Increase in Self-Injury as a Method of Self-Harm in Ghent, Belgium: 1987-2013.

BACKGROUND: Self-harm is a major health care problem and changes in its prevalence and characteristics can have important implications for suicide prevention. The objective was to describe trends in the epidemiology of self-harm based on emergency department (A&E departments) visits over a 26-year period in Ghent, Belgium. METHODS: We analyzed data on all self-harm presentations from the three large general hospitals in Ghent between 1987 and 2013. We investigated trends in prevalence (events by year per 100.000), methods and alcohol use. RESULTS: Rates of self-harm steadily decreased during

Extended-release naltrexone and harm reduction counseling for chronically homeless people with alcohol dependence.

BACKGROUND: Abstinence-based alcohol interventions are minimally desirable to and effective for chronically homeless individuals with alcohol dependence who have multimorbidity and high publicly funded service utilization and associated costs. Lower-barrier, patient-centered combined pharmacobehavioral interventions may more effectively treat this population. Harm reduction counseling involves a nonjudgmental, empathic style and patient-driven goal setting that requires neither abstinence nor use reduction. Extended-release naltrexone (XR-NTX), a monthly injectable formulation of an opioid rec

Overall survival as a safety guardrail in cancer drug trials.

Surrogate endpoints have accelerated access to oncology drugs but often leave uncertainty about net patient benefit. We propose a pragmatic framework that recentres overall survival (OS) as both the definitive patient-centred efficacy endpoint and a safety guardrail capable of detecting net harm that intermediate measures may miss. The framework comprises four pillars. First, designing for harm exclusion: protocols should prespecify a clinically meaningful OS harm margin, power event-driven follow-up to exclude or detect that margin with precision, and mandate independent data monitoring commi

Evaluation of a Depression Screening Program in Pediatric Orthopaedic Sports Clinics: Identifying At-Risk Adolescents.

BACKGROUND: Mental health is a contributing factor to the overall well-being of patients, and the 2-question Patient Health Questionnaire (PHQ-2) and 9-question Patient Health Questionnaire (PHQ-9) are reliable in-clinic tools to assess depression and self-harm. The prevalence of adolescents with depression symptoms within a pediatric orthopaedic sports clinic has not been assessed. HYPOTHESIS: That rates of depression and risk of self-harm would vary based on presenting pathology among adolescents sustaining a sports-related injury. STUDY DESIGN: Cross-sectional study; Level of evidence, 3.

Patient-initiated violence against dental staff: a survey in faculty clinic settings.

BACKGROUND: Workplace violence against healthcare personnel is an increasing concern. However, there is limited research on this issue within the dental field. AIM: This study aimed to examine the prevalence and characteristics of patient-initiated violence against dental staff. METHODS: A cross-sectional survey design was utilized, involving a convenience sample of dental clinic staff at Hadassah Medical Center, Jerusalem, Israel. Participants filled out an anonymous questionnaire that assessed patient aggression in three categories: physical violence, verbal abuse, and reputational harm. A

Ipsilateral blooming of microbleeds after Hyperintense Acute Reperfusion Marker sign in an ischemic Stroke patient, a case report.

BACKGROUND: Hyperintense Acute Reperfusion Marker (HARM) is a hyperintense subarachnoid signal on FLAIR MRI sequence caused by gadolinium contrast leakage into the subpial space. While, on FLAIR, HARM may mimic subarachnoid hemorrhage, it is differentiated from it on computed tomography (CT) and SWAN MRI sequences. Cerebral microbleeds are black, rounded spots on SWAN caused by blood-products deposition following red blood cell leakage from small cerebral vessels brain. Both microbleeds and HARM carry important prognostic implication as they are associated with blood-brain barrier disruption a

Dementia and patient safety in the community: a qualitative study of family carers' protective practices and implications for services.

BACKGROUND: Dementia is a cause of disability and dependency associated with high demands for health services and expected to have a significant impact on resources. Care policies worldwide increasingly rely on family caregivers to contribute to service delivery for older people, and the general direction of health care policy internationally is to provide care in the community, meaning most people will receive services there. Patient safety in primary care is therefore important for future care, but not yet investigated sufficiently when services are carried out in patients' homes. In particu

Severe and fatal medication errors in hospitals: findings from the Norwegian Incident Reporting System.

BACKGROUND: Even with global efforts to prevent medication errors, they still occur and cause patient harm. Little systematic research has been done in Norway to address this issue. OBJECTIVES: To describe the frequency, stage and types of medication errors in Norwegian hospitals, with emphasis on the most severe and fatal medication errors. METHODS: Medication errors reported in 2016 and 2017 (n=3557) were obtained from the Norwegian Incident Reporting System, based on reports from 64 hospitals in 2016 and 55 in 2017. Reports contained categorical data (eg, patient age, incident date) and f…

Exploring Australian pharmacists' views toward reducing the risk of medicines-related harm in aged care residents.

