Bortezomib in mantle cell lymphoma.

Mantle cell lymphoma (MCL) represents 6% of non-Hodgkin lymphomas, but is one of the most active fields of clinical investigation. Unfortunately, there is still no standard or curative therapy in MCL. Front-line therapy appears to benefit from intensification either through high-dose therapy with stem cell transplant consolidation or dose-intense chemotherapy with hyperfractionated cyclophosphamide, vincristine, adriamycin/doxorubicin and dexamethasone/rituximab. Most patients still relapse and a multitude of novel agents are currently being tested in this setting, including proteasome inhibit

CYP2C19 pharmacogenetics versus standard of care dosing for selecting antiplatelet therapy in patients with coronary artery disease: A meta-analysis of randomized clinical trials.

OBJECTIVES: This study aimed to evaluate the efficacy and safety of personalized genotype-guided selection of antiplatelet therapy versus standard of care in patients undergoing percutaneous coronary intervention (PCI). BACKGROUND: Clopidogrel is the most frequently used P2Y12 receptor antagonist in patients with coronary artery disease. However, genetic variations of clopidogrel are associated with inter-individual response variability which could limit its efficacy. METHODS: Electronic databases were searched for all randomized clinical trials (RCTs) evaluating genotype-guided therapy vers

Multigenetic pharmacogenomics-guided treatment shows greater improvements on motor symptoms compared to usual therapy in Parkinson's disease: a small real-word prospective cohort study.

BACKGROUND: Dopamine replacement therapy is a cornerstone of Parkinson's disease treatment. In clinical practice, there is considerable variability in patients' responses, tolerability, and safety regarding anti-parkinsonian medications, which is largely influenced by genetic polymorphisms in pharmacokinetic and pharmacodynamic genes. However, the application of multigenetic pharmacogenomics-guided treatment (MPGT) to optimize therapeutic outcomes in Parkinson's disease (PD) remains under-explored. In this study, we conducted a prospective cohort investigation to evaluate the potential benefit

Integrative Multi-Omics Analysis Prioritizes Candidate Genes for Essential Tremor and Reveals a Gap Between Computational Prediction and Experimental Validation.

BACKGROUND: The genetic architecture of essential tremor (ET) remains incompletely understood. A key challenge is translating genome-wide association study (GWAS) loci into specific effector genes to elucidate disease mechanisms and develop targeted therapies. OBJECTIVE: To implement a multistage computational framework to prioritize high-confidence candidate genes for ET and to assess these predictions against publicly available, patient-derived transcriptomic data. METHODS: We employed a convergent evidence strategy to prioritize genes, integrating cross-tissue (UTMOST) and tissue-specific

Genome-Wide Pharmacogenomic Study on Methadone Maintenance Treatment Identifies SNP rs17180299 and Multiple Haplotypes on CYP2B6, SPON1, and GSG1L Associated with Plasma Concentrations of Methadone R- and S-enantiomers in Heroin-Dependent Patients.

Methadone maintenance treatment (MMT) is commonly used for controlling opioid dependence, preventing withdrawal symptoms, and improving the quality of life of heroin-dependent patients. A steady-state plasma concentration of methadone enantiomers, a measure of methadone metabolism, is an index of treatment response and efficacy of MMT. Although the methadone metabolism pathway has been partially revealed, no genome-wide pharmacogenomic study has been performed to identify genetic determinants and characterize genetic mechanisms for the plasma concentrations of methadone R- and S-enantiomers. Genomics in psychiatric Nurse Practitioner practice: Moving from emerging trend to essential competency.

Genomics is increasingly influencing psychiatric practice as evidence links genetic variation with neurodevelopmental disorders, treatment response, and complex psychiatric presentations. Psychiatric nurse practitioners are well positioned to incorporate genomics into patient assessment and management; however, familiarity with testing options and clinical indications remains limited. This column reviews key genomic considerations for psychiatric practice, including pharmacogenomics, chromosomal microarray analysis, and exome or genome sequencing. Clinical "red flags," such as developmental de

Impact of the CYP2C19*17 Allele on Outcomes in Patients Receiving Genotype-Guided Antiplatelet Therapy After Percutaneous Coronary Intervention.

