Delayed gastric emptying in a variable proportion of patients with gastro-oesophageal reflux disease has been observed in most series, however a relationship between delayed gastric emptying and increased gastro-oesophageal reflux has not been convincingly demonstrated. Enhanced postprandial accommodation and delayed emptying of the proximal stomach have been described, but some controversy exists. Impaired primary peristalsis is often present especially in patients with oesophagitis and its prevalence increases with increasing severity of inflammatory mucosal lesions. Patients with gastro-oes…

Intraluminal manometric studies of normal swallowing activity fail to distinguish between the peristaltic response of striated and smooth muscle. Studies were performed to determine if the two types of musculature differ in their response to pharmacological stimulation. After the administration of d-tubocurarine, peristaltic amplitude in the striated muscle segment decreased by 26 to 51%, while amplitude in the smooth muscle portion was not significantly affected. The administration of atropine, on the other hand, abolished or diminished smooth muscle peristalsis without altering striated musc…

Gut peristaltic movements recognized as the wave-like propagation of a local contraction are crucial for effective transportation and digestion/absorption of ingested materials. Although the physiology of gut peristalsis has been well studied in adults, it remains largely unexplored how the cellular functions underlying these coordinated tissue movements are established along the rostral-caudal gut axis during development. The chicken embryonic gut serves as an excellent experimental model for elucidating the endogenous potential and regulation of these cells since peristalsis occurs even thou…

Movements of the ileum and sacculus rotundus occurred at the same frequency and were broadly coordinated. Conventional images and iodine maps were reconstructed to calculate the contrast-to-noise ratio (CNR). Additionally, …

BACKGROUND: In-vitro-fertilization (IVF) failure rates remain above 65% with unknown causes. Uterine receptivity, largely determined by uterine peristalsis, is believed to play a key role in the IVF success. Accurate assessment of uterine peristalsis holds the potential for improving the success rate of embryo implantation.
METHODS: This prospective study includes 62 IVF patients from multiple fertility centers under three different clinical settings. Four-minute B-mode transvaginal ultrasound (TVUS) scans were performed one hour before embryo transfer (ET). 25 features related to frequency, …

BACKGROUND: Functional dyspepsia (FD) is a disorder of gut-brain interaction, and its putative pathophysiology involves dysregulation of gastric motility and central processing of gastric afference. The vagus nerve modulates gastric peristalsis and carries afferent sensory information to brainstem nuclei, specifically the nucleus tractus solitarii (NTS). Here, we combine MRI assessment of gastric kinematics with measures of NTS functional connectivity to the brain in patients with FD and healthy controls (HC), in order to elucidate how gut-brain axis communication is associated with FD pathoph…

BACKGROUND: The lower esophageal sphincter (LES) overlaps the crural diaphragm (CD) in patients without hiatal hernia (HH). Swallowing induces esophageal peristalsis with longitudinal esophageal shortening, causing transient elevation of the LES above the CD. This phenomenon, visible on high-resolution manometry (HRM), is called swallow-induced transient HH (tHH).
METHODS: We assessed pathological implications of swallow-induced LES elevation. We included patients who underwent 24-h pH monitoring and HRM between January 1, 2017 and June 30, 2018. Patients with manometric HH were excluded. Pat…

Pediatric intussusception is frequently observed in the ileocecal region, where the terminal ileum invaginates into the colon. Previous studies have indicated an association between pediatric intussusception and inflammation as well as intestinal motility. However, the underlying mechanisms remain unclear, particularly with regard to the mechanics. We hypothesized that invagination occurs when longitudinal and circular smooth muscles are not coordinated during peristalsis. To test the hypothesis from a mechanical perspective, we developed a computational model of the terminal ileum, where the …

Intestinal epithelial cells are constantly exposed to pathogens and mechanical forces. However, the impact of mechanical forces on infections leading to diarrheal diseases remains largely unknown. Here, we addressed whether flow and peristalsis impact the infectivity of the human pathogen Shigella within a 3D colonic epithelium using Intestine-Chip technology. Strikingly, infection is significantly increased and minimal bacterial loads are sufficient to invade enterocytes from the apical side and trigger loss of barrier integrity, thereby shifting the paradigm about early stage Shigella invasi…

The dynamics of pyeloureteral flow is described when there is no peristalsis and for peristalsis of high and intermediate frequencies, on the assumption that the ureter is uniform except in the mid-ureter and at the outlet. The possibility of upstream transmission of bladder pressure variations to the renal pelvis is considered. The overall behaviour depends on three principal variables, the maximum tube pressure in the contraction waves, the intrinsic peristaltic carrying capacity and the peristaltic frequency f, expressed in the form fT where T is the time for a peristaltic contraction wave …

Segmentation is well known to digest the food rich in proteins, starch, and lipids; however, the mechanism leading to the digestion remains unclear. In this study, a theoretical model for segmental contractions of the small intestine is developed using lubrication method to explore the mechanisms involved. Here, the nonlinear partial differential equations governing the fluid flow were normalized in viscous regime and solved semi-analytically for a power law fluid under long wavelength approximation on a MatlabTM platform. Study indicates that shearing is highest at the 1st and 4th mid-occlusi

The main aim of the current study is to analyze couple stresses effects on MHD peristaltic transport of a micropolar non-Newtonian nanofluid. The fluid flows through a porous media between two horizontal co-axial tubes. The effects of radiation, chemical reaction, viscous and ohmic dissipation are considered. The inner tube is solid and uniform, while the outer tube has a sinusoidal wave traveling down its wall. The governing equations have been simplified using low-Reynolds number and long wave-length approximations, thus a semi-analytical solutions have been obtained using the homotopy pertu

It induced strong peristalsis in the ureter, manifested clinically as relief of pain and fluoroscopically as emptying of the ureter and the renal pelvis.