Medicines-related harm is common in older people living in residential aged care facilities (RACFs). Pharmacists offering services in the aged care sector may play a key role in reducing medicines-related injury. This study aimed to explore Australian pharmacists' views toward reducing the risk of medicines-related harm in older residents. Qualitative, semi-structured interviews were conducted with 15 Pharmacists across Australia providing services (e.g., through the provision of medication reviews, supplying medications, or being an embedded pharmacist) to RACFs identified via convenience sam

Evaluating the Potential Severity of Look-Alike, Sound-Alike Drug Substitution Errors in Children.

OBJECTIVE: Look-alike, sound-alike (LASA) drug name substitution errors in children may pose potentially severe consequences. Our objective was to determine the degree of potential harm pediatricians ascribe to specific ambulatory LASA drug substitution errors. METHODS: We developed a unified list of LASA pairs from published sources, removing selected drugs on the basis of preparation type (eg, injectable drugs). Using a modified Delphi method over 3 rounds, 38 practicing pediatricians estimated degree of potential harm that might occur should a patient receive the delivered drug in error an

An Examination of Medical Malpractice Claims Involving Physician Trainees.

PURPOSE: To identify patient-, provider-, and claim-related factors of medical malpractice claims in which physician trainees were directly involved in the harm events. METHOD: The authors performed a case-control study using medical malpractice claims closed between 2012-2016 and contributed to the Comparative Benchmarking System database by teaching hospitals. Using the service extender flag, they classified claims as cases if physician trainees were directly involved in the harm events. They classified claims as controls if they were from the same facilities, but trainees were not directly

Effectiveness without safety: a response to Duggan, Hardy, and Waller (2026).

This commentary responds to Duggan, Hardy, and Waller's systematic review and meta-analysis of cognitive-behavioural therapy (CBT) for outpatients with anorexia nervosa (AN). Although the review synthesises evidence of clinical benefit, it does not define, extract, or discuss iatrogenic harm or reasons for dropout. This omission limits what can be inferred from the review's conclusions. Effectiveness and safety are distinct evidentiary domains: improvement in weight or eating disorder (ED) symptoms does not demonstrate absence of iatrogenic harm, especially when clinical trials have not system

Identifying hazards for psychological harm in cancer care using incident reports: searching for the invisible.

BACKGROUND: Keeping patients safe means not only preventing physical harm events, but also psychological harm. However, little is known about psychological harm in health care. The aim of this study was to investigate whether and what types of hazards for preventable psychological harm (PPH) were present in incidents reported by cancer care healthcare workers. METHODS: Incidents reported by staff working in the wards and ambulatory infusion centres providing cancer care in three Swiss hospitals during the years 2022 and 2023 were qualitatively analysed in order to identify hazards for psychol

Attitudes of HIV healthcare Provider Attitudes Towards PWH who use Drugs and acceptance of harm reduction strategies: a cross-sectional study.

BACKGROUND: Negative attitudes toward people who use drugs (PWUD) can hinder their engagement in healthcare and contribute to poor clinical outcomes. While harm reduction-informed care may improve healthcare worker attitudes and patient experiences, limited research has examined its acceptability within HIV clinical settings. OBJECTIVES: This study aimed to assess healthcare workers' attitudes toward PWUD and their acceptance of harm reduction principles within HIV care clinics and to examine associated sociodemographic and work-related factors. DESIGN: Cross-sectional quantitative study. D

Reducing the risks when using benzodiazepines to treat insomnia: A public health approach.

Benzodiazepines are widely used but can cause considerable harm, including sedation, addiction, falls, fractures, and cognitive impairment, especially with long-term use and in elderly patients. The authors propose a public health approach to reduce the potential for harm when using benzodiazepines to treat insomnia. Primary prevention involves judicious patient selection and patient education. Secondary prevention requires keeping the duration of use as short as possible according to guidelines. Tertiary prevention, for patients who have been taking a benzodiazepine for a long time, uses shar

"It's nothing personal": understanding barriers to relational harm reduction practices during inpatient hospitalization.

BACKGROUND: People who use drugs (PWUD) have a higher incidence of infectious complications that require medical interventions only available in hospital settings. Hospitalizations for PWUD are also more likely to result in patient directed discharge (PDD) before completing medical treatment. Stigma directed at PWUD pressurizes and shapes clinical encounters, leading to poor communication between patient and provider. We explore how the relational philosophy of harm reduction - the premise that building relationships rooted in compassion and respect, as opposed to a transactional encounter - c
