Periodic breathing at high altitude.

Periodic breathing is often associated with heart disease or stroke, and commonly Cheyne-Stokes breathing has a period of about a minute. Periodic breathing also commonly occurs in healthy subjects at high altitude, and here the periods may be much shorter, of the order of 15-20 s. In this paper we study such periodic breathing using the classical model of Grodins et al. (1967, J. Appl. Physiol. 22, 260-276), together with a prescription for the dependence of ventilation on the blood CO2 concentration, modulated by the reduced oxygen pressure (the 'Oxford fan'). The model focusses on the fast

Recognizing periodic alternating nystagmus.

Congenital or acquired periodic alternating nystagmus (PAN) is characterized by nystagmus occurring in a cycle. The cycle consists of a left-beating nystagmus, a transition phase, a right-beating nystagmus, and a further transition phase. The purpose of this review is to assist the clinician in the recognition of periodic alternating nystagmus (PAN), either as a type of congenital nystagmus or in its acquired form, and to highlight why such identification is important. Recent studies using eye movement recordings are reviewed to point out the frequency of congenital PAN in samples of patients

[Periodic limb movement disorder].

The periodic limb movements (PLM) are defined as stereotyped, periodic movements of the legs and/or upper limbs during sleep. The patient exhibits dorsifilexion of the ankle and extension of the big toe with occasional flexion of the knee and hip. PLM originally was described as "nocturnal myoclonus" by Symonds in 1953. Recently, the term "nocturnal myoclonus" has been replaced with PLM, because the movements are slower than true myoclonic movement. The appearance of PLM was reported in sleep apnea syndrome, delayed sleep phase syndrome, narcolepsy, spinal cord tumor, diabetes mellitus and ure

[Parenteral nutrition in postoperative period].

Results of surgical procedures in malnutrished patients are worse than in well-nutrished patients. Perioperative nutritional treatment lead to diminished percentage of postoperative complications and mortality. Authors presents the influence of early postoperative enteral nutrition in 35 malnutrished patients on body composition and anthropometrical and biochemical parameters of their nutritional status. Results shows that enteral nutrition in postoperative phase can decrease catabolic effect of surgical treatment.

Fibromyalgia in hyperkalemic periodic paralysis.

A 43-year-old woman presented at the age of 38 with joint pains and muscle stiffness. Tender points were found fulfilling ACR criteria (1) for fibromyalgia. She had well developed muscles and decreasing muscle power since the age of 35. Muscle pains increased after exercise. Her 10-year-old son had similar symptoms and one paralytic attack. Muscle pain and fatigue increasing with age were found by history in three close relatives. Forearm cold water test produced myotonia in both mother and son. Electromyography was normal and muscle biopsy showed minor unspecific changes. Biochemical investig

Thyroid hypokalemic periodic paralysis.

An unusual presentation of thyrotoxicosis resulting in hypokalemic periodic paralysis is reported here. The patients with this disorder have very low serum potassium levels during the active phase of the disease. Very often, several months pass before the diagnosis is made. Once recognized and the thyrotoxicosis is treated, the patient does well.

Chronic medications in the perioperative period.

BACKGROUND: While most chronic medications can be safely continued in the perioperative period, decisions regarding certain medications remain controversial. METHODS: We mailed a questionnaire to all 150 anesthesiology program directors in the United States, with 113 (75%) responding. Opinions regarding preoperative discontinuance or modification of selected medications were assessed. RESULTS: Antiplatelet agents are typically discontinued preoperatively, although the number of days varies widely. Few respondents discontinue tricyclic antidepressants, oral contraceptives, diuretics, or oral

[Interventricular communication in the neonatal period].

OBJECTIVE: The aim of this study was to evaluate the prevalence and the follow-up in the long run of ventricular septal defect in the neonatal period. MATERIAL AND METHODS: 222 patients with ventricular septal defect from a total amount of 30,840 newborns admitted to our Hospital were evaluated between January 1991 and December 1996 (The incidence accounts for 7.19 per 1,000 newborns). 123 of these patients were males (55.4%) and 99 females (44.6%). The mean age at the time of examination was 3.09 +/- 1.67 days (range from 1 to 11 days). Diagnosis was suspected because of a harsh systolic mur

[Prevalence of gallstones in the neonatal period].

AIM: To assess the prevalence of gallstones in normal neonates. METHOD: We used sonography to assess the gallbladder in 3500 unselected infants between 1993 and 1995. Examination was performed during the first four days of life as part of a prospective neonatal screening study evaluating hip and renal abnormalities. RESULTS: We found sludge or gallstones in 19 children (0.5%). In 4 of 11 patients followed for up to 18 months, the gallstones persisted. In 3 of the cases, there was a family history of gallstones. None of the children had signs of cholestasis. Three were treated with ursodesoxy

[Periodic hypokalemic paralysis in thyrotoxicosis].

The case is reported of a 26-year-old Chinese cook hospitalized for hypokalemic paralysis, the cause of which was found to be thyrotoxicosis. In Asian males hypokalemic periodic paralysis occurs in 2% to 10% of thyrotoxic patients with a male:female ratio of close to 70:1. In the white population the frequency is smaller by a factor of 10. This association is however considered important, since the clinical symptoms of thyrotoxicosis may be scanty. By successful treatment of thyrotoxicosis hypokalemic paralysis disappears, but may return after cessation of treatment. Pathogenetically, hyperrea

Painful thyroiditis in postpartum period.

Destructive thyroiditis commonly occurs during the postpartum period, with a prevalence rate of 5% to 16%, and is mainly due to postpartum autoimmune thyroiditis (PPT) and, very rarely, to subacute thyroiditis. The thyroiditis is similar to Hashimoto's thyroiditis and is generally painless in nature, although cases with painful thyroiditis have been described. We report a case of painful destructive thyroiditis occurring during the postpartum period, which was clinically and biochemically indistinguishable from the variant of painful PPT or subacute thyroiditis. Fine needle aspiration cytology

Chromosomal abnormalities in the newborn period.

Chromosomal abnormalities account for a significant percentage of congenital malformations in the neonate. While some of the syndromes can be suspected on clinical grounds, the clinician will need to have a high index of suspicion based on the presence of multiple abnormalities that cannot be accounted for by other causes. Chromosome analysis should be performed promptly in these cases. Cultured lymphocytes are the standard preparation at present. However, new non-isotopic hybridization techniques are becoming available that allow analysis of interphase cells, and these may become more widely

[Pustular dermatoses in the neonatal period].

The diagnosis of pustular dermatosis occurring during the first months of life is usually based on clinical findings. However, some cases may require a few simple investigations including cytological studies, cultures, and skin biopsies. The most common causes of infectious pustular skin lesions include bacterial infections, which may be septicemic (with Listeria as the leading causative agent) or initially localized (staphylococcus); viral infections (varicella, herpes); fungal infections, i.e., candidiasis (congenital or neonatal) or the very recently described form of pustulosis due to Mala
