential (CMAP) recordings have been shown to be helpful diagnostic tools in adults, whereas their value in the intraoperative assessment of infants with OBPLs remains to be determined. Primiparity: a risk factor for brachial plexus injury in the presence of shoulder dystocia?

The objective of the study was to assess the risk factors associated with obstetric brachial plexus injury. It was a retrospective analysis over a 7-year period, of women whose labours were either complicated by shoulder dystocia or had neonates who sustained brachial plexus injury. The 133 women included were divided into two groups: (1) Non-brachial plexus injury (Non-BPI) group: 106 women with labours complicated by shoulder dystocia. (2) Brachial plexus injury (BPI) group: 27 women whose neonates sustained BPI. Comparison of ante-partum, intra-partum and post-partum factors was done. In th

Obstetric brachial plexus lesions: a study of 74 children registered with the British Paediatric Surveillance Unit (March 1998-March 1999).

Seventy-four children with obstetric brachial plexus palsy registered with the British Paediatric Surveillance Unit were prospectively followed for a minimum of 2 years. Thirty-nine (52.7%) spontaneously recovered to normal or nearly normal levels and a further 29 (39.3%) regained good function in the upper limb. The most important secondary deformity involved the gleno-humeral joint and 20 patients (27%) needed surgical correction. Two more children await operation for shoulder deformity. The brachial plexus was explored in nine patients (12.2%) and repaired in seven.

Obstetric brachial plexus palsy: a large animal model for traction injury and its repair. Part 1: age of the recipient.

A Sunderland type IV traction injury to the C6 root of the sheep or lamb brachial plexus was used as a model for obstetric traction injury to the C5 root in humans. The injury was created and immediately repaired using interfascicular nerve autografts in a group of adult sheep and a group of newborn lambs. The animals were examined using electrophysiological and morphometric techniques 1 year after operation. It was found that the recovery of neuromuscular function was superior in the lambs. The implication is that nerves in newborn animals have a better potential for regeneration than that se

Perinatal brachial plexus palsy.

Perinatal brachial plexus palsy (PBPP) has been traditionally classified into three types: upper plexus palsy (Erb's) affecting the C5, C6, and +/- C7 nerve roots, lower plexus palsy (Klumpke's) affecting the C8 and T1 nerve roots, and total plexus palsy. Although most cases will resolve spontaneously, the natural history of the remaining cases is influenced by contractures of uninvolved muscle groups and subluxation or dislocation of the shoulder and elbow. Microsurgical nerve repair has demonstrated to provide improved outcomes compared to conservative treatment, while advancements in second

Improving shoulder and elbow function in children with Erb's palsy.

Many children who sustain birth injuries to the brachial plexus suffer significant functional limitations due to various sequelae affecting the shoulder and elbow or forearm. The maintenance of full passive mobility during the period of neurological recovery is essential for normal joint development. Early surgical correction of shoulder contractures and subluxations reduces permanent deformity. Reconstruction of forearm rotation contractures significantly improves the appearance and use of the extremity for many basic activities. Each child must be carefully evaluated, therapy maximized, and

A case of neonatal spinal cord injury: magnetic resonance imaging and somatosensory evoked potentials.

This is the first case report on the diagnosis of spinal cord injury due to hemorrhage during the neonatal period using magnetic resonance imaging (MRI). Somatosensory evoked potentials are also helpful in the functional demonstration of this lesion. When discrepant signs, alert consciousness and intact cranial nerves are observed in newborn babies with flaccid extremities and respiratory disturbance, the immediate carrying out of MRI is mandatory in order to differentiate spinal cord injury from hypoxic-ischemic encephalopathy, cerebral hemorrhage, and neuromuscular disease.
