This study gives results of magnetic resonance imaging (MRI) in the evaluation of male (23 cases) and female (29 cases) pelvis. Thirty nine patients with abnormal pelvis were compared with CT and ultrasounds. MRI and CT have proven equally sensitive to the presence of disease, with a better visualization of calcified tumours or benign lesions with CT, and a superior display of soft tissue spreads and bone metastases with MRI. We studied a unique example of Wilms' tumor consisting entirely of a polypoid intrapelvic renal mass. Despite a gross appearance akin to sarcoma botryoides, this and related examples of Wilms' tumor with intrapelvic growth should be treated no differently from ordinary Wilms' tumors of similar stage and grade.

Mucinous and squamous cell metaplasia of the transitional cell epithelium occurs in response to chronic irritation and is predominantly found in the bladder. When the 2 forms of metaplasia occur simultaneously they usually are associated with malignancy. We report 2 cases of mucinous and squamous metaplasia of the epithelium of the renal pelvis that were not associated with malignancy. This is believed to be the fourth report of this divergent metaplasia. The mechanism for these changes is discussed.

The occurrence, relation to a particular type of fracture of the pelvis and the possible mechanism of early urinary complications were studied in 186 consecutive patients with fracture of the pelvis. Eighty-eight patients (47.3%) showed early urinary complications. These complications were minor in 73 patients (83.0%; 32.9% of all pelvic features) and major in 15 (17.0%; 8.1% of all pelvic fractures). The [Ca++] gradient within the rabbit renal pelvis was compared to its inherent pacemaker frequency. The pelvis was removed from the rabbit, sectioned, and [Ca++] ions assayed using wet ash techniques and atomic absorption spectrophotometry. Contraction frequency was measured isometrically in vitro. In the bisected pelvis it was found that both [Cu++] levels and contraction frequency were highest in the proximal section, 9.02 +/- 4.62 mEq/kg, and lower in the distal section, 6.59 +/- 2.43 mEq/kg wet weight. The excretory urograms of 14 children with ureteral notching were reviewed. In 10 of 14 cases the vascular impressions on the ureters were due to either venous obstruction and dilated collaterals or to dilated noncollateral veins. In two cases the notching was associated with arterial collaterals in renal ischemia and in two with dilated lymphatic channels. The implications of notching in children and the differences and similarities of this urographic sign between children and adults are discussed.

Examination of the renal pelvis of late fetal and neonatal kidneys shows that in about one quarter of the calyces examined the arrangement is similat to that in the rat, the outer medulla being separated from the calyceal cavity by a very thin epithelium. It is suggested that this presents a vulnerable area through which intrarenal damage can occur as a result of vesico-ureteric reflux in the immature kidney.

Angiographic findings in 21 cases of renal pelvic carcinoma are presented. Important characteristics include neovascularity, visible capillary phase, vascular encasement, wide pelviureteric artery and absence of arteriovenous shunting. Angiography may also be of value for the differential diagnosis of ureteric obstruction as demonstrated in 5 of 6 patients with carcinoma of the ureter.
