Use of triple antibiotic paste as a disinfectant for a traumatized immature tooth with a periapical lesion: a case report.

Elimination of microbial contamination from the root canal system is a precondition for successful root canal treatment. In this regard, mechanical instrumentation, irrigation, and intracanal medication are all important. This case report assesses the efficacy of a triple antibiotic paste consisting of metronidazole, ciprofloxacin, and minocycline in the disinfection of immature teeth with apical periodontitis. An 8-year-old girl presented at the Department of Pediatric Dentistry for the evaluation of right and left maxillary central incisors with crown fractures and luxation. The right centra

Increase Toll-like receptors 2 and 4 in apical periodontitis of rats with chronic liver disease.

The aim of this study was to evaluate the influence of liver fibrosis (LF) on the expression of Toll-like receptors (TLR) 2 and 4 in apical periodontitis (AP) in Wistar rats. Forty Wistar rats were allocated in the following groups (n = 10): C-control; AP-apical periodontitis; LF-liver fibrosis; AP + LF-rats with AP and LF. LF and AP were induced by established methodologies. Histological, bacteriological, and immunohistochemical analyses were performed according to pre-established scores. For comparisons between AP and AP + LF groups, the Mann-Whitney test was used (P < .05). The livers of th

Surgical Management of Radicular Cyst With the Application of a Natural Platelet Concentrate: A Case Report.

Traumatic dental injuries usually involve the dentoalveolar region, and they readily affect the teeth and their surrounding soft and hard tissues. The common sequelae of traumatic dental injuries lead to pulpal necrosis and apical periodontitis along with cystic changes. The current case report describes the surgical management of a radicular cyst in the periapical region of maxillary incisors and highlights the efficacy of natural platelet concentrate [platelet-rich fibrin (PRF)] used for postoperative healing. A 38-year-old male patient presented to the department with pain and mild swelling

Successful Nonsurgical Resolution of a Large Periapical Lesion With Pre-Existing Sealer Extrusion and Adjacent External Inflammatory Root Resorption: A Case Report.

The primary aim of root canal treatment is to eliminate intracanal infection, achieve an apical and coronal seal, and create conditions that allow repair of the periapical tissues. In root-filled teeth, persistent apical periodontitis may be associated with extensive periapical lesions, external inflammatory root resorption affecting the treated or adjacent teeth, and extrusion of endodontic materials. These complications may lead clinicians to consider surgical approaches. However, when canals are adequately cleaned, disinfected, and hermetically obturated, favorable healing may occur followi

An update of knowledge on the regulatory role of Treg cells in apical periodontitis.

Apical periodontitis (AP) is a prevalent infectious and inflammatory disorder that involves inflammation of periapical tissues and the disintegration of alveolar bone. AP may eventually lead to tooth loss if not timely treated. This disease is caused by pathogenic bacteria in the necrotic pulps and root canals, thereby triggering responses from the innate and adaptive immune system of the periapical tissues. Regulatory T (Treg) cells play a major role in maintaining immune homoeostasis and immunological self-tolerance; however, these only account for roughly 5%-10% of human peripheral CD4+ T c

Study of severe early childhood caries in children under general anesthesia from a single hospital.

Young children are a high-risk group for dental caries. Understanding the prevalence, characteristics of severe early childhood caries (S-ECC), and its associated complications (e.g., pulpitis and periapical periodontitis) is crucial for guiding the prevention, diagnosis, and treatment of these conditions. A retrospective analysis was conducted on the dental records of children with S-ECC who received treatment at Jinan Stomatological Hospital from 2018 to 2023. Cases were excluded if they did not meet the S-ECC diagnostic criteria, had intellectual impairment, had other severe systemic diseas…

Periapical status of non-root-filled teeth with resin composite, amalgam, or full crown restorations: a cross-sectional study of a Swedish adult population.

INTRODUCTION: Experimental studies show that dental pulp cells respond unfavorably to contact with resin composite restorative material. Hypothetically, in a random population, the frequency of apical periodontitis should be higher for teeth restored with resin composite than with amalgam. Therefore, the aim was to compare the periapical status of non-root-filled teeth restored with resin composite, amalgam, or laboratory-fabricated crowns in an adult Swedish population. METHODS: The subjects comprised 440 individuals from a randomly selected sample of 1,000 adult residents of a Swedish count

Optimizing Esthetics in Maxillary Anterior Periradicular Surgery: A Case Report on Vestibular Incision.

