Structured Assessment of Correctional Adaptation (SACA): a measure of the impact of incarceration on the mentally ill in a therapeutic setting.

Clinicians have observed that psychiatric patients with correctional histories evidence attitudes and behaviors that seem adaptive in penal environments but are maladaptive in mental health settings. This study sought to assess the reliability and concurrent validity of a rating scale designed to measure correctional adaptation using a sample of 64 patients from a state psychiatric hospital. Scale ratings were obtained through structured interviews, whereas predictor variables were gleaned from chart review and self-report. The scale demonstrated good interrater reliability (ICC = .83) and acc

The influence of French, Spanish, and English legal traditions on early mental health proceedings in Louisiana.

Unlike other jurisdictions in the United States where the basis of the law can be traced more exclusively to the common law of England, the legal system in Louisiana also has origins in the laws of France and Spain. In this article, the authors describe the laws and procedures that developed early in Louisiana's history for the involuntary care and civil commitment of the mentally ill, focusing on the laws that were passed after the American takeover in 1803.

Patient rights and law: tobacco smoking in psychiatric wards and the Israeli Prevention of Smoking Act.

In August 2001, the Israeli Ministry of Health issued its Limitation of Smoking in Public Places Order, categorically forbidding smoking in hospitals. This forced the mental health system to cope with the issue of smoking inside psychiatric hospitals. The main problem was smoking by compulsorily hospitalized psychiatric patients in closed wards. An attempt by a psychiatric hospital to implement the tobacco smoking restraint instruction by banning the sale of cigarettes inside the hospital led to the development of a black market and cases of patient exploitation in return for cigarettes. This

Poverty and severe psychiatric disorder in the U.S.: evidence from the Medical Expenditure Panel Survey.

BACKGROUND: Previous studies have shown that persons with severe psychiatric disorders are more likely to be poor and face disparities in education and employment outcomes. Poverty rates, the standard measure of poverty, give no information on how far below the poverty line this group falls. AIMS OF THE STUDY: This paper compares the poverty rate, poverty depth (distance from the poverty line) and poverty severity (inequality of incomes below the poverty line) of households with and without a working-age member with severe psychiatric disorder in the United States using data from the 2007 Med

When Mania Outruns the Law: Managing Risk in the Liminal Zone.

Patients with mania are often acutely unwell yet do not meet criteria for involuntary hospitalization. This creates a clinical and ethical dilemma the authors refer to as the liminal zone. These patients refuse treatment despite clear foreseeable harms, but the legal threshold for certification is not met. This situation produces moral discomfort for clinicians, disrupts team consensus, and is often equally distressing for families and friends who recognize impending consequences but lack formal authority to intervene. This article outlines practical approaches for managing liminal-zone patien

[Critical review of conceptual aspects on social phobia].

Two important nosological areas of debate currently exist concerning social phobia. The first focuses on the denomination of this disorder. In this respect, experts recommend a change in the denomination to Social Anxiety Disorder, since this better expresses the magnitude and the impact of the illness and focuses less on the behavioral aspect (avoidance) of the disorder. The second area refers to the current situation of overlap between social phobia and avoidance personality disorder. Experts on this field consider that avoidance personality disorder represents a severe form of generalised s

The indistinguishables: determining appropriate environments for justice involved individuals.

Criminalizing those with mental illness is a controversial topic with a long and complex history in the United States. The problem has traditionally been dichotomized between criminals (i.e., "bad") in need of placement in jails and prisons and the mentally ill (i.e., "mad") who are need of treatment in psychiatric facilities. Recent trends demonstrate significant increases in the rates of mental illness in jails and prisons, as well as increased rates of violence within psychiatric hospitals. This would suggest that there are a group of justice involved individuals who are "indistinguishable"

Axis I and II psychiatric disorders after traumatic brain injury: a 30-year follow-up study.

OBJECTIVE: Patients who had suffered traumatic brain injury were evaluated to determine the occurrence of psychiatric disorders during a 30-year follow-up. METHOD: Sixty patients were assessed on average 30 years after traumatic brain injury. DSM-IV axis I disorders were diagnosed on a clinical basis with the aid of the Schedules for Clinical Assessment in Neuropsychiatry (version 2.1), and axis II disorders were diagnosed with the Structured Clinical Interview for DSM-III-R Personality Disorders. Cognitive impairment was measured with a neuropsychological test battery and the Mini-Mental Sta

A quantitative analysis of the relationship between affective state and personality ratings in inpatient depression (RAPID).

BACKGROUND: The relationship between major depressive disorder (MDD) and personality disorders is complex, with implications for diagnosis and treatment. We sought to explore the relationship between these disorders quantitatively in an inpatient setting. METHODS: We conducted a structured observational study exploring symptoms of depression and selected neurocognitive functions over the span of an inpatient admission in those with depression and personality disorders. Sixty inpatients presenting with symptoms of depression completed ratings of mood and neurocognitive function. Diagnosis was

[Dysthymia and other depressive disorders: study on personality].

This work presents the results of an investigation made on a sample of depressive patients divided in two groups, one of them made up of dysthymics, another of patients diagnosed of any kind of depressive disease (in both cases following the ICD-10criteria). To all those patients it was realized the 16-PF personality questionnaire by RB Catwell. We analyse and compare one by one, the personality traits studied in the proof, specially the untuactions obtained for the patients grouping them im "means" and "extremes"; the conclusion is that with more frequency that in the rest of depressive patie

Spouse similarity for lifetime psychiatric history in the general population.

BACKGROUND: Most studies of spouse similarity for psychiatric disorders have focused on clinical samples and are thus limited by selection bias. This study is, to our knowledge, the first comprehensive investigation of spouse similarity for lifetime psychiatric history in a general population sample using standardized diagnostic criteria. METHODS: We studied 519 pairs of spouses residing in Edmonton, Canada who completed the Diagnostic Interview Schedule psychiatric interview. In each pair, one spouse belonged to a random subsample of persons who had participated in a large population survey

Civil Commitment for Opioid and Other Substance Use Disorders: Does It Work?

Many states are turning to civil commitment for substance use disorders as a potential solution to address rising rates of overdose deaths. Civil commitment allows family members or others to seek court-ordered involuntary treatment for a substance-abusing person. In contrast to mandatory treatment ordered by drug courts, civil commitment does not require involvement with the criminal justice system. Although these laws are understandably appealing, statutes and their implementation are highly variable, ethical concerns about deprivation of liberty continue to be raised, and outcome data are l

The 2023 Impact of Inflammatory Bowel Disease in Canada: Mental Health and Inflammatory Bowel Disease.

Psychiatric disorders are 1.5 to 2 times more prevalent in persons with inflammatory bowel disease (IBD) than in the general population, with pooled prevalence estimates of 21% for clinical anxiety and 15% for depression. Rates are even higher when considering mental health symptoms, as nearly one-third of persons with IBD experience elevated anxiety symptoms and one-quarter experience depression symptoms. Rates of these symptoms were much higher during periods of disease activity, more common in women than men, and more common in Crohn's disease than ulcerative colitis. There is robust eviden