Genotyping for CYP2C19 no function alleles to guide antiplatelet therapy after percutaneous coronary intervention (PCI) improves clinical outcomes. Although results for the increased function CYP2C19*17 allele are also reported, its clinical relevance in this setting remains unclear. A collaboration across nine sites examined antiplatelet therapy prescribing and clinical outcomes in 3,342 patients after implementation of CYP2C19-guided antiplatelet therapy. Risk of major atherothrombotic and bleeding events over 12 months after PCI were compared across cytochrome P450 2C19 isozyme (CYP2C19) me

5-Fluorouracil (5-FU)-induced organ toxicities: mechanisms, management, and prevention strategies.

Fluoropyrimidines are a class of chemotherapy drugs used to treat various solid tumors. 5-Fluorouracil (5-FU) an antimetabolite in the fluoropyrimidine family, which has shown remarkable efficacy against a variety of solid tumors, is a crucial medication in the treatment of cancer. However, severe organ toxicities frequently restrict its therapeutic potential. Our current knowledge of the intricate processes underlying 5-FU-induced toxicities, such as cellular absorption, metabolism, disruption of DNA synthesis, interference with RNA processing, and cell cycle regulation, is consolidated in th

Vincristine-Induced Peripheral Neuropathy in Childhood Acute Lymphoblastic Leukemia: Genetic Variation as a Potential Risk Factor.

Vincristine (VCR) is the first-line chemotherapeutic medication often co-administered with other drugs to treat childhood acute lymphoblastic leukemia. Dose-dependent neurotoxicity is the main factor restricting VCR's clinical application. VCR-induced peripheral neuropathy (VIPN) sometimes results in dose reduction or omission, leading to clinical complications or affecting the patient's quality of life. With regard to the genetic basis of drug responses, preemptive pharmacogenomic testing and simultaneous blood level monitoring could be helpful for the transformation of various findings into

Spontaneous bladder rupture mimicking a jejuno-ileal perforation.

BACKGROUND: The management of spine-related pain with narcotics, both before and after surgery, poses major challenges, including drug diversion, limited effectiveness, and worsening of pain symptoms over time. Chronic opioid use is associated with hypodopaminergia-induced hyperalgesia, whereby dopamine depletion increases pain sensitivity. Patients with inherently low dopaminergic function are particularly predisposed to hyperalgesia and reduced pain tolerance. METHODS: An alternative therapeutic strategy centers on genetically guided pro-dopamine regulation, which aims to transmodulate dopa

Challenges in drug development for functional gastrointestinal disorders. Part II: visceral pain.

There is a need to have predictive biomarkers to test novel experimental medicines in functional gastrointestinal disorders. The human pharmacodynamic models and biomarkers pertaining to two important conditions are reviewed in a two-part article: functional dyspepsia (part I) and visceral pain (part II). With visceral pain models, the large coefficient of variation in sensation end points in human studies precludes definitive conclusions such as go/no go decisions or dose selection for phase IIb or III studies, unless very large numbers of patients are evaluated in phase IIA pharmacodynamic s

Investigation on the association between Osteopontin and Apolipoprotein E gene polymorphisms and vancomycin-induced acute kidney injury: A pharmacokinetic/pharmacogenetic study in critically ill patients.

Vancomycin is a commonly administered antibiotic for various Gram-positive infections in critically ill patients. Vancomycin has a narrow therapeutic index and its main adverse drug reaction is acute kidney injury (AKI). In this regard, various pharmacokinetic parameters have been widely considered for therapeutic drug monitoring (TDM) purposes. Higher vancomycin trough concentration and area under the curve (AUC) values would be associated with higher rates of AKI. Therefore, dose adjustment based on targeted pharmacokinetic values would be essential to avoid toxicity and achieve optimal clin

Survey to inform personalised prescribing in a British South Asian community: pharmacogenomics and traditional medicine use.

BACKGROUND: Pharmacogenomics (PGx) uses genetic information to personalize medication, reducing adverse reactions and improving efficacy. Despite its promise, low public awareness and disparities in PGx acceptability among under-represented groups may exacerbate health inequalities. The objective of this study was to elucidate a British South Asian community's attitudes toward personalised prescribing. METHODS: Adults of Bangladeshi or Pakistani ancestry from the Genes & Health (G&H) study completed a survey. Community feedback guided theme prioritization. Multivariable logistic regression an

Decitabine Reprograms Temozolomide-Resistant Glioblastoma Through Epigenetic Reactivation and Mesenchymal Attenuation: A Multi-Omics Study.

