Rapid dissolution of right atrial thrombus in a patient with congestive cardiomyopathy.

The AA describe a patient with congestive cardiomyopathy, probably alcoholic, who exhibited signs and symptoms of congestive heart failure and a pericardial rub, due to a right atrial thrombus (4 X 2 cm). The thrombus dissolved completely and the symptoms decreased markedly after 14 days of oral anticoagulant therapy alone. We suggest that anticoagulant therapy should be considered before surgery, if an intracardiac mass with no signs of pulmonary embolism is found in a patient with cardiomyopathy.

Exceptions in Pediatric Heart Transplantation: Are We Prioritizing the Right Patients?

BACKGROUND: In 2018, UNOS changed policies, altering organ distribution patterns. We compare survival in 1A patients with and without exceptions after these policy changes. METHODS: Using UNOS, we included Status 1A patients (< 18 years of age) listed for a heart transplant from 2018 to 2025 and categorized them by exception status. Primary outcomes were survival from listing and post-transplant survival. Multivariable Cox regression assessed the independent association between exception status and survival, with a sub-analysis restricted to non-VAD patients. RESULTS: Of the 3993 patients, 8

Strangulated right obturator hernia in an elderly patient: A case report.

INTRODUCTION: An obturator hernia is a rare condition representing less than 1 % of abdominal hernias and responsible for 0.05 to 1.4 % of cases of mechanical obstruction of the small intestine [1] typically affecting elderly, emaciated, multiparous women. Strangulation is a frequent complication and is generally the main clinical presentation. The positive diagnosis is often difficult because of the low specificity, hence the importance of sectional imaging. Surgical management must be initiated urgently to reduce the rate of morbidity and mortality. PRESENTATION OF CASE: A 79-year-old woman

Right-Sided Aortic Torsion in Patients with Abdominal Aortic Aneurysms.

BACKGROUND: Altered flow dynamics within abdominal aortic aneurysms (AAA) may lead to changes in aneurysmal geometry, intraluminal thrombus (ILT) deposition, or aneurysmal progression. Aortic torsion is one geometric deviation that has been clinically observed but has not been formally evaluated in preoperative AAAs. This pilot retrospective cohort study investigates the degree and directionality of aortic torsion in patients with and without AAAs. METHODS: The inferior mesenteric artery (IMA angle) outlet angle was used to assess aortic torsion. Angles were measured with respect to the anter

Acute Thrombocytopaenia Induced by Teicoplanin in a Patient With Right Leg Cellulitis.

Platelets are one of the important cells in the human blood that regulate the coagulation system. Abnormalities in platelets may lead to thrombosis or bleeding. Acute thrombocytopaenia carries a wide differential in a hospitalised patient, and drug-induced thrombocytopaenia is one of the uncommon findings. This case report describes teicoplanin-induced thrombocytopaenia in a patient who was being treated with this antibiotic and then developed thrombocytopaenia to a critically low level. This low level of platelets predisposes to bleeding, either minor or major. However, prompt recognition and

Echocardiographic Assessment of Right Ventricular Function in Patients With Liver Cirrhosis.

BACKGROUND: In patients with chronic liver disease, the common endpoint of its course is liver cirrhosis which is a cause of cardiovascular morbidity and mortality. These abnormalities in the cardiovascular system, especially the heart, can be detected by echocardiography. Identifying and acting on these abnormalities can have an impact on their management thereby reducing morbidity and mortality of patients with liver cirrhosis. The aim of this study was to determine the prevalence of right ventricular systolic and diastolic dysfunction in liver cirrhosis patients. METHODS AND MATERIALS: A h

Management of a Patient With an Anomalous Right Coronary Artery: A Case Report.

Coronary artery anomalies are rare congenital defects that involve abnormalities in the origin, course, or termination of the three main epicardial coronary arteries. Due to the variety of aberrant coronary artery defects, the clinical presentation can differ. Anomalous origins of the right coronary artery include the pulmonary trunk, ascending aorta, left sinus of Valsalva, and a course that traverses between the great vessels. Diagnosis is made using multidetector computed tomography coronary angiography or coronary computed tomography angiography. Management may include lifestyle modificati

Early Right Heart Remodeling in Patients With Mitral Valve Prolapse.

PURPOSE: Mitral valve prolapse (MVP) has been associated with left heart remodeling. This study explored cardiac remodeling in patients with MVP without significant regurgitation, focusing on the right heart. METHODS: This single-center study enrolled consecutive patients referred to trans-thoracic-echocardiography (TTE) with MVP, excluding those with significant regurgitation or known cardiovascular or pulmonary diseases. A group of healthy controls was included. RESULTS: Forty-nine patients with MVP and 54 controls were finally selected (mean age of 62, 52-71; 52% males), and echocardiogra
