Safety of Compounded Medications.

Compounded medications are medications that are prepared by compounding pharmacies to meet individual patients' needs that cannot be met by FDA-approved brand name or generic medications. This may be the case when a patient needs a different dose or route of administration or if a patient is allergic to a certain ingredient in an FDA-approved medication. While compounded medications help to fill a necessary gap, they can have significant safety issues. Due to lesser regulatory oversight, patient safety may be compromised due to contamination or inaccurate potency of compounded medications. Com

Opioid-Associated Emergencies: A Review of Their Management and the Utility of Naloxone.

Although pain is most effectively treated through a multimodal approach, opioids remain a mainstay of treatment for chronic pain despite their considerable adverse effect profile and associated risks. Through modulation of the μ-opioid receptors, opioids can cause respiratory depression, which may result in death if not treated. When used in conjunction with other sedative substances, the risk of respiratory depression is potentiated. If an opioid emergency is suspected, responders should activate the emergency response system as outlined by the American Heart Association. Prompt and appropria

Pharmacist-led HIV and hepatitis C point-of-care testing and risk mitigation counseling in individuals experiencing homelessness.

INTRODUCTION: Over half a million people experience homelessness on any given night in the United States. As a result of increased exposure to disease, violence, stigma, substance misuse and limited accessibility to health services, individuals experiencing homelessness are disproportionately affected by communicable diseases such as HIV and HCV with prevalence estimates as high as 21% and 36%, respectively. Pharmacists, being some of the most trusted and accessible healthcare providers, are in a unique position to have a significant impact on the nationwide initiatives in Ending the HIV Epide

Clinical Update on Patient-Controlled Analgesia for Acute Postoperative Pain.

Patient-controlled analgesia (PCA) is an effective method for controlling acute pain, including postoperative pain in adults and in children from five years of age, pain resulting from labor, trauma, or other medical situations, or chronic and malignant pain. The treatment consists of a mini-computer-controlled infusion pump permitting the administration of on-demand, continuous, or combined doses of analgesic (mainly opioid) variations in response to therapy, which allows pain to be significantly controlled. Intravenous (IV)-PCA minimizes individual pharmacodynamics and pharmacokinetic differ

Preventing medication errors in anesthesia and critical care (abbreviated version).

Drug medication errors remain a major safety issue in anaesthesia and intensive care, and prevention measures need to be strengthened. This is why the French Society of Anaesthesia and Intensive Care and the French Society of Clinical Pharmacy have profoundly reviewed their previous recommendations published in 2007. The 2017 recommendations are based on the literature but also on feedback from field professionals targeting patient safety. They share many similarities with recommendations issued from other countries (European countries, North America and Australia in particular) on this subjec

Emergency contraception update: a Canadian perspective.

Barriers to hormonal emergency contraceptive (EC) access in Canada and the United States led professional and lay groups to lobby for levonorgestrel (LNG) (PLAN B, Barr Pharmaceuticals, Pomona, New York) to be made available over-the-counter. In December 2000, British Columbia, Canada, granted EC prescriptive authority to pharmacists, followed by Quebec in December 2001 and Saskatchewan in September 2003. In April 2005, Health Canada placed LNG on non-prescription, behind-the-pharmacy-counter status with no age restriction. After much controversy, in August 2006, the Food and Drug Administrati

Successful Retreatment of Chronic Hepatitis C Virus Genotype 1A Infection With Glecaprevir/Pibrentasvir in a Pediatric Patient With Developmental Disorder.

BACKGROUND/AIM: Although direct-acting antiviral (DAA) therapy is now available for pediatric patients with chronic hepatitis C virus (HCV) infection, the challenges associated with its use are similar to those observed in adults. In pediatric patients with chronic hepatitis C virus genotype 1A infection, the granular formulation of glecaprevir/pibrentasvir (GLE/PIB) can make oral administration difficult. CASE REPORT: We report a case of infection in a child with developmental disorder who achieved sustained virologic response after retreatment with a 12-week course of GLE/PIB, following ini

Optimizing Administration and Timing of Post Intubation Analgesia and Sedation in the Emergency Department.

INTRODUCTION: Recent literature suggests pain management and sedation in ED patients after rapid sequence intubation are done inconsistently, which impacts patient outcomes negatively. The purpose of this study is to compare rates and timing of post-intubation analgesia and sedation before and after an ED pharmacy practice improvement intervention. METHODS: We conducted a retrospective study of adult ED patients intubated over an 18-month period. The primary study endpoint was the frequency of post-intubation analgesia and sedation administration before and after implementation of the post-in

Lessons Learned From Helene: The Role of a Rural Community Hospital in Disaster Response After a Major Hurricane.

Hurricane Helene struck Western North Carolina in September 2024, leaving an entire region without power, clean water, or internet connectivity for more than a week. A small, rural, community hospital and its emergency department, which normally sees 84 patients per day, suddenly became the sole source of medical care for an estimated 200,000 residents and experienced a 70% surge in patient volume. Although structural damage to the hospital was limited, the complete loss of digital connectivity rendered electronic medical records, laboratory information systems, radiology, pharmacy, and routin

Successful administration of mitotane (O, p'-DDD) in pediatric oncology.

INTRODUCTION: Mitotane (o, p'-DDD) is a molecule that was developed many years ago for adrenal cortical carcinoma, but no suitable pediatric dosage form is available for administration to young children. Mitotane requires therapeutic drug monitoring because of its long half-life and difficulty in stabilizing plasma concentrations. Furthermore, Mitotane is a highly lipophilic drug that requires concurrent lipid administration. CASE REPORT: We present the case of a 3-year-old girl who was diagnosed with metastatic adrenal cortical carcinoma. Due to the difficulty in administering the tablets an