Background/Objectives: Glioblastoma (GBM) is the most lethal primary brain malignancy in adults, with a median overall survival of approximately 15 months. Temozolomide (TMZ) resistance develops in virtually all patients, and no second-line regimen has improved outcomes over the past two decades. The DNA methyltransferase inhibitor decitabine (DAC) has attracted interest as a chemosensitizer, but whether it directly reverses the TMZ-resistance transcriptome or operates through distinct, complementary mechanisms has not been tested at multi-omics resolution. Methods: We performed an integrative

Estimating the Frequency of False-Negative Pharmacogenetic Test Results by Self-Reported Ancestry.

Concerns about the applicability of pharmacogenetic (PGx) testing across diverse ancestry groups have risen from the underrepresentation of non-European populations in PGx research. Current PGx panels may fail to detect relevant variants in non-European populations, increasing the likelihood of false-negative results. To investigate this, we assessed reference allele (*1) and genotype (*1/*1) frequencies by self-reported ancestry in a cohort of 1086 youth aged 6-24 years who underwent PGx testing. Testing included 10 pharmacogenes (CYP2B6, CYP2C19, CYP2C9, CYP2D6, CYP3A4, CYP3A5, NUDT15, SLCO1

Toward pharmacogenetic SLCO1B1-guided dosing of methotrexate in arthritis using a murine Slco1b2 knockout model.

Low-dose methotrexate (MTX) is a first-line therapy for the treatment of arthritis. However, there is considerable interindividual variability in MTX exposure following standard dosing. Polymorphisms in SLCO1B1 significantly effect MTX clearance, altering therapeutic response. One decreased function variant, rs4149056 (c.521T>C, Val174Ala), slows MTX clearance and in vitro uptake of MTX. This phenotype was recapitulated in a mouse model using a knockout (KO) of the murine orthologue, Slco1b2. Our objective was to investigate the impact of this phenotype on the pharmacokinetics and therapeutic

Pharmacogenetic interactions in amyotrophic lateral sclerosis: a step closer to a cure?

Genetic mutations related to amyotrophic lateral sclerosis (ALS) act through distinct pathophysiological pathways, which may lead to varying treatment responses. Here we assess the genetic interaction between C9orf72, UNC13A, and MOBP with creatine and valproic acid treatment in two clinical trials. Genotypic data was available for 309 of the 338 participants (91.4%). The UNC13A genotype affected mortality (p = 0.012), whereas C9orf72 repeat-expansion carriers exhibited a faster rate of decline in overall (p = 0.051) and bulbar functioning (p = 0.005). A dose-response pharmacogenetic interacti

Systematic review of pharmacogenomics and adverse drug reactions in paediatric oncology patients.

Many paediatric patients with cancer experience significant chemotherapy side effects. Predisposition to drug reactions is governed by single nucleotide polymorphisms (SNPs). We performed a systematic review of the literature from 2006 through 2016. Outcomes of interest included patient characteristics, cancer type drug of interest, genes investigated, toxicity identified and genetic polymorphisms implicated. The primary toxicities studied were neurotoxicity cardiotoxicity, osteonecrosis, and thromboembolism and hypersensitivity reactions. The retrieved studies were grouped according to toxici

CYP2C19 Genotyping to Guide Clopidogrel Prescribing for Neurovascular Indications: A Survey of Institutional Approaches.

Dual antiplatelet therapy (DAPT) with aspirin and clopidogrel is commonly used for secondary prevention following acute ischemic stroke (AIS) and neurointerventional procedures. However, the efficacy of clopidogrel is reduced in carriers of CYP2C19 no-function alleles. While CYP2C19-guided DAPT is well-established for some cardiovascular indications, its use in neurovascular settings has remained limited until recently. This study characterized current implementation strategies for CYP2C19-guided DAPT for neurovascular indications across institutions in the Pharmacogenomics Global Research Net