BACKGROUND: Persistent periapical infections may necessitate surgical intervention when nonsurgical endodontic treatment fails. Conventional surgical approaches in the maxillary anterior region often lead to visible scarring and prolonged healing, compromising esthetic outcomes. The vestibular incision subperiosteal tunnel access (VISTA) technique offers a minimally invasive alternative that preserves soft tissue integrity and reduces scarring. CASE REPORT: A 32-year-old male presented with symptomatic apical periodontitis of the maxillary left central incisor; universal Tooth #9 following fa

Persistent endodontic lesion due to complex cementodentinal tears in a maxillary central incisor--a case report.

The cementodentinal tear is rarely detected by noninvasive procedures owing to its clinical picture simulating a root fracture or a periodontal or endodontic lesion. We present a case of complex cementodentinal tears in a 79-year-old woman who presented a repeated swelling at the labial mucosa of the left maxillary central incisor for 6 months. Periapical radiographs demonstrated a vertical radiolucent fracture line extending from the root apex along the mesial aspect of the root to near the middle portion of the root of the left maxillary central incisor. Because endodontic re-treatment faile

Late term complications of intruded permanent incisors: 2 case reports.

This report describes 2 cases of intrusive luxation injuries of maxillary immature permanent central incisors. In both cases, intruded teeth were left for spontaneous re-eruption. During periodic follow-ups for up to 2 years the teeth did not show any sign, clinically or radiographically, of pulp necrosis with apical periodontitis. However, in the first case tenderness to percussion and periapical radiolucency developed after 2.5 years and endodontic treatment was started. In the second case, the patient returned after 2 years 9 months with complaint and periapical radiolucency was observed ra

The Essential Role of 17-Octadecynoic Acid in the Pathogenesis of Periapical Abscesses.

INTRODUCTION: Periapical abscesses are 1 of the most frequent pathologic lesions in the alveolar bone. Recently, we have identified 17-octadecynoic acid (17-ODYA) as the highest unique metabolite in periapical abscesses. Therefore, the aim of this study was to investigate the immunologic and pathophysiological roles of this metabolite in the initiation and development of periapical abscesses. METHODS: Periodontal ligament fibroblasts and peripheral blood mononuclear cells were treated with 17-ODYA. Gene expression analysis and interleukin (IL)-8 release were determined using quantitative real

The Role of the Immune Response in Chronic Marginal and Apical Periodontitis.

The immune response is a complex, dynamic and strongly individual biologic network that plays an essential role in the pathogenesis of chronic apical and marginal periodontitis. Recent research in the field of periodontology has indicated that the major determinant of susceptibility to disease is the nature of the immunoinflammatory response as marginal periodontal tissue damage is thought to be primarily mediated by the host reaction. Whether the same rules apply for the development of apical periodontitis, however, remains largely unexplored. This review aims to draw parallels between the pa

Histopathological observations of periapical repair in teeth with radiolucent areas submitted to two different methods of root canal treatment.

Dogs' teeth with induced chronic periapical periodontitis were treated endodontically by two different methods, and the results were compared. A total of 40 root canals from the upper and lower premolars of two dogs were prepared chemomechanically. In method 1, a high-concentration (5.25%) hypochlorite solution was used during the instrumentation of the root canal, and an antibacterial dressing was applied between sessions, followed by filling of the root canal. In method 2, a low-concentration (0.5%) sodium hypochlorite solution was used as an adjunct to mechanical debridement, and the root c…

Using Young Tongue-Holding Forceps for Third Molar Autotransplantation as a New Method for Tooth Handling: A Case Report with a 1-Year Follow-Up :

For the first time in autotransplantation literature, all extraoral procedures on the donor tooth were carried out while holding it by a Young Tongue Holding Forceps, potentially reducing damage to the PDL and enhancing the handling of the tooth. In this case report, we present the autotransplantation of a mandibular third molar into the socket of the adjacent second molar in an 18-year-old patient. The clinical and radiographic evaluation revealed severe pain and a periapical lesion from the previous root canal treatment. The surgical and endodontic steps were carried out with a focus on main

[Transient apical breakdown after dental trauma: clinical features and therapeutic decisions].

Transient apical breakdown (TAB) may occur after dental trauma, which is manifested by obvious radiolucent shadows at the root apex of the affected tooth on X-ray examination within a certain period after trauma, or persistent widening of the periodontal ligament space for a long time after trauma. However, it can resolve spontaneously without intervention. At present, researches on TAB are rather limited, and clinicians have insufficient understanding of it, which may easily lead to misdiagnosis as apical periodontitis and subsequently unnecessary endodontic treatment. In view of this, this a
